However, questions and concerns about the risk and rewards of these new techniques and therapies continue to be raised. Immediate implant placement in the aesthetic zone in particular continues to be controversial. This presentation will address current concepts, techniques, clinical research, histologic evidence, and innovations relating to immediate post-extraction implant placement and provisional restoration. Ce critère reste prépondérant mais de nouvelles exigences sont apparues parmi lesquelles l intégration esthétique des restaurations implanto-portées dans le secteur antérieur. Cette intégration esthétique, particulièrement délicate à obtenir, reste principalement sous la dépendance de deux critères : un positionnement tridimensionnel adapté de l implant ainsi que la qualité et la quantité des tissus mous péri implantaires. Les techniques chirurgicales ayant pour but l amélioration des résultats esthétiques ont-elles une incidence sur les futures complications biologiques qui peuvent apparaître autour des implants et notamment la péri-implantite? Nous déterminerons la conduite à tenir dans un secteur esthétique à l aide des connaissances scientifiques actuelles et la manière d en évaluer le possible impact biologique sur le long terme. Evaluation of the success of implant therapy for many years was based on osseointegration alone. This criterion still predominates, but new requirements have emerged, one of which is the aesthetic integration of implantsupported restorations in the anterior region. Such aesthetic integration is challenging to achieve and still primarily depends on two criteria: the correct three-dimensional positioning of the implant and the quality and quantity of the peri-implant mucosal tissue. This presentation will focus on current scientific knowledge supporting the best approaches to treating the aesthetic region. Ils doivent être réalisés en un temps très court, être performants pour assurer la contention des implants pendant la phase d ostéo- intégration et apporter un confort immédiat aux patients. Le plus souvent, l aspect cosmétique de ces bridges et leur fonctionnalité passent au second plan en regard des objectifs précités. Aujourd hui, grâce à la chirurgie guidée, nous pouvons optimiser ces bridges provisoires et nous approcher ainsi du projet prothétique final et valider ainsi nos options thérapeutiques en situation réelle pendant la phase d ostéo-intégration. Immediate loading of full arch restorations remains a challenge for the treatment team. It must be carried out in a very short time period, while successfully protecting the osseointegrating implants and bringing immediate comfort to patients. More often than not, the cosmetic appearance and functionality of such restorations are subordinate concerns. Today, however, guided surgery enables the fabrication of provisional restorations to be optimized, bringing clinicians a step closer to achieving their ultimate prosthetic goals for patients. Toutefois dans le secteur antérieur, l obtention d un résultat esthétique stable dans le temps, reste un challenge difficile. L intégration esthétique des prothèses implanto-portées, passe par le développement et la stabilisation d un profil d émergence adéquat. Celui-ci est dépendant à la fois de paramètres chirurgicaux et prothétiques. La position de l implant doit être idéale et obéit à des règles précises. Un volume osseux est systématiquement développé pour permettre un bon positionnement des implants mais aussi assurer le soutien de l architecture gingivale. L utilisation de piliers implantaire temporaires et définitifs avec des profils transgingivaux très concaves préserve la vascularisation et l épaisseur de la gencive vestibulaire. Nos choix de stratégies chirurgicales du secteur antérieur seront abordés chronologiquement et nous évoquerons des concepts assez récents et prometteurs tels que «le socket shield». The success of implant osseointegration is well established, and implant systems continue to improve. Nevertheless, in the anterior region, obtaining a stable aesthetic outcome over time remains a difficult challenge. The aesthetic integration of implant-supported prostheses requires the development and stabilization of a suitable emergence profile. This in turn is affected by both surgical and prosthetic parameters. Achieving ideal implant positioning requires attention to specific rules. Bone volume must be systematically developed to allow for correct implant positioning and ensure the support of the gingival architecture. The use of provisional and definitive implant abutments with concave transgingival profiles can preserve the vascularization of the vestibular soft tissue, as well as its thickness. This presentation will systematically review surgical strategies for the anterior region. Dr Giancarlo BIANCA Avec un traitement implantaire conventionnel, le patient va vivre un grand inconfort et une exclusion sociale durant plusieurs semaines, liés au nombre d interventions et à l impossibilité de porter immédiatement la prothèse adjointe totale. De plus, cette prothèse amovible entraîne des risques biologiques, en augmentant le taux d échec implantaire ainsi que le nombre de péri-implantites. Par la perte tissulaire que cette prothèse génère dans le sens vertical, elle peut compromettre le résultat esthétique naturel des prothèses fixes supraimplantaires. Le protocole d extraction, d implantation et de pose d un bridge provisoire immédiat va résoudre en une seule séance ces problèmes et améliore nettement la prévisibilité esthétique. La réalisation d une prothèse fixe immédiate représentant le projet prothétique final permet de préserver et guider la cicatrisation tissulaire, tout en maintenant l intégrité esthétique et fonctionnelle des patients. PROGRAMME SAMEDI 28 MARS / SATURDAY 28 MARCH With conventional implant treatment, patients typically experience significant discomfort and social isolation for several weeks as a result of the number of interventions required and the impossibility of immediately wearing a dental prosthesis. Use of a removable prosthesis poses biological risks and may ultimately result in implant failure. Vertical tissue loss caused by such prostheses may prevent achievement of a natural aesthetic outcome. Tooth extraction followed by immediate placement of implants and delivery of a provisional prosthesis can help resolve these issues in a single sitting and significantly improve the aesthetic outlook. This presentation will explore the implementation of an immediate fixed prosthesis to foster better tissue healing while maintaining patients aesthetic and functional integrity. 11.30 12.30 Les implants courts et ultra-courts : une approche simplifiée dans le traitement implantaire de sites osseux réduits/short and Extra-short Implants: a Simplified Approach to Implant Therapy in Reduced Bone Sites Dr Philippe SEBBAG Le remplacement des dents postérieures constituent un challenge thérapeutique en raison de nombreuses difficultés auxquelles le praticien est confronté. La limitation du volume osseux disponible a longtemps empêché une implantation simple. Les greffes osseuses de la partie basse du sinus par os autogène puis biomatériaux de régénération osseuse ont permis de recréer des conditions anatomiques favorables. Toutefois ces techniques présentent des risques opératoires et constituent un investissement financier important pour le patient. La fiabilité de l ostéo-intégration a permis d évoluer vers des implants ultra-courts inférieurs à 8,5 mm. Dans ce cas précis, il est possible d optimiser des volumes osseux encore plus réduits, évitant des greffes lourdes ou bien de s exposer à des risques opératoires, limitant ainsi coût et morbidité. Cette présentation illustrera quelques-unes des indications des implants ultra-courts. The replacement of posterior teeth constitutes a therapeutic challenge for various reasons. Limited available bone volume often complicates implant placement. It may be possible to recreate favorable anatomical conditions by regenerating the bone with autogenous and/or alternative biocompatible bone graft material. However, such techniques pose surgical risks and require a significant financial investment from the patient. As an alternative, short (less than 8.5mm-long) implants have been developed. These can be placed in sites where the bone volume has been significantly reduced, while avoiding the trauma and surgical risks of bone-grafting procedures and limiting costs and morbidity.
