Gastroentorology, 118, OBJECTIF Le ydney wallow Questionnaire (Q) est un questionnaire auto-administré qui vise à évaluer la gravité de la dysphagie oropharyngée. Le Q a été développé pour la quantification des symptômes physiques (Wallace, Middleton, & Cook, 2000). PUBLIC CIBLE Le questionnaire a été validé chez des patients avec une dysphagie neuro-myogène oropharyngée et des survivants de cancers de la tête et du cou (Wallace et al., 2000 ; Dwivedi, t. Rose, Roe, Khan, Pepper, Nutting, et al., 2010). DECRIPTION Le questionnaire comprend 17 questions. A chaque question correspond une échelle visuelle analogique. Il s'agit d'une ligne horizontale, sans gradation, de 100 millimètres de long, allant de fonctionnement normal (à gauche) à dépréciation extrême (à droite). La personne place une marque

2 sur l endroit qui, selon lui/elle, décrit le mieux le degré de difficulté. Un score basé sur le nombre de millimètres est attribué à partir du début de l'échelle. Le score possible par question varie de 0 à 100. Le score maximum est de L évaluation visuelle analogique n'est pas utilisée pour la question 12. Pour cette question, les résultats s étendent de 0 à 5, correspondant au temps nécessaire pour la consommation d un repas ou d une crème glacée. Le score de la question 12 est multiplié par 20, ce qui génère à nouveau un score entre 0 et 100 (Wallace et al. 2000). FIABILITÉ La fiabilité réalisée par un test-retest a été calculée chez 18 patients. Les auteurs ont indiqué que cet échantillon était suffisant. Les scores pour les questions et le score total ont montré peu de différences (-2%) par rapport aux scores obtenus précédemment (95%, IC 11 % à 7 %)(Wallace et al., 2000). Le coeffficient de corrélation de pearman est élevé et calculé à 0.70 (p<0.0001) dans une autre étude (Dwivedi et al., 2010). Dans cette même étude, le coefficient alpha de Cronbach est calculé à 0,95 pour l'ensemble du questionnaire. VALIDITÉ La validité de contenu du questionnaire a été évaluée par 25 autorités internationalement reconnues et informées de la politique et de la recherche sur la dysphagie. En fin de compte, les experts ont déterminé que trois questions avaient peu d importance. Une de ces trois questions a été néanmoins été retenue, car elle est un facteur très important dans la «dysphagie» (identifié par l'analyse factorielle). Cette analyse a été utilisée pour évaluer la validité de contenu chez les patients atteints de dysphagie oropharyngée, afin de déterminer quelles questions ont montré une cohérence. Le facteur "Dysphagie" explique 56 % de la variance et augmente jusqu à 59 % après l'omission des deux questions définies comme ayant peu importance par les experts (Wallace et al 2000). Dans une étude récente (Dwivedi et al, 2010), la validité de contenu de la Q a été évaluée par une équipe multidisciplinaire. La validité du construit a été recherchée pour le score total du questionnaire par rapport à une note d'évaluation globale (basée sur un examen physique, les antécédents et une vidéofluoroscopie). Il y avait une corrélation significative (r = 0,69 ; p<0,0001) (Wallace et al, 2000). Dans l'étude de Dwivedi et al (2010), 3 questions de la Q et le MD Anderson dysphagie Inventaire (MDADI) ont été mis en concurrence afin de vérifier la validité. Le MDADI a été utilisé comme mesure-étalon et se compose d'un domaine général, affectif, physique et fonctionnel. Le MDADI est une échelle d'auto-évaluation de la déglutition associée à la qualité de vie des patients avec cancer de la tête et du cou. Les coefficients de corrélation de pearman ont été entre 0,46 et 0,77 (Dwivedi et al, 2010). Tant le score

3 total sur le questionnaire que la note d'évaluation globale (basée sur un examen physique, les antécédents et une vidéofluoroscopie) ont diminué de 70 % et 89 % (p<0,0001) après que les patients dysphagiques aient subi une opération (validité discriminante) (Wallace et al, 2000). Il y avait des différences significatives entre les patients en fonction de la localisation de la tumeur, de la comorbidité et du suivi. La validité concurrente a été examinée pour le domaine physique et général du MDADI et montre des résultats comparables à la Q. Les coefficients de corrélation de pearman se situent entre 0,64 et 0,83 (Dwivedi et al., 2010). CONVIVIALITÉ Dwivedi et al. (2010) ont indiqué que le Q est un questionnaire court et précis pour évaluer la fonction de déglutition. 5 Results reliability Results validity Commentary Test retest reliability The second inventory was completed 1-2 weeks after the first completion (18 patients within group 1). The second scores for individual questions and the total score did not differed significantly from the first scores. The total score showed a mean variation of - 2% (95% CI, -11% -7%). FV Twenty-five authorities (out of 32) who are internationally recognized in dysphagia management and research participated in an inventory. They rated the relevance of each question to the assessment of the severity of oropharyngeal dysphagia from low to high. If more than 30% of the raters classified the question as having low relevance, the question was defined as having low face validity. Three questions were classified as having low importance. Patient compliance was defined as poor if >5% of the patients failed to answer the question. CV Factor analysis A factor analysis identified 4 factors that accounted for 75% of the total variance. 18 questions contributed significantly to the variance of 1 factor: dysphagia. This factor accounted for 56 % of the total variance (p< ) and increased to 59% after deletion of 2 questions with poor patient compliance and face validity. Factor loadings for each question in factor 1 ranged from (17 questions). CrV Predictive validity The mean total score of patients in the control group were significantly lower in comparison with the neuromyogenic dysphagia group and the group with dysphagia, as a result of Zenker s diverticulum. A high correlation (r=0.69; p< ) was found for the total score in comparison with an independent global assessment severity score (based on history and physical examination and a videofluoroscopy by a clinician; the global score ranged from 0-10). Discriminant validity After surgery in patients (9 patients) with Zenker s diverticulum, the total score reduced with 70% (p<0.0001). The postoperative global assessment score (clinician) decreased with 89% (p<0.0001). In the present study, a global assessment score was used as gold standard, because a gold standard to measure symptom severity was lacking. The inventory was validated for neuromyogenic dysphagia patients, which had implications to the generalizability to other patient groups. The clinician who assigned the global assessment score was blinded to the subsequent inventory scores. There were significant differences when patients were grouped by tumor location, follow-up and co-morbidity. CrV pearman s rank correlation coefficients were calculated by comparing the swallowing related QOL, the total and the general domain of the Q with the physical (total Q: 0.83 and general Q: 0.71) and the general domain (QOL-Q: 0.78, total Q: 0.72 and general Q: 0.64) of the MDADI.
