1 c-kit pharmdx Code K tests for manual use English Intended use For In Vitro Diagnostic Use. The c-kit pharmdx assay is a qualitative immunohistochemical (IHC) kit system used for the identification of c-kit protein/cd117 antigen (c-kit protein) expression in normal and neoplastic formalin-fixed paraffinembedded tissues for histological evaluation. The c-kit pharmdx rabbit polyclonal antibodies specifically detect the c-kit protein in CD117 antigen-expressing cells. The c-kit pharmdx is indicated as an aid in the differential diagnosis of gastrointestinal stromal tumors (GIST). After diagnosis of GIST, results from c-kit pharmdx may be used as an aid in identifying those patients eligible for treatment with Gleevec /Glivec (imatinib mesylate). Results from hematoxylin and eosin (H&E) stains and a panel of antibodies can aid in the differential diagnosis of GIST. Interpretation must be made by a qualified pathologist, within the context of a patient s clinical history, proper controls, and other diagnostic tests. Note: This test is not intended as the sole basis for making the diagnosis of GIST and is not intended as the sole basis for selecting Gleevec/Glivec therapy. The outcome of c-kit negative GIST patients treated with Gleevec/Glivec has not been established. A negative result would not necessarily exclude the 1, 2, 3 diagnosis of GIST, nor should it preclude treatment with Gleevec/Glivec. All subjects in the Novartis Gleevec/Glivec clinical trials were selected using an investigational Novartis Clinical Trial Protocol (NCTP). The primary anti-c-kit rabbit polyclonal antibody reagent used in the NCTP was purchased from Dako. The c-kit pharmdx primary polyclonal antibody reagent underwent the same method of production, purification and quality control as did the NCTP polyclonal anti-c-kit reagent. Summary and explanation Introduction The proto-oncogene c-kit, otherwise known as CD117 antigen or Stem Cell Factor Receptor, is a 145 kd type III transmembrane tyrosine kinase receptor. The c-kit gene encodes a transmembrane tyrosine kinase receptor structurally similar to platelet derived growth factor receptors A and B as well as the colony stimulating factor 1 receptor and is thought to play an important role in hematopoiesis, spermatogenesis, and melanogenesis. The c-kit protein contains extracellular domains with 5 Ig-like loops, a highly hydrophobic transmembrane domain, and an intracellular domain with tyrosine kinase activity split by a kinase insert in an ATP-binding region and in a phosphotransferase domain. Receptor activation is accompanied by receptor dimerization, substrate phosphorylation and autophosphorylation, receptor internalization, activation of protein kinases and phospholipases, and transcription of different proto-oncogenes. 4 The c-kit tyrosine kinase receptor pathway has been shown to be important for tumor growth and progression in several cancers. 5 Mutations in the c-kit gene may lead to ligand independent phosphorylation (activation) of the c-kit tyrosine kinase, which are believed to have a central pathogenic role in e.g. gastrointestinal stromal tumors. 6 Gastrointestinal mesenchymal tumors have historically been difficult to differentiate and diagnose. In 2001, imatinib mesylate (Gleevec, Novartis, Basel, Switzerland) was approved by the US Food and Drug Administration for the treatment of gastrointestinal stromal tumors (GIST). Approval was based on a clinical study in which adults with GIST that expressed c-kit protein demonstrated by immunohistochemistry (Novartis Clinical Trial Protocol) were enrolled and treated with imatinib mesylate. 7 GIST cases are described as three morphologic categories: spindle cell, epithelioid and mixed types. Regardless of morphology, the majority of GIST s express c-kit protein in a significant proportion of the tumor cells It should be noted that a small percentage of GIST s do not express c-kit protein. Relatively few other tumors may be c-kit protein positive. These include metastatic melanoma, angiosarcoma, Ewing sarcoma, mastocytoma, seminoma, and pulmonary small cell carcinoma. GIST are usually also positive for heavy molecular-weight caldesmon, often positive for CD34 (60-80%), and generally negative for desmin and S100 protein. 12 Specificity Rabbit anti-c-kit polyclonal antibodies were obtained by subcutaneous injection of a 14 amino acid (aa) peptide (positions of the intracellular C terminus of the c-kit protein) coupled to thyroglobulin. The antiserum was specifically purified through antigen-bound, activated thiol AvidGel F affinity chromatography. A small number of soft tissue sarcomas have also been found to be c-kit protein positive. 6 Some of these specimens (e.g. leiomyosarcoma) were tested for c-kit expression. The study involved a comparison between the c-kit pharmdx assay and the Novartis Clinical Trial Protocol (NCTP) as used for the selection of patients for treatment with Gleevec in the clinical trials performed by Novartis. 2 specimens tested displayed positive staining. The result was consistent using the two protocols. All positive immunostaining was abolished after absorption of the primary antibody with a synthetic peptide (16 amino acid C-terminal end of the c-kit protein). Therefore the primary antibody used in the c-kit pharmdx assay specifically reacted with the c-kit protein in the two positively stained soft tissue sarcomas. In-house studies using c-kit pharmdx demonstrated c-kit protein expression in a variety of normal cells. These cells include ductal and myoepithelial cells of the breast, purkinje cell processes, lamina propria cells of the colon, tubular epithelium of the kidney, melanocytes and myoepithelial cells of the skin, interstitial Cells of Cajal of the small intestine, and mast cells. The cytoplasmic reactivity in granulocytes was caused by endogenous peroxidase activity and was visible with the Negative Control Reagent. Reagents Code K1906 is for Manual Staining. The materials listed are sufficient for 25 tests (25 patient slides and 10 control slides incubated with primary antibody reagent to c-kit protein and 25 slides incubated with the corresponding Negative Control Reagent). The number of tests is based on the use of the c-kit pharmdx Manual protocol with ready-to-use reagents. The kit provides materials sufficient for a maximum of 10 individual staining runs. Materials provided Quantity Description 1x4 ml c-kit pharmdx Polyclonal Rabbit IgG Polyclonal rabbit anti-human c-kit IgG in a Tris-HCl buffer, containing stabilizing protein and mol/l sodium azide. 1x4 ml Rabbit IgG Negative Control Reagent Polyclonal rabbit IgG at a greater than or equal to the positive anti-c-kit concentration in a Tris- HCl buffer, containing stabilizing protein and mol/l sodium azide. 2x5 slides c-kit pharmdx Control Slides Each slide contains sections of two pelleted, formalin-fixed, paraffin-embedded mouse (+) and human (-) cell lines, which represent both moderate and no expression levels of c-kit protein. The IHC staining scores of the cell pellets are 2+ and 0. Principle of procedure Materials required, but not supplied The c-kit pharmdx IHC kit contains polyclonal antibodies and control slides to complete an IHC staining procedure for formalin-fixed and paraffin-embedded specimens. Following incubation with the primary polyclonal antibodies to human c-kit protein, this kit is optimized for use with a ready-to-use visualization reagent based on dextran technology. This reagent consists of both secondary goat anti-rabbit antibody molecules and horseradish peroxidase molecules linked to a common dextran polymer backbone, thus eliminating the need for sequential application of link antibody and peroxidase conjugate. The enzymatic conversion of the subsequently added chromogen results in formation of a visible reaction product at the antigen site. The specimen may then be counterstained and coverslipped. Results are interpreted using a light microscope. Control slides containing two formalin-fixed paraffin-embedded mouse and human cell lines with staining intensity scores of 2+ and 0, respectively, are provided for quality control of the kit reagent performance. The c-kit pharmdx IHC protocol was validated using the following Dako staining reagents: Wash Buffer (code S3006) Dual Endogenous Enzyme Block (code S2003) Target Retrieval Solution (code S1699 or S1700) EnVision+ HRP, Anti-Rabbit (code K4002 or K4003) DAB+ (code K3467 or K3468) ( ) EFG_001 p. 2/40

