2 Intended Use The SeroCT - IgG kit is intended for the detection of IgG antibodies specific to C. trachomatis in human serum. The Savyon SeroCT - IgG kit is a new generation qualitative ELISA test which is based on Chlamydia trachomatis specific synthetic peptides. SeroCT is used as an aid in the diagnosis of C. trachomatis specific infection. SeroCT - IgG is intended to be run and interpreted in conjunction with the Savyon SeroCT - IgA kit. For In Vitro Diagnostic Use. Introduction Chlamydia is a gram negative obligate intracellular bacteria that causes acute and chronic disease in mammalian and avian species. The genus Chlamydia is comprised of four species: C. trachomatis, C. pneumoniae,c. psittaci and C. pecorum (1-4). C. trachomatis is divided into 15 serovars (5-8). Serovars A, B, Ba and C are agents of trachoma(9), the leading cause of preventable blindness, endemic in third world countries. Serovars L 1 -L 3 are the agents of lymphogranuloma venereum. Serovars D-K are the common cause of sexually transmitted genital infection worldwide: cervicitis, endometritis/salpingitis(10) in females and urethritis(11) in both males and females. Endometritis/salpingitis can lead to tubal occlusion with a higher risk of extrauterine pregnancy and infertility. Genital infection may cause an acute and persistent infection occasionally without any clinical signs. Generally, these infections are treatable once they are diagnosed. However, without any treatment the infection may progress to a severe chronic inflammation leading to infertility, ectopic pregnancy, induced abortion or child delivery. Moreover, infants to infected mothers can be infected during birth leading to conjunctivitis or pneumonia (12-14). The serology of C. trachomatis is more interesting in cases of chronic infections than in acute infections. C. pneumoniae is an important respiratory pathogen in humans and causes up to 10% of community-acquired pneumonia cases. It has been associated with acute respiratory diseases, pneumonia, asthma, bronchitis, pharyngitis, acute chest syndrome of sickle cell disease, coronary heart disease and Gullain-Barré syndrome (15-17). C. psittaci infects a diverse range of host species from molluscs to birds to mammals and also causes severe pneumonia. In animals, C. psittaci and C. pecorum are capable of inducing diverse disease syndromes, like pneumonia, enteritis, polyserositis, encephalitis and conjunctivitis. Serological testing, now an established approach in many countries, has been shown to provide a comprehensive answer for the detection of C. trachomatis infection. In suspected deep-seated infections, serum sampling reduces the necessity for invasive procedures which are required for direct antigen detection. In cases of lower urogenital infections, collection limitations such as effectiveness of scrape sampling procedure, E

3 specimen handling and transportation difficulties have to be weighted. Above all, there remains the issue that most Chlamydial infections are asymptomatic. Therefore, an infection may persist for a long time, ascend the upper genital tract causing deep and chronic infection, and increase the probability of false negative results in direct antigen detection. Serological testing for C. trachomatis, through the detection of various specific antibodies, is today an effective and highly accepted method option(10,11,18,19). New and accurate technologies apply the immuno markers IgM, IgA and IgG to characterize the presence and stage of the infection. Specific IgM is indicative of acute Chlamydial infection. Absence does not, however, preclude the presence of on-going infection, especially in recurrent and chronic cases. The use of specific IgA as a marker for active Chlamydial infection has been shown to have an important role because of its short half life time, while persisting as long as antigenic stimulation exists. IgA, however, is more suitable for post therapy follow-up. IgG is a marker for Chlamydial positive immune-response in either current, chronic or past infections. Serological cross reactions