Les formules ont évolué depuis l alimentation normale, les formules enrichies et maintenant celles élaborées pour répondre à des pathologies spécifiques. Cette évolution ainsi que l amélioration des protocoles d administration sont étroitement liées aux qualités de leurs conditionnements. / The development of clinical nutrition (parenteral enteral) is quite recent despite its obvious contribution to the efficacy of the medical and surgical therapies. The formulas have evolved from the normal foods, to enriched formulas and now to formulas targeted for specific pathologies. The formula evolution and the improvements in the administration techniques are closely related to the characteristics of their packaging. Les principaux critères pour les conditionnements en nutrition parentérale et entérale sont : protection (stérilité et stabilité) absence d interaction entre les nutriments et avec le contenant (migration) facilité et sécurité d administration y compris lors de l hospitalisation à domicile. / For parenteral as well as enteral nutrition the key criteria for packaging are: protection (keep sterility and stability) avoid interactions between components and with container (migration) facilitate administration including home care under safe conditions. Braun, Germany Cette présentation débutera par une analyse des besoins des patients et des indications typiques de nutrition spéciale dans un contexte hospitalier. Les différences entre nutrition parentérale et entérale seront discutées. En nutrition parentérale, les différentes possibilités de nutrition standard et sur mesure seront abordées, ainsi que les avantages et inconvénients des formulations entérales en poudre et liquides. Enfin, les besoins de conditionnement pour les différents usages seront soulignés. / The presentation will start with an analysis of patient needs and typical indications where special nutritional care is needed in the hospital. The differences between parenteral and enteral nutrition are discussed. Further the different possibilities of standard nutrition and patient individual nutrition will be discussed for the parenteral nutrition. Also the pros and cons of powder vs liquid formulations for the enteral nutrition will be shown. Finally the packaging demands for the different purposes will be highlighted. Thilo Brinkmann studied biology and philosophy in Giessen and Bochum in Germany followed by a Doctoral Thesis at the Max-Planck-Institute for Molecular Biology. He was Responsible for Bioethics and Biotechnology at the German Association of Research based Pharmaceutical Companies (VFA) for two years and joined B.Braun Melsungen AG as Assistant in Since 2005 Thilo Brinkmann is responsible for the OEM Business of B. Global Pharmaceutical Packaging Development, Tekni-Plex, Belgium Tekni-Plex Europe N.V. a développé une gamme de mélanges à base de polyoléfines et une technologie d extrusion spécifique pour films souples autoclavables totalement transparents pour applications pharmaceutiques et médicales comme, par exemple l IV ; les poches de dialyse ou de nutrition entérale. Tekni-Plex positionne ces films comme étant d aspect supérieur aux films actuels en PP. La transparence est aussi bonne que les polyamides de grande qualité et le coût de ces nouveaux films est très attractif. L on estime qu un film utilisé pour le 3

6 << Conferences conditionnement de fluides médicaux doit être très transparent pour permettre une inspection visuelle du contenu. Cette exigence de transparence est très importante car les fluides médicaux pour application IV doivent être totalement limpides et exempts de particules ; une inspection visuelle permet de déterminer si une contamination ou précipitation non désirée a eu lieu. Elle permet également de vérifier la turbidité de la solution. La souplesse du film est importante pour l écoulement des poches IV, pour que les patients reçoivent la quantité exacte. Dans l idéal, la transparence et la souplesse du film ne doivent ni être affectées par une stérilisation à la vapeur à la température habituelle de 121 C, ni par l humidité ou la pression. Le thermoscellage, la résistance à la ponction, la bonne tenue à la stérilisation (par autoclave ou toute autre méthode) et les propriétés barrières sont d autres exigences importantes pour tout film employé dans la fabrication de poches. Constituer une barrière à l oxygène est particulièrement important pour les poches remplies de solutions nutritives. Les poches IV sont actuellement fabriquées à partir de films PVC, qui ne sont pas exempts d additifs à bas poids moléculaire, comme les plastifiants, qui peuvent migrer dans le liquide médical contenu dans la poche. Aussi, le PVC n a pas de bonnes propriétés barrières, limitant ainsi l utilisation pour des solutions à ph élevé (à ph > 6.8, une précipitation peut se produire). Selon l application, on recommande de remplacer le PVC par des matériaux inertes comme les polyoléfines. Ces nouveaux films se rétractent peu jusqu à 140 C en MD et TD, peuvent être stérilisés à la vapeur à 121 C, résistent à la ponction et à la déchirure, sont thermoscellables et présentent d excellentes propriétés barrières par rapport à l humidité et/ou l oxygène. Tekni-Plex Europe N.V. has developed a range of polyolefin based compounds and a specific extrusion technology to produce cost effective glass clear autoclavable flexible sheet film for pharmaceutical and medical applications such as for instance IV; dialysis solution bags or Nutritional bags for enteral feeding. Tekni-Plex positions the films as superior in terms of appearance over current PP based sheet film. The clarity is as good as high-end polyamides in the skin layer, hence making the new films cost attractive. It is generally accepted that a film used for the containment of medical fluids needs to present highly transparent in order to allow visual inspection of their content. The transparency requirement is very important as medical fluids for intravenous application need to be absolutely clear and particle free and visual inspection will allow to determine whether or not contamination or undesired precipitation has taken place. It also allows to check for the turbidity of the solution. The flexibility of the film is important for the drainage of IV bags, so that patients receive exact solution quantities as intended. It is clear that ideally the transparency and flexibility of the film are not affected by steam sterilization at the commonly used temperature of 121 C, nor by humidity or pressure. Other important requirements for a film used for manufacturing bags are heat sealability, puncture resistance, the ability to withstand sterilization (by autoclave or other method) and barrier properties. Barrier to oxygen is particularly important for bags