We started new plans and initiatives of occupational health research and practice following the Cancun Congress. With renewed insights, we proceeded with interlinked outcomes as reported at the Midterm Meeting held this year. Reflecting this progress, we are preparing for the 2015 Seoul Congress with full involvement of our Scientific Committees and National/Area Secretaries. We hope to learn from the broad-ranging advances in this triennial get-together. The Chairs and Secretaries of the Scientific Committees I met during and after the Midterm Meeting as well as National Secretaries joining regional conferences I attended were all enthusiastic about promoting our share of work for the health of all workers. I could confirm substantial gains in promoting proactive research and ethical principles and expanding effective and basic occupational health services. In collaborating with our international allies, these three-year plans and outcomes really have positive impacts. Roles of international collaboration The main emphasis of our ICOH work is placed on fostering the knowledge-base and development of occupational health through this international collaboration, as stated in our Constitution. Advances are visible in the outcomes of the Scientific Committees conferences and our statements and guidance materials. A good example of our leadership role was noted at the Symposium on the Knowledge Network in Occupational Health, jointly organized by ICOH and ISSA (International Social Security Association) at the 20 th World Congress on Safety and Health at Work, held last August in Frankfurt by the ILO and ISSA. Chairing the symposium, Jorma Rantanen reported that the ongoing networks in occupational health, in most of which ICOH was involved, were proactively contributing to meeting new challenges and filling gaps in supporting all workers. Such bridging roles were found to accelerate preventive actions for emerging risks and needsoriented services for workplaces in difficult situations. We strongly support these catalytic roles of the knowledge networks. It is encouraging that the outcomes of our activities contribute to developing workplace-level actions in diverse settings. The ICOH Work Organization and Psychosocial Factors 2014 Congress, held in Adelaide last September, was a prominent occasion to provide guidance about workplace interventions for managing psychosocial risks and promoting health and well-being of workers in diverse work situations. The required framework is relevant to interventions to prevent multifaceted work-related risks and therefore its effectiveness needs to be assessed as part of daily occupational health practice in diversifying working situations. As this debate showed, attention is drawn to the need to develop competence for practical interventions workable in local situations. The Helsinki Declaration and follow-up Another prominent example was seen at the International Conference on Monitoring and Surveillance of Asbestos-related Diseases 2014, held in Helsinki with the collaboration of the ICOH Working Group on the Implementation of the Global Strategy of Occupational Health with a special reference to global ban of asbestos. The conference discussed the updating of the Helsinki criteria for detection, diagnosis and attribution of asbestos-related diseases. The Helsinki Declaration on Management and Elimination of Asbestos- Related Diseases (ARDs) adopted by the Conference thus confirms that primary prevention is the only effective way to eliminate ARDs. It calls for policies, regulations and practices for the prevention of risks and for effective protection of workers in removal and waste handling of asbestos from existing structure. The Declaration emphasizes the collaboration of all scientific and professional communities as guided by WHO, ILO and ICOH. All this is in line with the ICOH 2013 Statement: Global Asbestos Ban and the Elimination of Asbestos-Related Diseases. The guidance of the Declaration is essential for preventing the epidemic of ARDs from being repeated among workers and communities in the developing world. Joint efforts should be made at all levels of prevention, including regulatory and practical measures for the protection of workers and the implementation of the ban. As emphasized by the ICOH Statement, we need to continue to combine our effort based on professional competences needed for supporting comprehensive national efforts to eliminate ARDs. This need for joint follow-up of the Helsinki Declaration and the ICOH Statement should be incorporated in our plans of action in this triennium and beyond. While we are making steady progress in this area through our three-year cycles, we need to develop comprehensive preventive measures that can cover all workplaces. Guided by Bernardino Ramazzini A special event: Bernardino Ramazzini, Three-centenary from Death was held last October at the Aula Magna of the Padova University in Italy, co-organized by ICOH and the Italian Society of Occupational Medicine and Industrial Hygiene. This year marks the three-centenary from death of Bernardino Ramazzini ( ) who was a professor of practical medicine at Padova University since He is the author of the monumental work Morbis Artificum Diatriba (Diseases of Workers), a milestone for