3 Note: To ensure proper staining results, only these staining reagents should be used with c-kit pharmdx. Deviations from this recommended protocol have not been validated. Other materials required to perform c-kit pharmdx include the following: Hematoxylin, alcohol or water-based such as Dako s Hematoxylin (code S3301 or S3302) Coverslips Water bath, properly calibrated and capable of maintaining C or pressure cooker, properly calibr ated and capable of heating up to 125 C Distilled or deionized water (reagent-quality water) Drying oven, capable of maintaining C Ethanol, absolute and 95% Light microscope (4x 40x objective magnification) Mounting medium, such as Dako s Faramount (code S3025) or Dako s Glycergel (code C0563) or Dako s Ultramount (code S1964) Positive and negative tissues to use as process controls (see Quality control section) Slides, Fisher s SuperFrost Plus, poly-l-lysine-coated slides, charged slides, or Dako s Silanized Slides (code S3003) (see Specimen preparation section) Staining jars or baths Timer (capable of 2 30 minute intervals) Wash bottle Xylene, toluene, or xylene substitutes 1 ml pipette Humid Chamber Note: All reagents included or available separately such as Dako s Wash Buffer (code S3006) are formulated specifically for use with this test. In order for the test to perform as specified, no substitutions can be made. Precautions 1. For professional users. For In Vitro Diagnostic Use. 2. This product contains sodium azide (NaN 3), a chemical that is highly toxic in pure form. At product concentrations, though not classified as hazardous, build-ups of NaN 3 may react with lead and copper plumbing to form highly explosive metal azides. Upon disposal, flush with large volumes of water to prevent azide build-up in plumbing. 3. As with any product derived from biological sources, proper handling procedures should be used for reagents as well as specimens. 4. Incubation times, temperatures, or methods other than those specified may give erroneous results. 5. Do not substitute primary antibodies or negative control reagents with primary antibodies and negative control reagents of different manufactured lots (lot numbers appear on vial labels) or with reagents from other manufacturers. This may give erroneous results. 6. The kit reagents are optimally diluted for use with the recommended reagents. Further dilution is not recommended and may result in loss of antigen staining. The user must verify any such changes. 7. Wear appropriate Personal Protective Equipment to avoid contact with eyes and skin. 8. Unused solution should be disposed of according to local, State, and Federal regulations. 9. Safety Data Sheet available for professional users on request. Users must be carefully instructed in the proper work procedure, the dangerous properties of the product and the necessary safety instructions. (Per European Union Directive 94/33/EC). Please refer to Material Safety Data Sheet (MSDS) for additional information. (*DAB Chromogen is a material required but not included with this kit). Storage Store c-kit pharmdx kit at 2 8 C. Control slides must also be stored at 2 8 C. Do not use the kit after the expiration date printed on the outside of the kit box. There are no obvious signs to indicate instability of this product, therefore, positive and negative controls should be run simultaneously with patient specimens. If the reagents are stored under any conditions other than those specified in the package insert, they must be validated by the user. 13 Statement of quality The reagents of the c-kit pharmdx kit have been quality controlled by immunohistochemistry using the following conditions: target retrieval in a Pascal pressure cooker (code S2800) programmed for 30 seconds at 125 C, cooling to 90 C, followed by EnVision+ Rabbi t, HRP system. 4 Specimen preparation Biopsy specimens must be handled to preserve the tissue for IHC staining. Standard methods of tissue processing should be used for all specimens. 14 Paraffin-embedded sections Formalin-fixed and paraffin-embedded tissues are suitable for use. Alternative fixatives have not been validated and may give erroneous results. Specimens from the biopsy should be blocked into a thickness of 3 or 4 mm and fixed for the time period appropriate for the fixative. The tissues are then dehydrated and cleared in a series of alcohols and xylene, followed by infiltration by melted paraffin. The paraffin temperature should not exceed 60 C. Properly fixed and embedded tissue blocks expressing the c-kit protein will keep indefinitely 14, 15 prior to sectioning and slide mounting if stored in a cool place (15 25 C). Tissue specimens should be cut into sections of 3 5 µm. After sectioning, tissues should be mounted on Fisher s SuperFrost Plus, Dako s Silanized (code S3003), charged slides or poly-l-lysine coated slides and placed in drying racks. The slide racks should be pounded on an absorbent towel to remove water trapped under paraffin and on glass and then dried at room temperature for one hour. The rack of slides should then be placed in a C incubator for one hour. Any exc ess water remaining on slides after removal from the incubator should be removed by pounding slides on towels and drying for one additional hour in the incubator. After removal from the incubator, slides should be held at room temperature until cool and paraffin has hardened. To preserve antigenicity, tissue sections, mounted on slides, should be stained within 2 months of sectioning when held at room temperature (20 25 C). Consult the Dako Handbook: Immunochemical Staining Methods 16 or References 14 and 15 for further details on specimen preparation. The use of decalcified tissues has not been validated and is not recommended. The slides required for c-kit evaluation and verification of tumor presence should be prepared at the same time. A minimum of 5 slides should be prepared, 1 slide for tumor presence, 2 slides for c-kit protein evaluation, and 2 slides for back-up. Reagent preparation The following reagents must be prepared prior to staining: Target Retrieval Solution (code S1699) Prepare a sufficient quantity of Target Retrieval Solution by diluting Target Retrieval Solution 10x 1:10 using distilled or deionized water (reagent-quality water) for the wash steps. Discard Target Retrieval Solution after use. Note: When using Dako s Target Retrieval Solution (Code S1700) no dilution is necessary. Wash Buffer Solution (code S3006) Prepare a sufficient quantity of Wash Buffer by diluting Wash Buffer 10x, 1:10 using distilled or deionized water (reagent-quality water) for the wash steps. Store unused solution at 2-8ºC for no more than 7 days. Discard buffer if cloudy in appearance. Substrate-Chromogen Solution (DAB+) (code K3467 or K3468) This solution should be mixed thoroughly prior to use. Any precipitate developing in the solution does not affect staining quality. To prepare DAB+ Substrate-Chromogen Solution, add 1 drop of Liquid DAB+ Chromogen to one ml of DAB+ Substrate Buffer and mix. Discard any unused solution. Stability of prepared DAB+ is approximately 5 days when stored at 2 8 C. Important Note: The color of the Liquid DAB+ Chromogen in the bottle may vary from clear to light lavenderbrown. This will not affect the performance of this product. Dilute per the guidelines above. Addition of excess Liquid DAB+ Chromogen to the DAB+ Substrate Buffer will result in deterioration of the positive signal. Mounting Medium Non-aqueous, permanent mounting media such as Dako Ultramount (code S1964) is recommended. Aqueous mounting media such as Dako Faramount Mounting Medium, Ready-to-use (code S3025) or Dako Glycergel Mounting Medium (code C0563) is also suitable. Liquefy Glycergel by warming to approximately 40(±5) C prior to use. Staining procedure for manual use Procedural Notes The user should read these instructions carefully and become familiar with all components prior to use (see Precautions). All reagents should be equilibrated to room temperature (20 25 C) prior to immunostaining. Likewise, a ll incubations should be performed at room temperature. Do not allow tissue sections to dry during the staining procedure. Dried tissue sections may display increased nonspecific staining. To avoid drying place slides in a humid chamber. Note: The reagents and instructions supplied in this system have been designed for optimal performance when used with the recommended reagents and materials. Further dilution of the reagents or alteration of incubation times or temperatures may give erroneous or discordant results. ( ) EFG_001 p. 4/40

5 Deparaffinization and Rehydration Prior to staining, tissue slides must be deparaffinized to remove embedding medium and rehydrated. Avoid incomplete removal of paraffin. Residual embedding medium will result in increased nonspecific staining. STEP 1. Place slides in a xylene bath and incubate for 5 (±1) minutes. Change baths and repeat once. STEP 2. Tap off excess liquid and place slides in absolute ethanol for 3 (±1) minutes. Change baths and repeat once. STEP 3. Tap off excess liquid and place slides in 95% ethanol for 3 (±1) minutes. Change baths and repeat once. STEP 4. Tap off excess liquid and place slides in reagent-quality water for 5 (±1) minutes. STEP 5. Tap off excess liquid and place slides in Wash Buffer. Begin assay as outlined in Staining Procedure. Xylene and alcohol solutions should be changed after 40 slides. Toluene or xylene substitutes, such as Histoclear, may be used in place of xylene. Target Retrieval Recommended procedure: Water Bath STEP 1. Fill staining jars, e.g. Coplin jars, with the diluted Target Retrieval Solution (see Reagent preparation). Place staining jars containing Target Retrieval Solution in water bath. Heat water bath and the Target Retrieval Solution to C (do no t boil). Cover jars with lids to stabilize the temperature and avoid evaporation. STEP 2. Immerse room temperature deparaffinized sections in the preheated Target Retrieval Solution in the staining jars. Re-equilibrate temperature of the water bath and the Target Retrieval Solution back to C. Incubate for 20 (±1) minutes at C. STEP 3. Remove the entire jar with slides from the water bath. Allow the slides to cool in the Target Retrieval Solution