occur between the three different species of Chlamydia. Most of the serological diagnostic assays for Chlamydia use either purified elementary bodies: microimmunofluorecence (MIF) and ELISA tests, lipopolysaccharide (LPS), or purified major outer membrane protein, (MOMP) as antigens. Genus specific epitopes are present in all the above antigens, therefore, low species specificity is observed. Moreover, a large proportion of the population has been exposed to C. pneumoniae (with no clinical signs), the prevalence of anti-chlamydia antibodies is very high. Therefore, the differentiation between C. pneumoniae and C. trachomatis specific antibodies using conventional serological screening tests (MIF, ELISA, EIA etc.) is insufficient. Savyon Diagnostics has developed an assay in which C. trachomatis species specific epitopes, derived from different serotypes, are used in an Enzyme - Linked Immunosorbent Assay (ELISA). The test excludes cross-species reactive epitopes and enables more accurate and more specific determination of C. trachomatis IgG and IgA antibodies. Principle of the Test SeroCT plates are coated with C. trachomatis specific peptides. Serum to be tested is diluted and incubated with the precoated SeroCT plate 1h at 37 C. In this step C. trachomatis specific antibodies are bound to the immobilized C. trachomatis specific peptides. Non specific antibodies are removed by washing. Anti-human IgG conjugated to horseradish peroxidase (HRP) is added and incubated 1h at 37 C. In this step the HRP-conjugate is bound to the prebound antigen-antibody complex

4 Unbound conjugate is removed by washing. E Upon the addition of TMB-substrate, the substrate is hydrolyzed by the peroxidase, yielding a blue solution of the reduced substrate. Upon the addition of the stop solution, the blue color turns yellow and should be read by an ELISA reader at a wavelength of 450nm. The absorbance is proportional to the amount of the specific antibodies which are bound to the immobilized peptides. Warning and Precautions For In Vitro Diagnostic Use 1. This kit contains human sera which have been tested by FDA and CE approved techniques, and found to be negative for HBV antigen and for HCV and HIV 1&2 antibodies. Since no known method can offer complete assurance that products derived from human blood do not transmit infection, all human blood components supplied in this kit must be handled as potentially infectious serum or blood, E

7 according to the recommendations published in the CDC/NIH manual "Biosafety in Micro Biological and Biomedical Laboratories", TMB-Substrate solution is an irritant material to skin and mucous membranes. Avoid direct contact. 3. Diluted Sulfuric acid (1M) is an irritant agent for the eyes and skin. In case of contact with eyes, immediatley flush area with water and consult a physician. Do not pour water into this product. In case of an accident or discomfort consult a physician (if possible present the label). 4. All the components of this kit have been calibrated and tested by lot. It is not recommended to mix components from different lots since it might affect the results. Storage and Shelf-Life of Reagents 1. All the reagents supplied should be stored at 2-8 C. The unopened reagent vials are stable until the expiration date indicated on the kit pack. Exposure of originally stoppered or sealed components to ambient temperature for a few hours will not cause damage to the reagents. DO NOT FREEZE! 2. Once the kit is opened, it s shelf life is 90 days. 3. Unused strips must be resealed in the aluminum pouch with the desiccant card, by rolling the open end and sealing tightly with tape over the entire length of the opening. 