ET L ANTIBIO Recommandations SFED 2008 PROPOSITIONS D ANTIBIOPROPHYLAXIE EN CAS DE PROCÉDURE À HAUT RISQUE (EN DEHORS DES PATIENTS À HAUT RISQUE CARDIAQUE D ENDOCARDITE) 1. Avant une cholangiographie rétrograde endoscopique 750 mg ciprofloxacine orale 60-90 minutes avant le geste ou céphalosporine de 3e génération type céfotaxime, voire uréidopénicilline par voie parentérale. 2. Avant une gastrostomie percutanée endoscopique, une sclérose de varices œsophagiennes ou une ponction sous échoendoscopie à risque 1 g amoxicilline/acide clavulanique (perfusion de 30 minutes) ou 1 g céfotaxime parentéral 30 minutes avant la procédure. 3. Chez les patients neutropéniques, le métronidazole doit être ajouté à la prophylaxie décidée sauf en cas d utilisation d un inhibiteur des bêta-lactamases (acide clavulanique ou tazobactam) Pas de mention spécifique endocardites
ET L ANTIBIO Recommandations EUA (urologues) 2011 Only transrectal core prostate biopsy (LE: 1b, GR: A) and transurethral resection of the prostate (TURP) (LE: 1a, GR: A) are well documented. There is no evidence for any benefits of antibiotic prophylaxis in standard non-complicated endoscopic procedures and extracorporeal shockwave lithotripsy (ESWL), although it is recommended in complicated procedures and patients with identified risk factors. For open and laparoscopic surgery, the same rules as in abdominal and gynaecological surgery can be applied. No antibiotic prophylaxis is recommended for clean operations, whereas a single or 1-day dose is recommended in clean-contaminated/contaminated operations. Opening of the urinary tract is considered as clean-contaminated surgery. A single dose or a short course of antimicrobials can be given parenterally or orally. Recommandations EHS/EACTS 2012 3.2 Endocarditis prophylaxis ET L ANTIBIO The indication for antibiotic prophylaxis has been significantly reduced in the recent ESC guidelines. Antibiotic prophylaxis should be considered for high-risk procedures in high-risk patients, such as patients with prosthetic heart valves or prosthetic material used for valve repair, or in patients with previous endocarditis or congenital heart disease according to current ESC guidelines. However, the general role of prevention of endocarditis is still very important in all patients with VHD, including good oral hygiene and aseptic measures during catheter manipulation or any invasive procedure, in order to reduce the rate of healthcare associated infective encdocarditis. Recommandations bucco-dentaires, AFSSAPS 2012 Antibiothérapie prophylactique -est recommandée selon le risque infectieux du patient et l acte invasif pratiqué ; - est instaurée pour limiter un risque d endocardite infectieuse ou pour limiter un risque d infection locale et son extension éventuelle ; - est recommandée, chez le patient à haut risque d endocardite infectieuse, pour tout acte dentaire impliquant une manipulation de la gencive (p. ex. le détartrage) ou de la région périapicale de la dent et en cas d effraction de la muqueuse orale (exceptée l anesthésie locale ou locorégionale) ; Reprise des recommandations ESC 2009