prevention of occupational diseases for over three centuries. It outlines the health hazards of chemicals, repetitive or violent motions, odd postures and other work-related risks encountered by workers in 52 occupations. He proposes that physicians extend the list of questions they ask their patients by adding What is your occupation? His work translated into many languages and his cultural legacy serve today as a leading practical guide for occupational health practice. We realize the three-centenary development in our profession has evolved with a continuous spectrum of practice reflecting strategic thinking. This is closely related to the ethical principles of our conduct. In our cyclic collaborative effort, we need to continue to establish a common framework of action learning from this continuous spectrum. Within the scientific programme of the 2015 Seoul Congress, we are organizing a Global Forum on Future of Occupational Health. We hope to provide answers to the key questions about our strategies meeting challenges of decent working conditions conducive to occupational health and work ability and how the research and expert organizations provide support for these efforts. The Forum is expected to elucidate the future directions of our network activities in research knowledge, training and services based on our traditions and renewed insights. I look forward to seeing you all at the 2015 ICOH Congress in Seoul. 3 From the Editor Content of this Issue Occupational health professionals may face emergency care or treatment at workplace. There will be a protocol to deal with this condition in each country. However, there is no information at an international level on this matter. Dr. Alexis Descatha, ICOH National Secretary of France, has delivered on-line surveys to 51 ICOH National Secretaries and members of the Working Group for Emergency Care. The survey result is reported and will be used for the further development of international cooperation on emergency care. It is important to diagnose occupational diseases not only for compensation but also for prevention. The first step for diagnosis is to set the criteria. Each country has its own diagnostic criteria on occupational diseases. However, the stage of development is different from country to country according to economic development or other factors. Ten countries in South East Asia will unite to be ASEAN Community for Economic, Security and Cultural by the year They need to develop the diagnostic criteria for ASEAN countries because workers will travel freely in the region when they are united. Representatives of ten countries met in Singapore on 7-9 July 2014 and agreed with development of occupational disease diagnostic criteria in ASEAN countries. Dr. Adul Bandhukul, ICOH National Secretary of Thailand, reports the result of the meeting and they will also organize a special session on Development of Diagnostic Criteria in ASEAN countries during ICOH 2015 Congress. The Joint Congress of the African Regional Association of Occupational Health (ARAOH) and the South African Society of Occupational Medicine (SASOM) was held in Johannesburg, South Africa, 1-3 August 2014 under the support of the ICOH Working Group on Occupational Infectious Agents (WGOIA). Most of the 150 congress delegates and presenters were from South Africa and other African countries, but also from other continents. Claudina Nogueira, an ICOH Board member, reports on the conference. The 21 st Asian Conference on Occupational Health (ACOH) was held by hosting University of Occupational and Environmental Health (UOEH), Japan in conjunction with 24 th Japan-China-Korea Joint Conference in Fukuoka, Japan, 2-4 September, ACOH was established in 1954 in Tokyo and the next meeting will be held in Gaoshiung, Taiwan in During the Conference, National Secretary s meeting was organized and discussed issues on regional occupational health. Suvi Lehtinen, Vice-President of ICOH, summarized the meeting results. Election of new officers and board members This issue contains the candidates for ICOH officers and board members who will serve from 2015 to The President and Secretary General will be automatically elected because there are no competing candidates. Two Vice Presidents will be elected among 3 candidates. Board members will also be selected by voting because only 16 out of the 25 candidates can be elected. The ballot paper will be sent to active members by postal mail from ICOH Secretariat office in Italy. Only a vote using the ballot paper sent by post mail is valid. The result will be announced on Wednesday during the ICOH Congress 2015 in Seoul, Korea. The voting rate is usually low. It was approximately 30% in the last election. Active participation of members is necessary to elect the most eligible officers and board members. Please do not forget to send your vote using the ballot paper to the ICOH Secretariat on time. Ready for ICOH 2015 Congress The preparation of ICOH 2015 Congress in Seoul is going smoothly. As of November 2014, 114 special sessions are being proposed and 65 oral sessions are fixed. There are 1,135 abstracts from 920 applicants for oral or poster presentation. There will be approximately 2,000 abstracts including ones submitted for special sessions and parallel conferences. December