4. Crystals may form in the 20x concentrated Wash Buffer during cold storage, this is perfectly normal. Redissolve the crystals by warming the buffer to 37 C before diluting. Once diluted, the solution may be stored at 2-8 C for up to 21 days. Serum Collection Prepare sera from aseptically collected samples using standard techniques. Heat inactivated sera should not be used. The use of lipemic, turbidor contaminated sera is not recommended. Particulate material and precipitates in sera may cause erroneous results. Such specimens should be clarified by centrifugation or filtration prior to the test. Storage: Specimens should be stored at 2-8 C and tested within 7 days (adding of 0.1% Sodium Azide is highly recommended). If a longer storage period is anticipated, aliquot and store the specimens below -20 C. Avoid repeated thawing and freezing. Test Procedure A. Preparation of Reagents 1. Bring all components and clinical specimens to be tested to room temperature. Mix well the Positive Control, Negative Control and the clinical specimens before use. 2. Determine the total number of specimens to be tested. In addition to the specimens, the following must be included in each test: two wells of Negative Control and one well of Positive Control

8 3. Withdraw the microtiter plate from its aluminum pouch by cutting one end near the seal. Leave the required number of strips (according to the number of specimens to be tested) in the 96 well frame. 4. Dilute the Concentrated Wash Buffer 1/20 with double-deionized or distilled water. For example, in order to prepare one liter of Wash Buffer, add 50ml of the Concentrated Wash Buffer to 950ml of double-deionized or distilled water. B. Incubation of sera samples and controls 5. Dilute each patient serum 1/21 with the supplied Serum Diluent as follows: Add 10µl of patient serum to 200µl of Serum Diluent. 6. Pipette 50µl of Positive Control, Negative Control and 1/21 diluted sera into separate wells of the test strip. The Negative Control should be pipetted into two separate wells. 7. Cover the strips with a plate cover and incubate for 1h at 37 C in a moisture chamber. 8. Discard the liquid content of the wells. 9. Washing step: Fill each well with wash buffer ( µl) up to the end of the well and discard the liquid, repeat this step two times, for a total of three washing steps. 10. Dry the strips and frame by gently tapping them over clean absorbent paper. C. Incubation with Conjugate 11. Concentrated HRP-Conjugate anti-human IgG should be diluted to working solution shortly before use. Dilute the concentrated HRP-conjugated 1/300 with Conjugate Diluent. For example, for two strips prepare a minimum of 3 ml of diluted HRP-Conjugate- (10µl of Concentrated HRP-conjugate is mixed with 3ml of Conjugate Diluent). 12. Pipette 50µl of diluted conjugate into each well. 13. Cover the strips with a plate cover and incubate for 1h at 37 C in a moisture chamber. 14. Discard the liquid content and wash as described in steps D. Incubation with TMB - Substrate 15. Pipette 100µl TMB-Substrate into each well, cover the strips with a plate cover and incubate at room temperature for 15 minutes. 16. Stop the reaction by adding 100µl of Stop Solution (1M H 2 SO 4 ) to each well. E. Determination of Results 17. Determine the absorbance at 450nm and record the results. Determination should not exceed 30 minutes following stopping of chromogenic reaction E

9 Note: Any air bubbles should be removed before reading. The bottom of the ELISA plate should be carefully wiped. Test Validation For the test to be valid the following criteria must be met. If these criteria are not met the test should be considered invalid and should be repeated. 1. Positive Control: The absorbance value should be 0.8 at 450nm. 2. Negative Control: The average absorbance value of the Negative Control performed in duplicate should be 0.1< NC 0.4 at 450nm. Calculation of Cut-Off Value (COV) and Cut-Off Index (COI) The cut-off value is calculated according to the following formula: COV = NC x 2 NC = The average absorbance at 450nm of the Negative Control run in duplicate. The serum samples used in each study were defined by MIF as either negative for both C. trachomatis and C. pneumoniae antibodies (MIF Ct-/Cp-) or as negative for C. trachomatis, positive for C. pneumoniae (MIF Ct- /Cp+). 