for 20 (±1) minutes at room temperature. STEP 4. Decant Target Retrieval Solution and rinse sections in Wash Buffer (see Reagent Preparation). STEP 5. For optimal performance, soak sections in Wash Buffer for 5 (±1) minutes after target retrieval and prior to staining. Target Retrieval Pressure Cooker (30 seconds at 125 C) It is very important to maintain consistency in the target retrieval protocol to ensure reproducibility of staining results. The pressure cooker used must be capable of achieving and maintaining 125 C and each pressure cooker run should include the same total volume of liquid (Target Retrieval Solution and water in pressure cooker pan) as well as the same total number of slides. The optimal protocol is as follows: STEP 1. For each run add a consistent volume of reagent quality water to pressure cooker pan (500 ml or consult manufacturer s instructions). STEP 2. Fill each of two heat-tolerant plastic staining containers that will accommodate a 24-slide rack with 250 ml of prepared Target Retrieval Solution (see Reagent preparation). STEP 3. Immerse room temperature deparaffinized sections in Target Retrieval Solution in the staining containers. Note: For consistency, during each run 48 slides should be placed in the pressure cooker, with blank slides used to fill staining racks if fewer than 48 specimen slides are to be processed; if 24 or fewer slides are to be processed, to conserve Target Retrieval Solution the second staining container can be filled with 250mL of water. STEP 4. Place the two staining containers into the pressure cooker and securely tighten the lid. STEP 5. Set pressure cooker to heat to 125 C; incub ate slides at 125 C for 30 seconds. Allow pressure cooker to cool for 30 minutes, without venting of pressure, prior to opening. STEP 6. Before opening pressure cooker ensure that pressure has released. Remove the staining containers, decant Target Retrieval Solution, and rinse sections in Wash Buffer or reagent quality water. STEP 7. Soak sections in Wash Buffer for 5 (±1) minutes after target retrieval prior to staining. Note: The Target Retrieval Solution is designed for single use application only. Do not re-use. Manual Staining Protocol STEP 1. Dual Endogenous Enzyme Block (code S2003) Tap off excess water. Using a lintless tissue, carefully wipe around the specimen to remove any remaining liquid and to keep reagent within the prescribed area. Apply enough Dual Endogenous Enzyme Block to cover specimen [minimum 3 drops (100µL)]. Incubate 5 (±1) minutes. Rinse gently with Wash Buffer from a wash bottle (do not focus stream directly on tissue or tissue may be washed off of slide). Place in a fresh Wash Buffer bath for 5 (±1) minutes. STEP 2. Primary Antibody or Negative Control Reagent Place slides in a humid chamber during the Primary Antibody/Negative Control Reagent and Labeled Polymer incubations to avoid drying of tissues. 6 Tap off excess buffer and wipe slides as before. Apply enough Primary Antibody or Negative Control Reagent to cover specimen [minimum 3 drops (100 µl)]. Incubate 30 (±1) minutes in a humid chamber. Rinse slides as in Step 1. STEP 3. EnVision + HRP, Anti-Rabbit (code K4002 or K4003) Tap off excess buffer and wipe slides as before. Apply enough Labelled Polymer to cover specimen [minimum 3 drops (100 µl)]. Incubate 30 (±1) minutes in a humid chamber. Rinse slides as in Step 1. STEP 4. DAB+ Substrate-Chromogen Solution (code K3467 or K3468) Tap off excess buffer and wipe slides as before. Apply enough prepared DAB+ Substrate-Chromogen Solution to cover specimen [minimum 3 drops (100 µl)]. Incubate 10 (±1) minutes. Rinse gently with reagent-quality water from a wash bottle (do not focus flow directly on tissue or tissue may be washed off of slide). Collect DAB+ Substrate-Chromogen Solution waste in a hazardous materials container for proper disposal. Place in a reagent-quality water bath for 2 5 minutes. Proceed to Counterstain and Mounting. Counterstain (instructions for Hematoxylin) The colored end-product of the staining reaction is alcohol and water insoluble. Hematoxylin, either alcohol or water-based such as Dako Hematoxylin (code S3301, automated; or S3302, manual), may be used. Do not use regressive counterstains. STEP 1. Immerse slides in a bath of hematoxylin. Incubate for 2 5 minutes, depending on the strength of hematoxylin used. STEP 2. Rinse gently in a reagent-quality water bath. Ensure that all residual hematoxylin has been cleared. Note: The use of Dako s Hematoxylin (code S3302) is strongly recommended. Using a 3-minute incubation, this counterstain produces a mild purple/blue end product that does not obscure specific immunostaining. Strong counterstaining may mask weak c-kit/cd117 expression. STEP 3. Rinse gently in a reagent-quality water bath for 2 5 minutes. Mounting Non-aqueous, permanent mounting media such as Dako Ultramount (code S1964) is recommended. Aqueous mounting media such as Dako Faramount Mounting Medium, Ready-to-use (code S3025) or Dako Glycergel Mounting Medium (code C0563) is also suitable. Liquefy Glycergel by warming to approximately 40(±5) C prior to use. Note: It is recommended that slides are read within six weeks of staining. However, some fading may occur, depending on several factors including, but not limited to; counterstaining, mounting materials and methods and slide storage conditions. To minimize fading, store slides in the dark at room temperature (20 25 C). Quality Control Deviations in the recommended procedures for tissue fixation, processing and embedding in the user s laboratory may produce significant variability in results, necessitating regular performance of in-house controls in addition to the Dako-supplied Control Slides. In the USA, consult the quality control guidelines of the College of American Pathologists (CAP) Certification Program for Immunohistochemistry. See also CLSI Quality Assurance for Immunocytochemistry Approved Guideline 17 and References for additional information. c-kit pharmdx Control Slides (provided) Each of the supplied Control Slides contains two pelleted, formalin-fixed, paraffin-embedded mouse (+) and human (-) cell lines with staining intensity scores of 2+ and 0. One slide should be stained in each staining procedure. The evaluation of the Dako-supplied Control Slide cell lines indicates the validity of the staining run. They should not be used as an aid in interpretation of patient results. Positive Control Tissue Controls should be fresh autopsy/biopsy/surgical specimens fixed, processed and embedded as soon as possible in the same manner as the patient sample(s). Positive tissue controls are indicative of correctly prepared tissues and proper staining techniques. One positive tissue control for each set of test conditions should be included in each staining run. The tissues used for the positive tissue controls should give weak positive staining so they can detect subtle changes in the primary antibody sensitivity. The Control Slides supplied with this system or specimens processed differently from the patient sample(s) validate reagent performance only and do not verify tissue preparation. Known positive tissue controls should only be utilized for monitoring the correct performance of processed tissues and test reagents, NOT as an aid in formulating a specific diagnosis of patient samples. If the positive tissue controls fail to demonstrate appropriate positive staining, results with the test specimens should be considered invalid. Normal cell types that may be used as a weak c-kit positive control include basal epidermal melanocytes and ( ) EFG_001 p. 6/40

7 glandular myoepithelial cells of the skin, as well as epithelial and myoepithelial cells of the breast. Strong internal controls can be neuronal (Interstitial cells of Cajal) and mast cells in the intestine. Negative Control Tissue Use a negative control tissue (known to be c-kit protein negative) fixed, processed and embedded in a manner similiarl to the patient sample(s) with each staining run to verify the specificity of the primary antibody and to provide an indication of specific background staining. The variety of different cell types present in most tissue sections offers internal negative control sites (this should be verified by the user). If specific staining occurs in the negative control tissue, results with the patient specimens should be considered invalid. Tissue elements which may be used as negative controls include cardiac myocytes of the heart. Pancreatic acinar cells are also c-kit negative, whereas pancreatic bile duct epithelium stains positively. Non-specific Negative Control Reagent Use the supplied Negative Control Reagent in place of the primary antibody with a section of each patient specimen to evaluate nonspecific staining and allow better interpretation of specific staining at the antigen site. The incubation period for the Negative Control Reagent should correspond to that of the primary antibody. When panels of antibodies are used on serial sections, the negatively staining areas of one slide may serve as negative/non-specific binding background controls for other antisera. Assay Verification Prior to initial use of an antibody or staining system in a diagnostic procedure, the user should verify the antibody's specificity by testing it on a series of in-house tissues with known IHC performance characteristics representing known positive and negative tissues. Refer to the Quality control procedures previously outlined in this section of the product insert and to the Quality control requirements of the CAP Certification Program for Immunohistochemistry and/or CLSI Quality Assurance for Immunocytochemistry, Approved Guideline. 