4 The Organizing Committee has finished the invitation process for 10 plenary speakers, and 33 semi-plenary speakers,whose list is now available on the website. The plenary is allocated according to the daily themes: Day 1 - Decent Workplace: Social and Psychological Environment; Day 2 - Equality and Safety Culture; Day 3 - Management of the Global Burden of Occupational Risk; Day 4 - Blind Spot and Newly Found Risk; and Day 5 - Revival of Old Problems. The semi-plenary sessions have also been allocated in each day according to the daily themes. A new type of semi-plenary will take place as a Policy Forum on Tuesday morning (day 2), which will give participants the global perspectives of occupational health and safety through representatives from international organizations and high government officials from each continent. Directors Forum is held on Tuesday and Thursday afternoon, which will be delivered by directors of national or public institutes of occupational health and safety. There are two parallel international conferences during the congress of 30 th APOSHO(Asian Pacific Occupational Safety and Health Organization) annual conference and 25 th KCJ (Korea-China-Japan) Joint Conference, where participants of ICOH Congress as well as the parallel conferences can participate and enjoy all sessions of the conferences. It will definitely broaden participants view for the global trend on occupational health and safety. Deadline for early bird registration has been extended to December 15, 2014 for the convenience of possible participants. The organizer set a remarkably affordable fee to participants compared with that of the previous Congresses. Thanks to the Korean government, those from developing countries and young participants under age of 36 can enjoy the privilege of the half price for registration even from the reasonable fee. According to the special promotion, the organizer hopes to bring more participants from developing countries to achieve the motto of the Congress, Global Harmony for Occupational Health; Bridge the world. 5 Results from an Online Survey on Emergency Management in the Workplace Many occupational practitioners have to face emergencies in the occupational setting, from life-threatening emergencies to urgent care, especially when they work in large facilities far from urban zone and regular emergency medical system. Furthermore, workplace emergencies tend to have singularities that typical emergency teams are not familiar with (e.g. disaster medicine, first response, etc.). However, neither networks nor federations on emergency care, management, or response in the workplace exist at an international level. Indeed, the knowledge of emergency health care at workplace relied upon only scarce nationwide guidelines on specific aspects (cardiac arrest, first aid implementation) in some countries. In the context of a proposal of a new working group on this topic after the symposium (organized in 2012 in the previous International Commission on Occupational Health (ICOH) Congress), we decided to send a short online-survey focused on emergency care to each country represented by the National Secretaries of our ICOH. An online survey was sent via to the 51 National Secretaries (one per country) and to the 7 members of the ECOH working group, between September 2013 and January 2014, which included 12 questions covering their country/continent and their emergency procedures, equipment, and teams available. After acceptance by the ICOH Board, an was sent with two reminders. We obtained 35 reports from as many countries with 28 being from the National Secretaries, 6 being from the working group, and the data of the remaining country being reported by another member of ICOH. The main results of multiple choice questions are shown in Table 1. Differences between equipment, teams, training and procedures were found, depending on the regulations, emergency response systems, size of the companies, occupational hazards, and different types of persons involved (Figure 1). Improvement of emergency management was considered as a top or high priority in 45.7% of answers (Figure 2). The opinions of the reported priorities are presented in Figure 3. Despite limitations based upon the sampling methodology that we used, this simple survey gave us a representative picture of the heterogeneity around the world in the management of workplace emergencies, and the importance of proposing a new Scientific Committee on this major topic and improving networking of ICOH members interested in this field. 