Has a list of skilled workers providing care at home to any disabled person (child, adolescent or adult without intellectual impairment). Transportation assistance to take children to medical appointments. Accommodation services Service d hébergement La Maison Caméléon de l Estrie 4761, Gaspé Street Sherbrooke (Québec) J1N 2C7 ( 819 repitcameleon@hotmail.com Short stay or weekend (Friday 17:30 - Sunday 16:30) and services during school holidays. This service is for anyone with a physical or intellectual impairment or with multiple impairments. The Chameleon House can accommodate up to 8 children or adults to sleep. 5 Programs and grants Equipment and supplies You can get a grant for special equipment and medical supplies (to cover, for example, the cost of diapers for children over age three). You must apply either through your Health and Social Services Center / Centre de soins et de services sociaux (CSSS), the Local Community Service Centre / Centre local de services communautaires, (CLSC) or your local employment office if you or your child are receiving Employment Insurance. The transport-accommodation program You can have your travel and accommodation expenses reimbursed if your child needs medical treatment or rehabilitation frequently or on an ongoing basis in a rehabilitation center or a hospital. Apply through your CSSS /CLSC. Living environment (at home) You can get services or a grant for keeping your child at home if he requires physical assistance (for example, bathing a child who becomes too heavy to lift). You should contact your CSSS/CLSC. Residential adaptation program and services You can get a grant to adapt your home (house or apartment) to make it accessible. You may need a ramp, wider bedroom doors or to rearrange the bathroom. Contact your municipality and discuss it with an occupational therapist. For more information about the Programme d adaptation de domicile (PAD) (the residential adaptation program), you can contact the Housing Corporation of Quebec. 6 Parking stickers for people with disabilities and adapted vehicles Form available at the Société d assurance du Québec (the Quebec automobile insurance board or SAAQ) ( (toll-free) Income Supplement for handicapped children The supplement for handicapped children is a financial assistance measure which is intended to help families assume responsibility for, and provide for the care and education of a child whose physical or mental health significantly limits his daily life activities for at least one year. Whatever their income, families with a disabled child can claim the Supplement for handicapped children, the amount of which is the same for everyone. Contact: Régie des rentes du Québec ( (sans frais) : Child Disability Benefit (CDB) This is a non-taxable benefit, paid by the Government of Canada, designed for families with low or moderate income who care for a child under age 18 with mental retardation or severe, prolonged physical impairment.