17 These quality control procedures should be repeated for each new antibody lot, or whenever there is a change in assay parameters. Gastrointestinal Stromal Tumors (GIST) with known c-kit protein staining intensities and negative tissues are suitable for assay verification. Use intact cells for interpretation of staining results; necrotic or degenerated cells often stain non-specifically. 14,17 Positive and negative staining cell lines are included in each c-kit pharmdx kit to validate assay performance. Appropriate staining of the control slides provides evidence that the c-kit pharmdx reagents are functioning properly and the protocol has been performed accurately. No staining of the HT-29 control cell line (0) and moderate brown cytoplasmic or paranuclear staining in the P815 control cell line (2+) indicates that the staining run is valid. Deviation from the optimal staining intensity of the positive control cell line (too weak or too strong) may result in a false negative or a false positive result and indicate that the test should be repeated. 8 Evaluation of the positive tissue control should be performed. Weak staining of skin myoepithelial cells indicates that the reagents are working properly. If the positive tissue controls fail to demonstrate positive staining, any results with the test specimens should be considered invalid. The negative tissue control should be examined after the positive tissue control to verify the specificity of the labeling of the antigen by the primary antibody. The absence of specific staining in the negative tissue control confirms lack of antibody cross-reactivity to cells/cellular components. If specific staining occurs in the negative tissue control, results with the patient specimen should be considered invalid. Nonspecific staining, if present, usually has a diffuse appearance. Sporadic staining of connective tissue may also be observed in sections from excessively formalin-fixed tissues. Use intact cells for interpretation of staining results. Necrotic or degenerated cells often stain nonspecifically. 18 Examine patient specimens last. Positive staining intensity should be assessed within the context of any nonspecific background staining of the negative reagent control. Interpretation of c-kit/cd117 protein expression must be made within the general morphological and clinical context of the tumor. There are three morphologic categories of GIST: spindle cell (70%), epithelioid (20%), or mixed types. 22 Regardless of morphology, the majority of GISTs express c-kit protein in a significant proportion of the tumor cells. Homogeneous immunostaining with c-kit pharmdx is primarily seen in the cytoplasm, with or without a Golgi/paranuclear (dot-like) pattern, and in the cell membranes, with either complete or incomplete circumferential staining. Some cases show a heterogeneous (a mixture of strong or weak cytoplasmic and/or membrane) staining pattern. Staining has also been observed in the extracellular spaces. c-kit pharmdx test results should be reported as positive or negative, using cytoplasmic and membrane staining as the evaluable structure (Table 2). Positivity for c-kit protein expression is defined as any specific cytoplasmic and/or membrane staining in the tumor cells. Focal positivity or staining in less than 10% of tumor cells should be interpreted with caution. 23 As with any immunohistochemistry test, a negative result means that the antigen was not detected, not that the antigen was absent in the cells/tissues assayed. It should also be noted that a small percentage of GISTs do not express c-kit protein. Therefore, absence of c-kit protein staining does not necessarily exclude the diagnosis of GIST. 9 Limitations General limitations IHC is a multistep diagnostic process that requires specialized training in the selection of the appropriate reagents; tissue selection, fixation, and processing; preparation of the IHC slide; and interpretation of the staining results. Tissue staining is dependent on the handling and processing of the tissue prior to staining. Improper fixation, freezing, thawing, washing, drying, heating, sectioning, or contamination with other tissues or fluids may produce artifacts, antibody trapping, or false-negative results. Inconsistent results may be due to variations in fixation and embedding methods, or to inherent irregularities within the tissue. The clinical interpretation of any positive staining or its absence must be evaluated within the context of clinical presentation, morphology and other histopathological criteria. The clinical interpretation of any staining, or its absence must be complemented by morphological studies and proper controls as well as other diagnostic tests. It is the responsibility of a qualified pathologist who is familiar with the antibodies, reagents and methods used to interpret the stained preparation. Staining must be performed in a certified licensed laboratory under the supervision of a pathologist who is responsible for reviewing the stained slides and assuring the adequacy of positive and negative controls. This product is not intended for flow cytometry. Performance characteristics have not been determined for flow cytometry. Tissues from persons infected with hepatitis B virus and containing hepatitis B surface antigen (HBsAg) may exhibit nonspecific staining with horseradish peroxidase. 24 Reagents may demonstrate unexpected reactions in previously untested tissues. The possibility of unexpected reactions even in tested tissue groups cannot be completely eliminated due to biological variability of antigen expression in neoplasms, or other pathological tissues. 20 Contact Dako technical support with documented unexpected reactions. Product specific limitations A small percentage of GISTs do not express c-kit protein. Therefore, the absence of c-kit staining does not exclude the diagnosis of GIST. False-positive results may be seen due to non-immunological binding of proteins or substrate reaction products. They may also be caused by pseudoperoxidase activity (erythrocytes) and endogenous peroxidase activity (cytochrome C). 19 For optimal and reproducible results, the c-kit protein requires target retrieval when tissues are routinely fixed (neutral buffered formalin) and paraffin embedded. Dako recommends the use of c-kit pharmdx on a Dako Autostainer or Autostainer Plus. Use of c-kit pharmdx on alternative automated platforms has not been validated and may give erroneous results. Stained control cell lines should be used only for validation of the staining run and should not be used to score the staining reaction in tissue sections. Specimens preserved by routine processing in neutral buffered formalin are suitable for testing with c-kit pharmdx. Tissues fixed in alternative fixatives have not been validated. Performance characteristics Specificity The c-kit antibody reagent has been tested for reactivity against cell lines expressing c-kit protein. In Western blotting of a lysate of the small cell lung carcinoma cell line SY that over-expresses c-kit mrna, the antibody reagent labeled a band of 145 kd corresponding to the c-kit protein. The labeled band is rather broad, from 120 to 155 kd. An additional band of 100 kd was also labeled. 25 Further, applying a second anti-c-kit antibody, a 100 kd protein was, likewise, labeled. This labelling was abolished when the antibody reagent was incubated with the synthetic c-kit peptide antigen used for immunization. 26 In additional studies, the c-kit antibody reagent was tested using Western blotting for cross-reactivity to proteins that share structural homology, such as; Platelet Derived Growth Factor Receptor (PDGFRa), macrophage colony stimulating factor receptor (c-fms); and FMS-like tyrosine kinase 3 (FLT-3). No cross-reactivity was observed with these homologous proteins. Furthermore, in Western blotting the Dako antibody reagent did not react with a lysate of the adenocarcinoma cell line HS, which does not express the c-kit gene. 25 Comparison Studies Immunostaining using the c-kit pharmdx assay was performed on formalin-fixed, paraffin-embedded sarcomas and multi-tissue arrays (MTAs) containing a selection of sarcomas that included positive and negative c-kit protein expression. The tissues were tested using the c-kit pharmdx assay after the use of the two recommended methods of heat induced epitope retrieval (HIER). Briefly, water bath (95 99 C) for 20 minutes and pressure cooker (121 C) for 5 minutes o f heating, both followed by 20 minutes of cool down, were used as heat sources. The remaining steps of the staining procedure were identical. These assays were compared to a concurrently run simulation of the Novartis Clinical Trial Protocol (NCTP) that had been used previously to select patients for the clinical trial that obtained FDA approval for the use of Gleevec/Glivec with diagnosed GIST patients. The NCTP used a 2X higher concentration of Dako primary polyclonal antibody against c-kit (A4502) (the same reagent used in c-kit