8 New Method for Disability Evaluation Currently, it is difficult to identify whether interventions to improve the work functioning of workers with health problems are effective. Researchers regularly evaluate the effectiveness of interventions on single aspects of work functioning. However, work-functioning is a multi-facet concept. Interventions may have conflicting effects on various domains of work functioning; high productivity may for instance require high personal effort. When addressing and measuring the work functioning of health-impaired workers, researchers and practitioners should not only record the central indicators of the concept work functioning, but also take into account the relative importance of its different indicators. This insight, resulting from a 2-year study conducted by the Coronel Institute of Occupational Health, a department of the Academic Medical Center of the University of Amsterdam, led to a new and improved method for the measurement of the work functioning of health-impaired workers. The measurement method captures the central indicators of the concept work functioning, namely quality and quantity of work as well as recovery from work and capacity to meet physical and psychological work demands, and takes into account, via a weighting procedure, the relative importance of these different indicators. A subsequent validation study showed that the measurement method has good convergent, fair divergent, and good discriminative, construct validity. The Netherlands Organization for Health Research and Development funded the research. The measurement method has value for researchers and practitioners. By recording and monitoring the work functioning of health-impaired workers, health professionals and organizations can offer timely support to those health-impaired workers who struggle to meet their work demands. Further, the newly developed measurement method enables researchers to test whether their interventions actually help health-impaired workers in functioning at work. 9 Meetings of National / Area Secretaries National / Area Secretaries in Latin American region The Latin American regional meeting of National/Area Secretaries was held on 3 October 2013 in Buenos Aires, Argentina, in connection with the 13th ALSO Congress. A total of 9 National Secretaries attended, together with ICOH Vice President, Past President and one ICOH Board member. The wide participation of NSs from the Region and other experts was made possible by an exceptional arrangement of financial support by the Local Organizers of the Congress, and that was gratefully acknowledged. training of occupational health personnel in the countries. National / Area Secretaries in Asian region National / Area Secretaries in Africa ARAOH, the African Regional Association on Occupational Health, organized an international Conference on 1~3 August 2014 in Johannesburg, South Africa. The African Regional Meeting of ICOH National Secretaries was arranged in connection with the Conference. Four ICOH NSs were present: South Africa, Nigeria, Senegal and Zimbabwe. In addition, a colleague from Benin attended the meeting in order to learn about ICOH activities. Ms. Claudina Nogueira, an ICOH Board member, with her colleagues facilitated a connection through Skype so that Suvi Lehtinen could participate in the meeting from Finland, and Dr. Kazutaka Kogi, ICOH President, was present on site in Johannesburg. The reports and feedback from the participating countries were encouraging, showing good emphasis on the ACOH 2014 was organized by the Japan Society for Occupational Health. The Asian regional meeting of ICOH National/Area Secretaries was arranged in connection with this 21st ACOH Conference. Discussions were also held at both the Dinner and Lunch events of the Conference. Three ICOH National/Area Secretaries from Japan, Taiwan and Thailand were present. The topical issues of ICOH and the countries represented in the meeting were discussed. Thailand is actively developing occupational health and safety by establishing a national OH&S institute and organizing medical examinations for migrant workers. Japanese colleagues have been active in the development of activities in several ICOH Scientific Committees. In addition, ICOH members in Japan convened for an annual meeting in connection with the Japan Society for Occupational Health meetings. In Taiwan, a great deal of development has focused on the diagnosis and reporting system of occupational diseases. Brief reports by SL December

10 ICOH and WGOIA representation at the Joint Congress of ARAOH-SASOM The Joint Congress of the African Regional Association of Occupational Health (ARAOH) and the South African Society of Occupational Medicine (SASOM) was held in Johannesburg, South Africa, 1-3 August Most of the 150 congress delegates and presenters were from South Africa and other African countries (e.g. Senegal, Nigeria, Zimbabwe, Benin, Uganda, Cameroon, Botswana, Namibia) and there were also participants from the USA, Italy, Hungary and Japan. The ICOH Working Group on Occupational Infectious Agents (WGOIA) contributed to the scientific programme of the congress by organising an ICOH track, and inviting ICOH affiliated occupational health professionals to make presentations: (1) Keynote address of the ICOH track The keynote address of the ICOH track was given by the President of ICOH, Dr. Kazutaka Kogi (Japan), who graciously accepted the invitation to present on a topic very relevant to occupational health in Africa. Promoting good occupational health practice in smallscale workplaces: experiences from international networking focussed on the primary prevention of work-related risks in small-scale workplaces using participatory steps which build on local good practice and make use of locally adapted tools. (2) Presentation on health care workers and biohazards In June 2014, WGOIA member Prof. Stefano Porru (Italy) finalised the development of a referenced, evidence-based guidance document on clinical best practice