14 SeroCT - IgG Verwendungszweck Der SeroCT -IgG Kit dient zum Nachweis von IgG Antikörpern spezifisch für C. trachomatis im Humanserum. Der SeroCT - IgG Kit gehört zu einer neuen Generation qualitativer ELISA Tests, der auf Chlamydia trachomatis spezifischen, synthetischen Peptiden basiert. Der SeroCT - IgG Kit ist zur Durchführung und Interpretation zusammen mit dem Savyon SeroCT - IgA Kit vorgesehen. Nur zur in vitro Diagnostik Einführung Chlamydia sind gram-negative, obligat intrazelluläre Bakterien, die akute und chronische Krankheiten in Säugern und Vögeln verursachen. Die Gattung Chlamydia beinhaltet vier Spezies: C. trachomatis, C. pneumoniae, C. psittaci und C. pecorum (1-4). C. trachomatis ist in 15 Serovare unterteilt (5-8). Die Serovare A, B, Ba und C verursachen Trachoma (9), eine unbehandelt zur Erblindung führende Infektion, besonders häufig in tropischen und subtropischen Regionen mit niedrigem Hygienestandard. Die Serovare L1-L3 sind Erreger des Lymphogranuloma venereum (LGV, Lymphopathia venerea). Die Serovare D-K sind weltweit häufig die Ursache von sexuell übertragenen Genitalinfektionen: Zervizitis, Endometritis/Salpingitis (10) bei Frauen und Urethritis (11) bei Männern als auch bei Frauen. Die Genitalinfektion kann als akute und bisweilen chronische Infektion ohne klinische Symptomatik verlaufen. Im allgemeinen sind diese Infektionen, wenn diagnostiziert, kurabel. Ohne Behandlung können sie sich zu schweren chronischen Entzündungen entwickeln, die zu Unfruchtbarkeit durch Tubenverschluss, ektopisher Schwangerschaft, Aborten oder Frühgeburten führen. Des weiteren können Neugeborene von infizierten Müttern während der Geburt infiziert werden, was zu Konjunktivitis und Pneumonie führen kann (12-14). Die Serologie von C. trachomatis ist bei chronischen Infektionen von höherem Interesse als bei akuten Entzündungen. C. pneumoniae ist ein wichtiges Pathogen der Atemwege bei Menschen und verursacht bis zu 10% der Fälle von Lungenentzündung. Es wurde akuten Krankheiten der Atemwege zugeordnet, wie Pneumonie, Asthma, Bronchitis, Pharingitis, akutes Brustsyndrom der Sichelzellkrankheit, Koronarherzleiden und Guillain-Barré Syndrom (15-17). C. psittaci infiziert eine Reihe verschiedenartiger Spezies von Weichtieren, Vögeln und Säugetieren und verursacht u.a.. schwere Pneumonien in Tieren. C. psittaci und C. pecorum können verschiedene Krankheitsbilder wie Lungenentzündung, Polyserositis, Enzephalitis und Konjunktivitis hervorrufen G

15 Infektionen des Menschen kommen vor, sind aber offenbar nicht häufig. Serologische Tests auf Chlamydia trachomatis AK sind heute eine akzeptierte Methode in vielen Ländern und haben gezeigt, daß sie für den Nachweis von Chlamydia trachomatis - Infektionen wertvolle Informationen liefern. Beim Verdacht auf tiefsitzende Infektionen vermindert die Serumentnahme die Notwendigkeit invasiver Prozeduren, die für den direkten Antigen-Nachweis erforderlich sind. In Fällen von Urogenitalinfektionen sind die Unsicherheit der Abstrichprozedur, sowie die Schwierigkeiten beim Handhaben und dem Transport der Proben in Betracht zu ziehen. Es ist seit langem bekannt, daß viele Chlamydia - Infektionen asymptomatisch verlaufen können. Eine Infektion kann daher lange Zeit bestehen, in den oberen Genitaltrakt aufsteigen und schwerwiegende, evtl. chronische Entzündungen verursachen und die Wahrscheinlichkeit falsch negativer Resultate beim direkten Antigen-Nachweis erhöhen. Serologische Tests zum Nachweis verschiedener spezifischer Antikörper sind eine effektive und häufig angewandte Methode zur Feststellung von C. trachomatis- Infektionen (10,11,18,19), Neue Technologien nutzen die Immunomarker IgM, IgA und IgG, um Bestehen und Stadium einer solchen Infektion zu charakterisieren. Spezifisches IgM ist ein Hinweis auf akute Chlamydia-Infektionen. Sein Fehlen schliesst jedoch - besonders in rezidivierenden und chronischen Fällen - eine Infektion nicht aus. Der Einsatz von spezifischem IgA als Marker für aktive Chlamydia-Infektionen erwies sich als wichtig wegen der kurzen Halbwertzeit, wohingegen es wohl erhalten bleibt, solange eine ausreichende antigene Stimulation vorhanden ist. IgA ist darüber hinaus wichtig als posttherapeutische Kontrolle. IgG ist ein