pharmdx) and no HIER. Results from the concurrent comparison of the NCTP and the c-kit pharmdx assay using pressure cooker as the heat source are listed in Table 4 and similar results (overall agreement = 98.7%), were seen when a water bath was used as a heat source. Of these patients 21 were treated with Gleevec (58.3%). When the same specimens were retested with the simulated NCTP and c-kit pharmdx 20 were found to be positive with all three procedures, whereas 1 specimen was found to be negative. Among the 147 patients treated with Gleevec in the Novartis Gleevec Clinical Trial it was reported that 54% had a partial response, and 28% had stable disease 7. For a subset of 259 specimens presented in Table 4, the c-kit test result was determined at the time the specimens were put into the MTAs. Thirty-five of the cases were enrolled in the Gleevec clinical trials (n=35), and the remainder were tested in the same laboratories that had participated in the Gleevec trials. Results from 179 specimens (the total was less than 259 due to specimen loss or lack of tumor in the MTAs) are presented in Table 5 below. These specimens demonstrated an overall agreement to the original c-kit status of 94.9% (95% exact CI: 90.7% %). In the 30 tissues tested, the staining intensity varied by no more than +/- one staining intensity grade with the following exceptions: At sites 1 and 3, one slide (each) varied by two intensity grades. This was a result of tissue disruption at site 1. Overall excellent reproducibility (100%) was seen for positive versus negative results. Inter-laboratory reproducibility (Manual and Automated Staining using the Dako Autostainer) Immunohistochemical staining of 30 randomized and masked specimens of various expression levels of c-kit (10 negative and 20 positive), was performed at three geographically separated laboratories. Cut slides were forwarded to each testing laboratory for manual and automated staining and evaluation by a pathologist. At two sites, positive/negative determination of c-kit protein expression was perfect (100%) within and between laboratories in both manual and automated testing procedures. At site 2, duplicate slides of two tissues, designated before the study to be negative were found to be, in actuality, positive. Follow-up analysis revealed that a small area (approximately 20% of the tumor cells) was stained positively. This section of tumor was not seen in the specimens tested at the other laboratories. Intra-run reproducibility (Manual Staining) At each of the study sites, 5 slides of a same positive specimen were included in the set of 30 specimens stained manually. Staining intensity and percent of tumor staining were assessed. The tissue result remained positive at each study site, and the staining intensity remained within +/- 1 intensity grade variation. 11 Immunoreactivity A summary of the c-kit pharmdx assay immunoreactivity on the recommended panel of normal tissues is presented in Table 6. All tissues were formalin-fixed and paraffin-embedded. The instructions provided and the reagents recommended in this package insert were used to perform 30 normal tissue testing on the DAKO Autostainer. Le dosage c-kit pharmdx est un kit de dosage immunohistochimique (IHC) qualitatif utilisé pour identifier l'expression de la protéine c-kit/antigène CD117 (protéine c-kit) dans des tissus sains et néoplasiques fixés au formol et inclus en paraffine pour évaluation histologique. Résumé et explication Introduction Le proto-oncogène c-kit, également appelé antigène CD117 ou Récepteur du facteur de croissance des cellules souches, est un récepteur tyrosine kinase transmembranaire de type III, de 145 kd. Le gène c-kit code pour un récepteur tyrosine kinase transmembranaire, similaire sur le plan structural aux récepteurs A et B du facteur de croissance dérivé des plaquettes (PDGFR), ainsi qu au récepteur du facteur de stimulation des colonies 1 (CSF-1) ; on pense que ce récepteur joue un rôle important dans l hématopoïèse, la spermatogenèse et la mélanogénèse. La protéine c-kit contient des domaines extracellulaires à 5 boucles de type Ig, un domaine transmembranaire hautement hydrophobe et un domaine intracellulaire ayant une activité tyrosine kinase divisé par un insert kinase dans une région de liaison de l ATP et dans un domaine phosphotransférase. L activation du récepteur s accompagne d une dimérisation du récepteur, d une phosphorylation du substrat et d une autophosphorylation, d une internalisation du récepteur, de l activation des protéines kinases et phospholipases et de la transcription de différents proto-oncogènes. 4 La voie du récepteur tyrosine kinase c-kit s est avérée importante pour l étude de la croissance et de la progression des tumeurs dans divers cancers. 5 Des mutations du gène c-kit peuvent déboucher sur une phosphorylation (activation) de la tyrosine kinase de la c-kit, indépendante du ligand, qui semblent jouer un rôle pathogène central notamment dans les tumeurs stromales gastro-intestinales. 6 Les tumeurs mésenchymateuses gastro-intestinales se sont toujours révélées difficiles à différencier et à diagnostiquer. Les TSGI sont également généralement positives à la caldesmone de haut poids moléculaire, souvent positives à la CD34 (60-80 %) et généralement négatives à la desmine et à la protéine S Spécificité Les anticorps polyclonaux de lapin anti-c-kit ont été obtenus par injection sous-cutanée d un peptide de 14 acides aminés (positions 963 à 976 de la partie C-terminale intracellulaire de la protéine c-kit) couplé à de la thyroglobuline. L'antisérum a été purifié de manière spécifique par chromatographie d'affinité sur gel AvidGel F, portant des groupements thiols activés avec liaison antigénique. Un petit nombre de sarcomes des tissus mous se sont également révélés positifs à la protéine c-kit. 6 ( ) EFG_001 p. 13/40

14 L expression de la protéine c-kit a été testée sur certains de ces échantillons (ex. : léiomyosarcome). L étude reposait sur une comparaison entre le dosage c-kit pharmdx et le Protocole d essai clinique Novartis, tel qu utilisé dans la sélection des patients à traiter au Gleevec dans les essais cliniques réalisés par Novartis. Sur vingt-huit échantillons, deux ont présenté une coloration positive. Le résultat était cohérent d un protocole à l autre. Toutes les colorations immunologiques positives ont été supprimées après absorption de l anticorps primaire avec un peptide de synthèse (16 acides aminés de l'extrémité C-terminale de la protéine c-kit). Par conséquent, l anticorps primaire utilisé dans le dosage c-kit pharmdx a réagi de manière spécifique avec la protéine c-kit dans les deux sarcomes des tissus mous ayant présenté une coloration positive. Des études internes utilisant le kit c-kit pharmdx ont montré l'expression de la protéine c-kit dans diverses cellules normales. Ces cellules comprennent les cellules canalaires et myoépithéliales mammaires, les processus des cellules de Purkinje, les cellules de la lamina propria du côlon, l'épithélium tubulaire du rein, les mélanocytes et les cellules myoépithéliales de la peau, les cellules interstitielles de Cajal de l'intestin grêle et les mastocytes. La réactivité cytoplasmique dans les granulocytes a été provoquée par une activité peroxydasique endogène et était rendue visible par le réactif de contrôle négatif. Réactifs La référence K1906 correspond à la coloration manuelle. Les matériels indiqués permettent d'effectuer 25 tests (25 lames de patient et 10 lames de contrôle incubées avec l anticorps primaire dirigé contre la protéine c-kit et 25 lames incubées avec le réactif de contrôle négatif correspondant). Le nombre de tests est basé sur l'utilisation du protocole manuel c-kit pharmdx incluant les réactifs prêts à l'emploi. Le kit fournit la quantité suffisante de matériels pour un maximum de 10 cycles de coloration. Matériels fournis Quantité Description 1 x 4 ml IgG polyclonales de lapin c-kit pharmdx Immunoglobulines polyclonales de lapin anti-c-kit humaine dans un tampon Tris-HCl contenant une protéine stabilisante et de l'azide de sodium à 0,015 mol/l. 1 x 4 ml Réactif de contrôle négatif IgG de lapin Immunoglobulines polyclonales de lapin à une concentration égale ou supérieure à la concentration en anticorps anti-c-kit positif dans un tampon Tris-HCl, contenant une protéine stabilisante et de l'azide de sodium à 0,015 mol/l. 2 x 5 lames Lames de contrôle c-kit pharmdx Chaque lame contient des coupes de deux lignées cellulaires de souris (+) et humaines (-), fixées au formol, incluses en paraffine et enrobées, présentant toutes les deux un niveau modéré d'expression et aucune expression de la protéine c-kit. Les notes de coloration IHC des culots cellulaires sont 2+ et 0. Principe de la procédure Le kit d'ihc c-kit pharmdx contient des anticorps polyclonaux et des lames de contrôle permettant d effectuer une procédure de coloration IHC sur des échantillons fixés au formol et inclus en paraffine. Après incubation avec les anticorps polyclonaux primaires dirigés contre la protéine c-kit humaine, ce kit est optimisé pour être utilisé avec un réactif de visualisation prêt à l'emploi à base de dextrane. Ce réactif comporte des molécules d'anticorps secondaires de chèvre anti-lapin et des molécules de peroxydase de raifort liées à une chaîne polymère de dextrane commune, ce qui permet d'éliminer l'application séquentielle du conjugué peroxydase et de l'anticorps de liaison. La conversion enzymatique du chromogène ajouté par la suite entraîne la formation d'un produit de réaction visible sur le site de l'antigène. L'échantillon peut alors être contre-coloré et recouvert d'une lamelle de protection. Un microscope optique est utilisé pour l'interprétation des résultats. Aux concentrations du produit, bien que non classé comme dangereux, l accumulation de NaN 3 peut réagir avec le cuivre et le plomb des canalisations pour former des azides métalliques hautement explosifs. Lors de l élimination, rincer abondamment à l eau pour éviter toute accumulation d azide dans les canalisations. 