Please print on white paper. Note application forms cannot be printed double sided. 3 Passport phos: Color passport phos 2 wide x 2 high on white background taken within past 6 months. Please do not staple pictures the application form. Flight itinerary: Please submit a copy of your flight itinerary showing your name, entry/departure dates. Employment Letter: Please include your original employment letter with the application. Proof of funds be submitted show evidence of sufficient financial means that is less than 30 days old. A signed bank statement by an agent of the bank will suffice. Travel itinerary: Please submit a copy of your day by day travel itinerary with your application form. Hotel confirmation A certificate of lodging signed by the competent DRC authority at the locality or hotel reservation is required. Personal Invitation: Invitation letter signed by the host in DRC (family member or friend) and signed by the competent Congo DRC authority for a private stay is required, as well as a copy of the document that proofs of a genuine link the applicant. For non-congo hosts, please also provide proof of their legal status in Congo (DRC). Yellow Fever: A certificate for vaccination against yellow fever is obligary. Please submit your original yellow vaccination book with your application form. Other vaccines and anti-malaria treatment are recommended. Consult your primary physician for details. For non-canadian passport holders only: Proof of Legal Status in Canada is required. Please include a copy of your Permanent Resident Card, Work Permit or Student Visa. Please contact our office verify whether the original document or a phocopy is required.