Marker für Chlamydia-positive Immunantworten bei akuten, chronischen oder früheren Infektionen. Serologische Kreuzreaktionen zwischen den drei verschiedenen Spezies von Chlamydia treten auf. Die meisten serologischen Tests auf Chlamydia-AK benützen entweder gereinigte (MIF) Elementarteilchen, Lipopolysaccharide (LPS), oder gereinigtes Protein der Aussenmembran (MOMP) als Antigene. Genus-spezifische Epitope sind in all diesen Antigenen vorhanden, geringe Spezies- Spezifität ist daher die Folge. Des weiteren ist ein Grossteil der Bevölkerung mit C. pneumoniae (ohne klinisches Erscheinungsbild) durchseucht und das Vorkommen von anti-chlamydia Antikörpern ist sehr häufig. Daher ist die Differenzierung zwischen C. pneumoniae und C. trachomatis-spezifischen Antikörpern mit Hilfe der üblichen serologischen Screeningverfahren (MIF, ELISA, EIA usw.) noch mangelhaft. Savyon Diagnostics hat einen Test entwickelt, in welchem C. trachomatis Speziesspezifische Epitope, aus verschiedenen Serotypen stammend, in einem Enzyme Linked Immunosorbent Assay (ELISA) verwendet werden. Der Test schliesst Speziesüberkreuzende, reaktive Epitope aus und ermöglicht so die spezifische Bestimmung von C. trachomatis IgA und IgG-Antikörpern

16 Grundprinzip des Tests SeroCT platten sind mit C. trachomatis spezifischen Peptiden beschichtet. Das zu untersuchende Serum wird verdünnt und in der vorbeschichteten SeroCT Platte bei 37 C inkubiert. In diesem Durchgang werden C. trachomatis-spezifische Antikörper an die immobilisierten C. trachomatis-spezifischen Peptide gebunden. Nicht spezifische Antikörper werden durch waschen entfernt. Anti-Human IgG konjugiert mit Meerrettich Peroxidase MRP (Horseradish P. HRP ) wird zugesetzt und 1 Stunde bei 37 C inkubiert. In diesem Durchgang wird das MRP-Konjugat an den vorher gebildeten Antigen/Antikörper-Komplex gebunden. Nicht gebundenes Konjugat wird durch waschen entfernt. Nach zufügen des TMB Substrats wird das Substrat durch die Peroxidase hydrolysiert, was eine blaue Farbe des reduzierten Substrats ergibt. Nach zufügen der Stoplösung erfolgt ein Farbumschlag nach gelb. Mit einem ELISA Photometer wird bei einer Wellenlänge von 450nm die Absorption gemessen. Die Absorption ist proportional zur Menge der spezifischen Antikörper, die an die immobilisierten Peptide gebunden sind. Daher müssen alle in diesem Testkit enthaltenen Reagenzien menschlichen Ursprungs nach den Empfehlungen, veröffentlicht in der CDC/NIH Anleitung Biosafety in Micro Biological and Biomedical Laboratories 1988", grundsätzlich als potenziell infektiöses Serum oder Blut angesehen und entsprechend behandelt werden. 2. TMB Substratlösung ist ein Stoff, der Haut und Schleimhäute reizt. Direkter Kontakt ist zu vermeiden. 3. Verdünnte Schwefelsäure (1M) ist ein Reizmittel für Augen und Haut. Im Fall eines Kontaktes mit den Augen, spülen Sie sofort mit viel Wasser, und konsultieren Sie einen Arzt. Gießen Sie kein Wasser in das Produkt. 4. Alle Bestandteile dieses Kits wurden in der Lieferserie kalibriert und getestet. Es wird empfohlen, keine Bestandteile aus verschiedenen Chargen zu vermischen, da es die Ergebnisse beeinträchtigen könnte. Lagerung und Lagerfrist der Reagenzien 1. Alle gelieferten Reagenzien sollten bei 2-8 C gelagert werden.ungeöffnete Reagenzfläschchen sind bis zu der auf der Verpackung angegebenen Verfallszeit stabil. Reagenzien die noch original verschlossen sind und die für einige Stunden der Aussentemperatur ausgesetzt waren, werden keinen Schaden erleiden. NICHT EINFRIEREN! 2. Einmal geöffnet, hat der Kit eine Lagerfrist von 90 Tagen. 