3. Comme avec tout produit d origine biologique, des procédures de manipulation appropriées doivent être utilisées pour les réactifs comme pour les échantillons. 4. Toute durée, température ou méthode d'incubation autres que celles indiquées peuvent entraîner des résultats erronés. 5. Ne pas remplacer les anticorps primaires ou les réactifs de contrôle négatif par des anticorps primaires ou des réactifs de contrôle négatif provenant d autres lots (les numéros de lot figurent sur les étiquettes des flacons) ou par des réactifs fournis par d'autres fabricants. Ceci peut produire des résultats erronés. 6. Les réactifs du kit sont dilués de manière optimale pour être utilisés avec les réactifs recommandés. Une dilution supplémentaire n'est pas recommandée et peut entraîner une perte de coloration antigénique. L'utilisateur doit vérifier toute modification de ce type. 7. Porter un vêtement de protection approprié pour éviter le contact avec les yeux et la peau. 8. Les solutions inutilisées doivent être éliminées conformément aux réglementations locales et nationales. 9. 16 Conservation Conserver le kit c-kit pharmdx entre 2 et 8 C. Les lames de contrôle doivent être conservées entre 2 et 8 C. Ne pas utiliser le kit après la date de péremption indiquée sur la boîte. Aucun signe évident n'indique l'instabilité de ce produit. Par conséquent, les contrôles positif et négatif doivent être analysés en même temps que les échantillons de patient. Si les réactifs sont conservés dans des conditions autres que celles indiquées dans la notice, celles-ci doivent être validées par l utilisateur. 13 Déclaration de qualité Préparation des échantillons La qualité des réactifs du kit c-kit pharmdx a été contrôlée par immunohistochimie dans les conditions suivantes : restauration des cibles dans un autocuiseur Pascal (réf. S2800) programmé pour une durée de 30 secondes à 125 C, refroidissant jusqu'à 90 C, suivi par le système EnVision+ Rabbit, HRP. Les échantillons de biopsie doivent être manipulés de sorte à préserver le tissu pour la coloration IHC. Des méthodes standard de traitement des tissus doivent être utilisées pour tous les échantillons. 14 Coupes incluses en paraffine Les tissus fixés au formol et inclus en paraffine sont adaptés à l utilisation du kit c-kit. D'autres fixateurs n'ont pas été validés et peuvent produire des résultats erronés. Les échantillons de biopsie doivent être inclus dans 3 à 4 mm de paraffine et fixés pendant la durée appropriée au fixateur. Les tissus sont ensuite déshydratés et nettoyés dans une série de bains d'alcools et de xylène, puis infiltrés par la paraffine fondue. La température de la paraffine ne doit pas dépasser 60 C. Les blo cs de tissus inclus et fixés correctement et exprimant la protéine c-kit se conservent indéfiniment avant leur coupe et le montage sur lame s ils sont conservés dans un 14, 15 endroit frais (entre 15 et 25 C). L'épaisseur des coupes d'échantillons de tissu doit être comprise entre 3 et 5 µm. Après avoir été coupés, les tissus doivent être montés sur des lames SuperFrost Plus de Fisher, des Silanized Slides de Dako (réf. S3003), des lames chargés ou des lames enduites de poly-l-lysine, puis placés sur des portoirs pour être séchés. Les portoirs de lames doivent être tapotés sur une serviette absorbante afin d'éliminer l'eau piégée entre le verre et la couche de paraffine, puis séchés à température ambiante pendant une heure. Le portoir de lames doit alors être placé dans un incubateur entre 56 et 60 C pendant une heure. Tout excès d'eau r estant sur les lames après leur sortie de l'incubateur doit être éliminé en tapotant les lames sur des serviettes et en les faisant sécher pendant une heure supplémentaire dans l'incubateur. À leur sortie de l'incubateur, les lames doivent être maintenues à température ambiante jusqu'à refroidissement et durcissement de la paraffine. Afin de conserver l'antigénicité, les coupes de tissu, montées sur lames, doivent être colorées dans les 2 mois suivant la coupe, en étant conservées à température ambiante (entre 20 et 25 C). Consulter le Manuel de Dako : «Méthodes de coloration immunohistochimique» 16 ou les références 14 et 15 pour plus de détails sur la préparation des échantillons. L'utilisation de ce test sur des tissus décalcifiés n'a pas été validée et n'est pas recommandée. Les lames nécessaires à l'évaluation de la protéine c-kit et à la vérification de la présence de tumeur doivent être préparées simultanément. Il est nécessaire de préparer au moins 5 lames : 1 lame pour détecter la présence de tumeur, 2 lames pour l'évaluation de la protéine c-kit et 2 lames de secours. Préparation des réactifs Les réactifs suivants doivent être préparés avant de procéder à la coloration : Target Retrieval Solution (Solution de restauration des cibles) (réf. S1699) Préparer une quantité suffisante de solution de restauration des cibles en diluant la Target Retrieval Solution 10x, au 1/10ème en utilisant de l'eau distillée ou déionisée (eau de qualité réactif) pour les étapes de lavage. Jeter la Target Retrieval Solution après utilisation. Remarque : Si vous utilisez la Target Retrieval Solution de Dako (réf. S1700), aucune dilution n'est nécessaire. Wash Buffer Solution (Solution de tampon de lavage) (réf. S3006) Préparer une quantité suffisante de tampon de lavage en diluant du Wash Buffer 10x, au 1/10ème en utilisant de l'eau distillée ou déionisée (eau de qualité réactif) pour les étapes de lavage. Conserver la solution inutilisée entre 2 et 8 ºC pendant 7 jours maximum. Jeter le tampon s'il semble trouble. Substrate-Chromogen Solution (DAB+) (Solution de substrat chromogène DAB+) (réf. K3467 ou K3468) Cette solution doit être bien mélangée avant utilisation. La formation d un précipité dans la solution n'affecte pas la qualité de la coloration. Pour préparer la solution de substrat chromogène DAB+, ajouter 1 goutte de Liquid DAB+ Chromogen (chromogène DAB+ liquide) dans 1 ml de DAB+ Substrate Buffer (tampon substrat DAB+) et mélanger. Jeter toute solution diluée non utilisée. Le DAB+ préparé est stable pendant environ 5 jours s'il est conservé entre 2 et 8 C. Remarque importante : La couleur du Liquid DAB+ Chromogen (chromogène DAB+ liquide) dans le flacon peut varier de transparent à lavande-marron clair. Cela n'affecte en rien les performances de ce produit. Diluer conformément aux instructions indiquées ci-avant. L'ajout d'une quantité trop importante de Liquid DAB+ Chromogen (chromogène DAB+ liquide) dans le DAB+ Substrate Buffer (tampon substrat DAB+) entraîne une détérioration du signal positif. 17 Milieu de montage Un milieu de montage permanent non aqueux, tel la Dako Ultramount (réf. S1964), est recommandé. Les milieux de montage aqueux tels le Dako Faramount Mounting Medium, prêt à l'emploi (réf. S3025) ou le Dako Glycergel Mounting Medium (réf. C0563) sont également adaptés. Liquéfier le Glycergel en le chauffant à environ 40 C (± 5 C) avant utilisation. Procédure de coloration pour utilisation manuelle Remarques sur la procédure L utilisateur doit lire attentivement les présentes instructions et se familiariser avec tous les composants avant utilisation (voir les Précautions). Tous les réactifs doivent être portés à température ambiante (20 25 C) avant la coloration immunologi que. De même, toutes les incubations doivent être effectuées à température ambiante. Ne pas laisser les lames sécher pendant la procédure de coloration. Les coupes de tissus séchées peuvent présenter une coloration non spécifique plus importante. Pour éviter leur dessèchement, placer les lames dans une chambre humide. Remarque : Les réactifs fournis dans ce kit et les instructions correspondantes ont été conçus pour des performances optimales lors de leur utilisation avec les réactifs et matériels recommandés. Toute dilution supplémentaire des réactifs du kit, ainsi que toute modification des temps ou températures d incubation, peut produire des résultats erronés ou discordants. Déparaffinage et réhydratation Avant la coloration, les lames de tissu doivent être déparaffinées afin d'éliminer le milieu de montage, puis réhydratées. Éviter toute élimination incomplète de la paraffine. Tout résidu de milieu d'inclusion entraîne une coloration non spécifique plus importante. ÉTAPE 1. Placer les lames dans un bain de xylène et incuber pendant 5 minutes (± 1 minute). Renouveler les bains et recommencer l'opération une fois. ÉTAPE 2. Tapoter afin d'éliminer l'excès de liquide et placer les lames dans de l'éthanol absolu pendant 3 minutes (± 1 minute). Renouveler les bains et recommencer l'opération une fois. ÉTAPE 3. Tapoter afin d'éliminer l'excès de liquide et placer les lames dans de l'éthanol à 95 % pendant 3 minutes (± 1 minute). Renouveler les bains et recommencer l'opération une fois. ÉTAPE 4. Tapoter afin d'éliminer l'excès de liquide et placer les lames dans de l'eau de qualité réactif pendant 5 minutes (± 1 minute). ÉTAPE 5. Tapoter pour éliminer l'excès de liquide et placer les lames dans du Wash Buffer. Lancer le dosage comme indiqué dans la Procédure de coloration. Les solutions de xylène et d'alcool doivent être renouvelées toutes les 40 lames. Du toluène ou des substituts de xylène, comme Histoclear TM, peuvent être utilisés à la place du xylène. Restauration des cibles Procédure recommandée : Bain-marie ÉTAPE 1. Remplir les cuves de coloration (ex. : jarres de Coplin) avec la Target Retrieval Solution diluée (voir Préparation des réactifs). Placer les cuves de coloration contenant la Target Retrieval Solution dans un bain-marie. Chauffer le bain-marie et la Target Retrieval Solution à C (sans faire bouillir). Couvrir les cuves à l'aide de leur couvercle pour stabiliser la température et éviter l'évaporation. ÉTAPE 2. Immerger les coupes déparaffinées et amenées à température ambiante dans la Target Retrieval Solution préchauffée qui se trouve dans les cuves de coloration. Rééquilibrer la température du bain-marie et de la Target Retrieval Solution à C. Incuber pendant 20 minutes (± 1 minute) entre 95 et 99 C. ÉTAPE 3. Retirer la cuve contenant les lames du bain-marie. Laisser les lames refroidir dans la Target Retrieval Solution pendant 20 minutes (± 1 minute) à température ambiante. ÉTAPE 4. Laisser décanter la Target Retrieval Solution et rincer les coupes dans du Wash Buffer (voir Préparation des réactifs). ÉTAPE 5. Pour des performances optimales, laisser tremper les coupes dans le Wash Buffer pendant 5 minutes (± 1 minute) après restauration des cibles et avant coloration. Restauration des cibles Autocuiseur (30 secondes à 125 C) Il est très important de conserver l uniformité du protocole de restauration des cibles afin de garantir la reproductibilité des résultats de coloration. L'autocuiseur utilisé doit être capable d'atteindre et de conserver une température de 125 C et chaque cycle de l'autocuiseur doit inclure le même volume total de liquide (Target Retrieval Solution et eau dans le bassin de l'autocuiseur), ainsi que le même nombre total de lames. Le protocole optimal est le suivant : ÉTAPE 1. Pour chaque cycle, ajouter un volume constant d'eau de qualité réactif dans le bassin de l'autocuiseur (500 ml ou consulter les instructions du fabricant). ÉTAPE2. Remplir chacun des deux récipients de coloration en plastique résistants à la chaleur qui peuvent contenir un portoir de 24 lames avec 250 ml de Target Retrieval Solution préparée (voir la section ( ) EFG_001 p. 17/40

18 Préparation des réactifs). ÉTAPE 3. Immerger les coupes déparaffinées et amenées à température ambiante dans la Target Retrieval Solution dans les récipients de coloration. Remarque : Pour des raisons d uniformité, 48 lames doivent être placées dans l'autocuiseur pendant chaque cycle, avec des blancs (lames sans tissu) utilisés pour remplir les portoirs à lames si moins de 48 lames sont traitées, si un nombre inférieur ou égal à 24 lames est traité, il est possible de remplir le deuxième récipient de coloration avec 250 ml d'eau pour économiser la Target Retrieval Solution. ÉTAPE 4. Placer les deux récipients de coloration dans l autocuiseur et fermer hermétiquement le couvercle. ÉTAPE 5. Régler l'autocuiseur à une température de 125 C ; incuber les lames à 125 C pendant 30 secondes. Laisser l'autocuiseur refroidir pendant 30 minutes, sans laisser échapper la pression, avant l'ouverture. ÉTAPE 6. S'assurer que la pression a été libérée avant d'ouvrir l'autocuiseur. Retirer les récipients de coloration, laisser décanter la Target Retrieval Solution et rincer les coupes dans du Wash Buffer ou de l'eau de qualité réactif. ÉTAPE 7. Laisser tremper les coupes dans le Wash Buffer pendant 5 minutes (± 1 minute) après restauration des cibles et avant coloration. Remarque : La Target Retrieval Solution est conçue pour une application unique. Ne pas réutiliser. Protocole de coloration manuelle ÉTAPE 1. Dual Endogenous Enzyme Block (Agent bloquant double des enzymes endogènes) (réf. S2003) Secouer pour enlever l excès d eau. À l aide d un chiffon non pelucheux, essuyer avec précaution autour de l échantillon pour enlever tout liquide restant et maintenir le réactif dans la zone indiquée. Appliquer suffisamment de Dual Endogenous Enzyme Block (Agent bloquant double des enzymes endogènes) pour couvrir l échantillon [minimum 3 gouttes (100 µl)]. Incuber pendant 5 minutes (± 1 minute). Rincer doucement avec du Wash Buffer (Tampon de lavage) contenu dans un flacon de lavage (ne pas diriger le jet directement sur les tissus car ils pourraient se détacher de la lame). Placer dans un nouveau bain de Wash Buffer pendant 5 minutes (± 1 minute). ÉTAPE 2. Anticorps primaire ou Réactif de contrôle négatif Placer les lames dans une chambre humide pendant l incubation de l anticorps primaire/réactif de contrôle négatif et du polymère marqué afin d éviter le dessèchement des tissus. Tapoter pour enlever l excès de tampon et essuyer les lames comme décrit ci-avant. Appliquer suffisamment d anticorps primaire ou de réactif de contrôle négatif pour recouvrir l'échantillon [minimum 3 gouttes (100 µl)]. Incuber pendant 30 minutes (± 1 minute) dans une chambre humide. Rincer les lames comme indiqué à l étape 1. ÉTAPE 3. EnVision+ HRP, Anti-Rabbit (EnVision+ HRP, anti-lapin) (réf. K4002 ou K4003) Tapoter pour enlever l excès de tampon et essuyer les lames comme décrit ci-avant. Appliquer assez de polymère marqué pour recouvrir l'échantillon (au moins 3 gouttes, soit 100 µl). Incuber pendant 30 minutes (± 1 minute) dans une chambre humide. Rincer les lames comme indiqué à l étape 1. ÉTAPE 4. Substrate-Chromogen Solution DAB+ (Solution de substrat chromogène DAB+) (réf. K3467 ou K3468) Tapoter pour enlever l excès de tampon et essuyer les lames comme décrit ci-avant. Appliquer suffisamment de solution de DAB+ Substrate-Chromogen (solution de substrat chromogène DAB+) pour recouvrir l échantillon [minimum 3 gouttes (100 µl)]. Incuber pendant 10 minutes (± 1 minute). Rincer doucement avec de l eau de qualité réactif contenue dans un flacon de lavage (ne pas diriger le jet directement sur les tissus car ils pourraient se détacher de la lame). Recueillir les déchets de solution de DAB+ Substrate-Chromogen (solution de substrat-chromogène DAB+) dans un conteneur pour déchets biologiques pour les éliminer de manière appropriée. Placer dans un bain d'eau de qualité réactif pendant 2 à 5 minutes. Procéder à la contre-coloration et au montage. Contre-coloration (instructions pour l'hématoxyline) Le produit final de la réaction de coloration n'est soluble ni dans l'alcool ni dans l'eau. Il est possible d utiliser de l hématoxyline, à base d'alcool ou d'eau, tel le produit Hematoxylin (réf. S3301, procédure automatisée ou S3302, procédure manuelle) de Dako. Ne pas utiliser de contre-colorants de régression. ÉTAPE 1. Immerger les lames dans un bain d'hématoxyline. Incuber pendant 2 à 5 minutes, en fonction de la puissance de l hématoxyline utilisée. ÉTAPE 2. Rincer doucement dans un bain d'eau de qualité réactif. Vérifier qu il n y a plus de résidus d'hématoxyline. Remarque : L'utilisation de l Hematoxylin (réf. S3302) de Dako est fortement recommandée. Après une incubation de 3 minutes, ce contre-colorant entraîne la formation d'un produit bleu/mauve qui n'obscurcit pas la coloration immunologique spécifique. Une forte contre-coloration peut masquer une faible expression de c- kit/cd117. ÉTAPE 3. Rincer doucement dans un bain d'eau de qualité réactif pendant 2 à 5 minutes. ( ) EFG_001 p. 18/40

19 Montage Contrôle qualité Un milieu de montage permanent non aqueux, tel la Dako Ultramount (réf. S1964), est recommandé. Les milieux de montage aqueux tels le Dako Faramount Mounting Medium, prêt à l'emploi (réf. S3025) ou le Dako Glycergel Mounting Medium (réf. C0563) sont également adaptés. Liquéfier le Glycergel en le chauffant à environ 40 C (± 5 C) avant utilisation. Remarque : Il est recommandé d examiner les lames dans les six semaines suivant la coloration. Cependant, une atténuation peut se produire et est liée à plusieurs facteurs, notamment la contre-coloration, les matériaux et méthodes de montage, ainsi que les conditions de conservation des lames. Pour limiter cette atténuation, conserver les lames dans l obscurité à température ambiante (20-25 C). Des différences dans les procédures recommandées pour la fixation, le traitement et l'inclusion des tissus dans le laboratoire de l'utilisateur peuvent générer une variabilité significative des résultats, ce qui requiert des contrôles internes réguliers, en complément des lames de contrôle fournies par Dako. Aux États-Unis, consulter les consignes de contrôle qualité du Certification Program for Immunohistochemistry du College of American Pathologists (CAP). Voir également le document Quality Assurance for Immunocytochemistry Approved Guideline du CLSI 17 et les références 18 à 21 pour plus d'informations. Lames de contrôle c-kit pharmdx (fournies) Chacune des lames de contrôle fournies contient deux lignées cellulaires de souris (+) et humaines (-), fixées au formol, incluses en paraffine et enrobées, présentant une intensité de