for health care workers, titled Risk assessment, health surveillance and fitness for work in health care workers exposed to infectious agents, with input and endorsement from the members of the WGOIA. The guidance document was officially launched at the ARAOH / SASOM Congress in the form of a complimentary copy handed out to congress participants, following the presentation on the document, given by Prof. Porru. His presentation titled Health care workers and biohazards: scientific evidence and occupational health practice, detailed the objectives of the guidance document, which are to cover occupational health practices and provide evidence-based recommendations for health care workers in terms of risk assessment, health surveillance, immunisation and fitness for work. The guidance document will be offered to the WGOIA members for adaptation and use by their respective national occupational medicine associations, and will be shared with the ICOH National Secretaries, for adaptation and use in their respective countries. Additionally, the guidance document has been made available in electronic format in the public domain, for all interested parties to access: org/library/default.aspx?searchkeyword=fitness%20 for%20work (3) Presentation on the ICOH WGOIA - Mandate and Outcomes: Prof Mary Ross (WGOIA Chair) and Ms Claudina Nogueira (WGOIA Secretary), both from South Africa, presented on the mandates, tasks and deliverables of the WGOIA during the current triennium, with special attention paid to products / outputs developed for the focus areas of the WGOIA, viz.: health care workers; occupational travel medicine; HIV and tuberculosis; and priority groups. To conclude the triennium, the WGOIA will contribute six sessions to the ICOH Congress 2015 in Seoul, Korea, in line with its mandate and collaborations. 10 ICOH Newsletter Vol.12 No. 2,3

11 (4) Meeting of the ICOH National Secretaries To take full advantage of the presence of congress participants from a number of African countries, a meeting of the ICOH National Secretaries was held during the congress. The meeting was chaired by Dr. Kogi, and Ms. Suvi Lehtinen (ICOH Vice President) participated via Skype. The following secretaries attended the congress and the National Secretaries meeting: Drs Adriaan Combrinck (South Africa); Okon Akiba (Nigeria); Dingani Moyo (Zimbabwe); and Cheikh Al Khalifa Aboubacar Cisse (Senegal). 12 ASEAN Diagnostic Criteria on Occupational Diseases the agreement in the first meeting between representatives of each country on who were the top persons in policy on occupational health, the format of the diseases criteria, the most common occupational diseases occurring in each country and the occupational diseases that each country would cover and send back to each country for correction and acceptance. Ten ASEAN countries were in Bankok on 8 July 2014: Brunei Darussalam, Cambodia, Indonesia, Laos, Malaysia, Myanmar, Philippines, Singapore, Thailand and Vietnam committed to implementing the ASEAN Diagnostic Criteria on Occupational Diseases. The importance of these criteria is that these ASEAN countries will unite as one ASEAN community by economic, security and sociocultural in 2015, their workers will work freely in each country and must have the same occupational health standard. These criteria will help the workers to have the same occupational diseases diagnosis in ASEAN countries and will have the same compensation and prevention. The statistics of occupational diseases will be registered with accuracy. The result of the meeting on the 8 th of July was The first group of ASEAN Diagnostic Criteria on Occupational Disease that will be done were occupational hearing loss, occupational dermatitis, heat stress, asthma and silicosis. The meeting had also set up the International committee for ASEAN Diagnostic Criteria on Occupational Diseases. The meeting was sponsored by Nopparat Rajathanee Hospital, department of medical service Ministry of Public Health and the association of occupational and environmental disease of Thailand, while China and Japan participated as observers in the meeting. Thailand had prepared this meeting since August 2013 with Prof. Tar Ching Aw and Prof. Jun De Wang being the consultants of this program. 15 ICOH Elections Tenure Announcement of the candidates for the election of ICOH Officers and members of the Board for Triennium The candidates must be eligible under Article 5, section 2 and 8 of the ICOH Constitution. The Officers and the members of the Board shall be elected as set forth in the Bye-Laws for the triennial period separating sessions of the General Assembly. They may be re-elected to the same office for no more than one additional term, with exception of the Secretary-General, who may be elected for more than one additional term. The newly elected Officers take office at the General Assembly convened immediately after the ICOH Congress A member of the ICOH can be a candidate for one post only at a time. Out of these names, the Secretary-General will prepare the ballot form, with the names of the candidates for each office in random order. The Secretariat will send the ballot papers and envelop by mail to all ICOH members in good-standing according to Bye-Law 6, section 3, at the beginning of the new year.