3. Nicht verwendete Streifen müssen in der Alutasche mit der Exsikkatorkarte erneut versiegelt werden, indem das offene Ende gefaltet und dicht mit Klebeband über die ganze Länge der Öffnung verschlossen wird. 4. Kristalle können sich in dem 20x konzentrierten Waschpuffer während der Kühllagerung bilden. Das ist nicht ungewöhnlich. Lösen Sie die Kristalle durch erwärmen des Puffers auf 37 C vor dem Verdünnen auf. Wenn bereits verdünnt, kann die Lösung bei 2-8 C bis zu 21 Tage gelagert werden. Gewinnung der Seren Der vorliegende Test wurde zur Untersuchung Serum konzipiert, Hyperlipämische, hämolysische, kontaminierte oder hitzeinaktivierte Seren können zu falschen Ergebnissen führen, Solches Material sollte mit diesem Test möglichst nicht untersucht werden. Die Gewinnung der Seren sollte fachgerecht erfolgen. Lagerung Serumproben sollten bei einer Temperatur von 2-8 C gelagert und innerhalb von höchstens 7 Tagen untersucht werden. Für längere Lagerung sollte 0.1% Natriumazid zugefügt werden und die Proben sollten bei einer Temperatur unter -20 C eingefroren werden. Grössere Volumina sollten in praktikable Aliquots geteilt werden. Häufigeres einfrieren und auftauen sollte vermieden werden. Nach auftauen der Proben sollten diese gut gemischt werden. Testablauf A. Vorbereitung der Reagenzien 1. Alle Bestandteile und klinischen Proben, die zu prüfen sind, müssen auf

20 Zimmertemperatur gebracht werden. Die Positiv Kontrolle, Negativ Kontrolle und die klinischen Proben vor Verwendung gründlich mischen. 2. Die Gesamtzahl der zu testenden Proben feststellen. Ausser den zu testenden Proben, müssen in jedem Test zwei Vertiefungen für die Negativ Kontrolle und eine Vertiefung für die Positiv Kontrolle vorgesehen werden. 3. Entnehmen Sie die Mikrotiterplatte aus ihrer Alutasche indem Sie ein Ende nahe dem Verschluss abschneiden. Fixieren Sie die benötigte Anzahl von Streifen (gemäss der Zahl der zu testenden Proben) im Rahmen. 4. Verdünnen Sie das Waschpufferkonzentrat 1/20 mit doppelt entionisiertem Wasser oder aqua dest. Zum Beispiel, um 1 Liter Waschpuffer zuzubereiten, fügen sie 50 ml des Waschpufferkonzentrats zu 950ml von doppelt entionisiertem Wasser oder aqua dest. B. Inkubation der Serumproben und Kontrollen 5. Verdünnen Sie jedes Patientenserum 1/21 mit der mitgelieferten Serumverdünnung wie folgt: 10µl Patientenserum zu 200µl Serumverdünnung hinzufügen µl der Positiv Kontrolle, Negativ Kontrolle und der 1/21 verdünnten Seren in separate Vertiefungen des Teststreifens pipettieren. Die Negativ Kontrolle soll in zwei separate Vertiefungen pipettiert werden. In den Vertiefungen sollten keine Luftblasen sein! 7. Plattendeckel auflegen und für eine Stunde bei 37 C in einer feuchten Kammer inkubieren. 8. Flüssigkeitsinhalt aus den Vertiefungen entfernen. 9. Waschschritt: jede Vertiefung bis unter den Rand ( µl) mit Waschpuffer füllen und dann Flüssigkeit verwerfen. Wiederholen sie diesen Schritt noch zwei weitere Mal, also insgesamt sollte der Waschschritt dreimal erfolgen. 10. Die Streifen im Rahmen durch leichtes abklopfen auf sauberem Absorbierpapier trocknen. C. Inkubation mit Konjugat 11. Konzentriertes MRP-konjugiertes Anti-Human IgG soll kurz vor der Verwendung zur einer Arbeitslösung verdünnt werden. Verdünnen Sie das konzentrierte MRPkonjugierte Anti-Human IgG 1/300 mit Konjugatverdünnung. Zum Beispiel: Für zwei Streifen bereiten Sie 3ml verdünntes MRP-konjugiertes Anti-human IgG (10µl des konzentrierten MRP-konjugierten Anti-Human IgG werden mit 3ml Konjugatverdünnung gemischt) µl des verdünnten Konjugats in jede Vertiefung pipettieren. Luftblasen vermeiden! 13. Plattendeckel auflegen und für eine Stunde bei 37 C in einer feuchten Kammer inkubieren G

21 14. Flüssigkeitsinhalt entfernen und waschen, wie unter Ziffer 9 beschrieben. 15. Die Streifen im Rahmen durch leichtes abklopfen auf sauberem Absorbierpapier trocknen. D. Inkubation mit TMB-Substrat Anmerkung: Das TMB-Substrat ist in gebrauchsfertiger Form µl TMB-Substrat in jede Vertiefung pipettieren, Plattendeckel auflegen und 15 Minuten bei Raumtemperatur inkubieren. 17. Reaktion durch zufügen von 100µl Stoplösung in (1µ H 2 SO 4 ) jede Vertiefung unterbrechen. E. Bewertung der Ergebnisse 18. Absorption bei 450nm messen und Ergebnisse aufzeichnen. Die Messung sollte innerhalb von 30 Minuten nach stoppen der chromogenen Reaktion erfolgen. Anmerkung: Vor dem Ablesen sind Luftblasen aus den Vertiefungen zu entfernen. Der Boden der Reaktionsstreifen sollte vorsichtig von Kondenswasser und eventuellen Verunreinigungen befreit werden. Testbewertung Für die Gültigkeit des Tests müssen folgende Kriterien erfüllt sein. Falls diese nicht erfüllt sind, ist der Test als ungültig anzusehen und sollte wiederholt werden. 