coloration de 2+ et 0. Une lame doit être colorée lors de chaque procédure de coloration. L'évaluation des lignées cellulaires des lames de contrôle fournies par Dako indique la validité du cycle de coloration. Elles ne doivent pas être utilisées à des fins d'interprétation des résultats de patient. Tissu de contrôle positif Les contrôles doivent être des échantillons d'autopsie/de biopsie/de chirurgie récents fixés, traités et inclus aussi rapidement que possible de la même manière que le ou les échantillon(s) de patient. Les contrôles de tissu positifs indiquent si les tissus ont été préparés correctement et si des techniques de coloration appropriées ont été utilisées. Un contrôle de tissu positif pour chaque ensemble de conditions d'analyse doit être inclus dans chaque cycle de coloration. Les tissus utilisés pour les contrôles de tissu positifs doivent présenter une faible coloration positive pour pouvoir détecter des changements minimes de sensibilité de l'anticorps primaire. Les lames de contrôle fournies avec ce kit ou les échantillons traités différemment du ou des échantillon(s) de patient valident les performances des réactifs uniquement et non la préparation des tissus. Les contrôles de tissu positifs connus ne doivent être utilisés que pour vérifier les bonnes performances des tissus traités et des réactifs de test, et NON pour établir un diagnostic spécifique aux échantillons de patient. Si les contrôles de tissu positifs ne présentent pas la coloration positive appropriée, les résultats des échantillons à analyser doivent être considérés comme non valides. Les types de cellules normales pouvant être utilisées comme contrôle positif faible envers c-kit incluent les mélanocytes épidermiques basaux et les cellules myoépithéliales glandulaires de la peau, ainsi que les cellules épithéliales et myoépithéliales du sein. Des contrôles internes plus forts peuvent être des cellules neuronales (cellules interstitielles de Cajal) et les mastocytes de l intestin. Tissu de contrôle négatif Utiliser un tissu de contrôle négatif (connu pour être négatif à la protéine c-kit) fixé, traité et inclus de la même manière que le ou les échantillon(s) de patient avec chaque cycle de coloration pour vérifier la spécificité de l'anticorps primaire et fournir une indication de la coloration de fond spécifique. La variété des différents types cellulaires présents dans la plupart des coupes de tissu offre des sites de contrôle négatifs internes (cela doit être vérifié par l'utilisateur). Si une coloration spécifique se produit dans le tissu de contrôle négatif, les résultats des échantillons de patient doivent être considérés comme non valides. Les éléments tissulaires pouvant être utilisés comme contrôles négatifs incluent les myocytes cardiaques. Les cellules acinaires du pancréas sont également négatives à la c-kit, alors que l'épithélium du canal biliaire pancréatique présente une coloration positive. Réactif de contrôle négatif non spécifique Utiliser le réactif de contrôle négatif fourni au lieu de l'anticorps primaire avec une coupe de chaque échantillon de patient afin d'évaluer la coloration non spécifique et d obtenir une meilleure interprétation de la coloration spécifique au niveau du site de l'antigène. La période d'incubation du réactif de contrôle négatif doit correspondre à celle de l'anticorps primaire. Si des panels d anticorps sont utilisés sur des coupes en série, les zones sans coloration d une lame peuvent servir de contrôle négatif/de contrôle du bruit de fond non spécifique pour d'autres antisérums. Vérification du dosage Avant toute première utilisation d'un kit de coloration ou d un anticorps lors d'une procédure diagnostique, l'utilisateur doit vérifier la spécificité de l anticorps en le testant sur une série de tissus obtenus en interne dont les caractéristiques de performances IHC sont connues, représentant des tissus positifs et négatifs connus. Consulter les procédures de contrôle qualité mentionnées ci-avant dans la présente notice, ainsi que les exigences de contrôle qualité du Certification Program for Immunohistochemistry du CAP et/ou du document ( ) EFG_001 p. 19/40

20 Quality Assurance for Immunocytochemistry, Approved Guideline du CLSI. 17 Ces procédures de contrôle qualité doivent être reproduites pour chaque nouveau lot d anticorps ou dès lors qu un changement est apporté aux paramètres du dosage. Les tumeurs stromales gastro-intestinales (TSGI) avec des intensités de coloration à la protéine c-kit connues et des tissus négatifs conviennent pour la vérification du dosage. Tableau 1 : Objectif du contrôle qualité quotidien Tissu : Fixé et traité comme un échantillon de patient Lame de contrôle fournie par Dako Contrôle positif : Tissu ou cellules contenant l'antigène cible à détecter (peut être localisé dans le tissu du patient). Le contrôle idéal est un tissu présentant une faible coloration positive pour être plus sensible à l'anticorps ou à une dégradation de l'antigène. Contrôle négatif : Tissus ou cellules devant être négatifs (peuvent être localisés dans un tissu de patient ou un tissu de contrôle positif). Tissu de patient. Anticorps spécifique et kit de détection Contrôle la procédure de coloration uniquement. L'évaluation des lignées cellulaires des lames de contrôle fournies par Dako indique la validité du cycle de coloration. Contrôle toutes les étapes de l'analyse. Valide les réactifs et les procédures utilisés pour la coloration de la protéine c-kit. Détection d'une réactivité croisée inattendue de l anticorps avec les cellules/éléments cellulaires. Fond : Anticorps non spécifique (réactif de contrôle négatif de lapin) Détection d'une coloration de fond non spécifique. Détection d'une coloration de fond non spécifique. Détection d'une coloration spécifique. Détection d'une coloration de fond non spécifique. Interprétation de la procédure de coloration L'évaluation des lames doit être effectuée par un pathologiste à l'aide d'un microscope optique. Tous les examens doivent être réalisés dans la région tumorale de l'échantillon. Pour l'évaluation de la coloration immunocytochimique et de l'interprétation, un objectif à grossissement de 10X ou 20X est approprié. Utiliser des cellules intactes pour l'interprétation des résultats de coloration ; les cellules nécrotiques ou dégénérées se colorent souvent de manière non spécifique. 14,17 Des lignées cellulaires positives et négatives sont fournies avec chaque kit c-kit pharmdx pour valider les performances du dosage. Une coloration appropriée des lames de contrôle apporte la preuve que les réactifs c-kit pharmdx fonctionnent correctement et que le protocole a été réalisé avec exactitude. Aucune coloration de la lignée cellulaire de contrôle HT-29 (0) et une coloration cytoplasmique ou paranucléaire brune modérée dans la lignée cellulaire de contrôle P815 (2+) indiquent que le cycle de coloration est valide. Un écart par rapport à l'intensité de coloration de la lignée cellulaire de contrôle positif (trop faible ou trop forte) peut entraîner un faux positif ou un faux négatif et indique que le test doit être répété. Un examen du contrôle de tissu positif doit être réalisé. Une faible coloration des cellules myoépithéliales de la peau indique que les réactifs fonctionnent correctement. Si les contrôles de tissu positifs ne présentent pas de coloration positive appropriée, les résultats des échantillons à analyser doivent être considérés comme non valides. Le contrôle de tissu négatif doit être examiné après le contrôle de tissu positif pour vérifier la spécificité du marquage de l antigène par l anticorps primaire. L absence de coloration spécifique dans le contrôle de tissu négatif confirme l absence de réactivité croisée de l anticorps avec les cellules/composants cellulaires. Si une coloration spécifique se produit dans le contrôle de tissu négatif, les résultats de l'échantillon de patient doivent être considérés comme non valides. Lorsqu elle est présente, une coloration non spécifique présente généralement une apparence diffuse. Une coloration sporadique du tissu conjonctif peut également être observée dans des coupes provenant de tissus fixés au formol de manière excessive. Utiliser des cellules intactes pour l interprétation des résultats de coloration. Les cellules nécrotiques ou dégénérées se colorent souvent de manière non spécifique. 18 Examiner les échantillons de patients en dernier lieu. L intensité de la coloration positive doit être évaluée dans le contexte d une coloration de fond non spécifique du contrôle de réactif négatif. L interprétation de l expression de la protéine c-kit/cd117 doit être effectuée en tenant compte du contexte morphologique et clinique général de la tumeur. Les TSGI se divisent en trois catégories morphologiques : cellules fusiformes (70 %), épithélioïdes (20 %) et mixtes. 22 Quelle que soit leur morphologie, la majorité des TSGI exprime la protéine c-kit dans une proportion significative des cellules tumorales. Une coloration immunologique homogène avec le kit c-kit pharmdx est principalement visible dans le cytoplasme, avec ou sans schéma paranucléaire/appareil de Golgi (comme des points), et dans les membranes cellulaires, avec coloration périphérique complète ou incomplète. Certains cas présentent un schéma de coloration hétérogène (un mélange de coloration membranaire et/ou cytoplasmique fort ou faible).