containing Nutritional solutions. Currently medical IV bags are typically made from films based on PVC. However, such PVC films are not free of low molecular weight additives, such as plasticizers, which can migrate from the IV bag into the medical fluid contained therein. Also PVC does not have high barrier properties and limits the use for high ph solution bags (at ph > 6.8 precipitation may occur). Therefore it is recommended for several applications 4 to replace PVC by inert materials such as polyolefin materials. The new films shows little shrink, up to 140 C in MD and TD, are suitable for steam sterilization at 121 C, are puncture and tear resistant, heat sealable and have high moisture and/or oxygen barrier properties. After finishing his education in Electro Mechanical Engineering and Material Sciences, Michiel van den Berg started his professional career in the European Packaging Development team of Procter & Gamble in In 1995, he left the world of fast moving consumer goods for a role pharmaceutical packaging with Eli Lilly and Company as a Packaging Engineer and later as manager of the Global Packaging Technology and Development group. Annick Tibi, a hospital-based practitioner at the General Equipment and Health Products Agency, AP-HP, is in charge of the Institutional Clinical Trials Pharmaceutical Management Unit. She is also assistant professor at Paris Descartes University, Faculty of Pharmacy, in Health safety of health products and clinical research and an active member of several AFSSAPS working groups. Annick Tibi has done considerable work and made numerous publications in the fields of biomedical research and the quality of drugs and medical devices. Cependant, suivant les phases cliniques et leur voie d administration, leur présentation et design peuvent être différents. Les particularités du conditionnement des médicaments pour essais cliniques seront abordées au travers d exemples. Ce conditionnement doit être réalisé en collaboration avec les départements cliniques, afin de sécuriser l administration au malade et l observance du patient. / Just like medicines on the market, medicines for clinical trials are manufactured by pharmaceutical companies in accordance with good practices. However, depending on the clinical phases and their mode of administration, the presentation and design may be different. The specificities of clinical trial drug packaging will be addressed by means of examples. This packaging must be designed in conjunction with clinical departments in order to make patient administration safe and ensure compliance. La traçabilité est présente tout au long de cette chaine depuis la fabrication du PA jusqu à l administration au malade. / The distribution of drugs for clinical trials, often at controlled temperature, throughout the world, calls for organised, validated logistics. Traceability is present throughout this chain, from manufacturing to patient administration. Marylène Delaunay, Docteur en Pharmacie Docteur de l Université Paris XI (Chatenay Malabry) has 20 years experience in drug development, first with the Laboratoires Cassenne (Rousel-Uclaf subsidiary), where she began her career in pharmacology followed by quality assurance, and then Hoescht Marion Roussel, Aventis Pharma and Sanofi- Aventis, where she was in charge of the packaging and distribution of drugs for clinical trials. Dominique Deldycke, Pharmacist, graduated from René Descartes University (Paris V), has 30 years experience in drug development with Rhône Poulenc followed by Aventis and Sanofi-Aventis, more specifically in the fields of formulation, manufacturing and packaging of products for clinical trials. Me3.3 La production de médicaments avec des équipements de process à usage unique - Application aux lots cliniques / Drug production with single-use process equipment - Clinical batches application Jean-Pascal ZAMBAUX, Disposable-Lab, France Le marché demande aujourd hui plus qu hier une réactivité de leur CMO pour la production de lots cliniques. Le développement des produits d origine biologique nécessite des procédés de fabrication pointus ou la contamination croisée doit être maitrisée et de fait des procédures de nettoyage entre chaque lot qui deviennent longues et coûteuses. Disposable-Lab va présenter ici sa nouvelle unité de Bordeaux basée sur le concept modulaire et des équipements à usage unique pour la production. Pour illustrer ceci nous allons vous présenter un cas concret de réalisation d un nouveau spray nasal avec préparation du device et mise en production des lots cliniques en comparant notre concept à une unité classique tant sur le plan technique que sur le plan gain de temps et aussi sur le plan économique. Whereas, in the past Insulin predominantly was filled in a vial in order to be withdrawn and injected by a disposable syringe, pen systems to administer Insulin to Diabetes patients have become especially in Europe the gold standard. This presentation will describe the dosage form evolution from the cumbersome manual application using a vial and disposable syringe to today s cartridge based pen systems. Within this presentation, the increasing chemical and functional requirements of the individual container closure components (from glass vial, rubber stopper, disposable syringe and disposable syringe plunger to glass cartridge, cartridge plunger and lined seal) will be presented and discussed. In addition, the benefits of the pen based application to the patient and the efficacy of the therapy will be highlighted. Toutes les seringues sécurisées ne sont pas équivalentes. Les seringues pré-remplies qui comportent des dispositifs de sécurité d origine deviennent la norme dans tous les pays développés. BD a développé un nouveau système de seringue sécurisée à pré-remplir avec des caractéristiques bien spécifiques qui assurent le bon fonctionnement primaire d une seringue (injecter la dose complète de médicament) tout en réduisant le nombre de piqûres accidentelles. And all safety syringes are not equivalent. Prefilled syringes including safety engineered features to prevent accidental needle stick injuries are becoming the standard of the industry in all developped countries. BD has developed a new safety prefillable syringe system including very specific features that enables to maintain consistently the primary function of a syringe (injecting the complete dose of drug) while contributing to the reduction of accidental needlestick injuries. This improved syringe system was then evaluated comparatively to similar products already marketed to assess end-users' preference. The results obtained both in term of functionality and preference are highlighting the value of the system approach from the design to the manufacturing of the drug delivery system.