1. Positiv Kontrolle: Der Absorptionswert soll 0.8 bei 450nm. 2. Negativ Kontrolle: Der durchschnittliche Absorptionswert der Negativ Kontrolle, zweifach durchgeführt, soll 0.1< NC 0.4 bei 450nm sein. Berechnung des Cut-Off-Wertes (COV) und des Cut-Off-Index (COI) Der Cut-Off-Wert wird laut folgender Formel berechnet: COV = NC x 2 NC = die durchscnittliche Absorption der zwei Negativ Kontrollen bei 450nm. 23 Testeinschränkungen 1. Kein einzelner serologischer Test sollte für eine endgültige Diagnose herangezogen werden. Alle klinischen und Laborbefunde sollten in Betracht gezogen werden. 2. Proben, die zu früh während einer Erstinfektion entnommen wurden, enthalten möglicherweise keine nachweisbaren Antikörper. Falls Verdacht auf eine Chlamydia-Infektion besteht, sollte eine weitere Probe Tage später entnommen und parallel mit der Originalprobe getestet werden. 26 SeroCT - IgG F Utilisation SeroCT - IgG permet de détecter les anticorps spécifiques anti-chlamydia trachomatis de type IgG dans le sérum humain. Le coffret SeroCT - IgG est une technique ELISA de nouvelle génération, utilisant des peptides de synthèse spécifiques de C. trachomatis. Le coffret SeroCT - IgG peut être utilisé avec le coffret SeroCT - IgA de Savyon pour une meilleure interprétation de la sérologie. Usage Diagnostique IN VITRO Introduction Les chlamydiae sont des bactéries intracellulaires essentielles à gram négatif qui provoquent des affections aiguës et chroniques chez les mammifères et les oiseaux. Le genre Chlamydiae se compose de quatre espèces: C. trachomatis, C. pneumoniae, C. psittaci et C. pecorum (1-4). C. trachomatis se divise en 15 serovars (5-8). Les serovars A, B, Ba et C sont les agents infectieux à l'origine du trachome (9), la première cause de cécité évitable dans le tiers monde. Les serovars L 1 -L 3 sont les agents infectieux à l'origine de la lymphogranulomatose vénérienne. Les serovars D-K sont couramment en cause dans les infections génitales sexuellement transmissibles dans le monde entier, infections telles que cervicite, endométrite, salpingite (10) chez les femmes, et urétrite (11) chez les hommes et les femmes. L'endométrite ou la salpingite peuvent mener à l'oblitération des trompes, entraînant une élévation du risque de grossesse extra utérine et de stérilité. Les infections génitales peuvent aussi apparaître sous forme d'infections persistantes sans symptômes cliniques. En général, ces infections sont traitées une fois diagnostiquées. Lorsqu'aucun traitement n'intervient, elles peuvent évoluer vers une grave inflammation chronique et aboutir à la stérilité à une grossesse ectopique, à des avortements à répétition et à des naissances prématurées. De plus, les nourrissons nés d'une mère infectée peuvent être infectés à leur tour durant l'accouchement et contracter une conjonctivite ou une pneumonie (12-14). La sérologie de C. trachomatis est plus intéressante dans les infections chroniques que dans les infections aiguës. C. pneumoniae est un agent pathogène important chez l'être humain, à l'origine de 10% des pneumonies contractées en communauté. Il est associé à des affections respiratoires aiguës, à la pneumonie, à l'asthme, à la bronchite, la pharyngite, au syndrome pulmonaire aigu de la drépanocytose, aux maladies coronariennes et au syndrome de Gullain-Barre (15-17). C. psittaci affecte un large éventail d'espèces, des mollusques aux oiseaux et aux mammifères et provoque également de graves pneumonies. Chez les animaux, C

27 psittaci et C. pecorum peuvent induire divers syndromes tels que la pneumonie, l'entérite, la polysérite, l'encéphalite et la conjonctivite. Les analyses sérologiques ont abouti à une réponse fiable au problème posé par la détection des infections à C. trachomatis. Lorsque l'on soupçonne une infection déclarée, le prélèvement d'échantillon de serum limite le besoin de procéder à des techniques agressives, nécessaires au diagnostic direct des agents pathogènes. En cas d'infection de la partie inférieure des voies uro-génitales, il faut tenir compte des limites propres aux techniques de prélèvement, telles que l'efficacité du prélèvement par frottis et les difficultés de manipulation et de transport des échantillons. Le problème reste avant tout que la plupart des infections à Chlamydia sont asymptomatiques. Une telle infection peut donc durer longtemps, progresser le long des voies génitales supérieures et y causer une infection profonde et chronique et, de ce fait, augmenter la probabilité de résultats faussement négatifs au diagnostic direct des agents pathogènes. Le diagnostic sérologique de C.trachomatis par la détection de divers anticorps spécifiques représente désormais une technique efficace et souvent mise en œuvre (10, 11, 18, 19). Des technologies nouvelles et affinées utilisent les marqueurs de l'immunoglobuline IgM, IgA et IgG afin de déceler la présence de l'agent pathogène et la phase de l'infection. L'IgM spécifique indique une infection aiguë à Chlamydiae. Cependant, l'absence d'igm n'exclut pas la présence d'une infection continue, surtout dans les cas d'affection récurrente et chronique. On a pu prouver que l'iga spécifique est un marqueur indiquant une affection active à Chlamydiae, étant donné la brièveté de sa durée de vie et le fait que l'iga n'est présente que tant qu'elle est stimulée par un antigène. De plus, l'iga permet également un suivi post thérapeutique. Un taux d'igg élevé marque une réponse immunitaire positive provenant d'une infection à Chlamydiae en cours, chronique ou passée. Les réactions croisées sérologiques sont fréquentes entre les différentes espèces de Chlamydiae. La plupart des analyses sérologiques destinées au diagnostic des Chlamydiae utilisent soit des corps élémentaires purifiés (analyses par microimmunofluorescence (MIF) et analyses (ELISA), soit des lipo-polysaccharides (LPS) ou des protéines purifiées des membranes externes (MOMP) comme antigènes: puisque les épitopes spécifiques de genre sont présents dans tous les antigènes ci-dessus, la spécificité des espèces est généralement faible. De plus, puisqu'une forte proportion de la population a subi une exposition à C. pneumoniae sans toutefois présenter de symptômes cliniques, les anticorps spécifiques des Chlamydiae sont très largement répandus. La différenciation entre les anticorps spécifiques de C. pneumoniae et ceux spécifiques de C. trachomatis à l'aide de tests de dépistage sérologiques conventionnels (MIF, ELISA, EIA etc.) s'avère donc déficiente. Savyon Diagnostics Ltd. a mis au point un test permettant le dépistage spécifique de C. trachomatis à l'aide d'une méthode immunoenzymatique (ELISA), capable de distinguer entre les différents anticorps des infections aux Chlamydiae décrits. Ce test utilise comme antigène des peptides spécifiques de C. trachomatis, ce qui exclut les épitopes des réactions croisées entre les espèces et permet donc de déceler de façon plus précise et plus spécifique les anticorps IgG et des IgA des infections à C. trachomatis

28 Principe du test Des plaques de micro-titration SeroCT sont enduites de peptides spécifiques de C. trachomatis. Le sérum à tester est dilué puis mis à incuber à 37 C dans les plaques pré-enduites du test SeroCT. Lors de cette étape, les anticorps spécifiques de C. trachomatis de l'échantillon de sérum vont se fixer aux peptides spécifiques de C. trachomatis immobilisées. Les anticorps non spécifiques sont éliminés par lavage. On introduit une IgG anti-humaine conjuguée à de la peroxydase de raifort (HRP). Durant cette étape, le conjugué HRP va se fixer au complexe antigène-anticorps fixé auparavant. Le conjugué non fixé est éliminé par lavage. L'introduction de substrat de TMB provoque l'hydrolyse du substrat par la peroxydase, ce qui donne une solution bleue de substrat réduit. On introduit la solution d'arrêt, la couleur bleue vire au jaune, puis on la lit à l'aide d'un lecteur ELISA à une longueur d'onde de 450nm. L'absorbance est proportionnelle à la quantité d'anticorps spécifiques fixés par les peptides immobilisées.
