Obert Introduction La fixation pérenne des tubérosités autour d un implant huméral pour fracture dépend d abord de la hauteur de la tige : celle-ci peut être placée de façon reproductible grâce au repère du tendon du grand pectoral. L ostéosuture demeure l outil indispensable pour amarrer les tubérosités. Le principe des fils boucles Cette fracture est la seule à être fixée par ostéosuture. Idéalement un fil boucle pourra avantageusement remplacer les fils simples. L intérêt de la boucle est double S agissant d une chirurgie tendineuse, la quantité de fil est un élément de résistance, mais le fil boucle, utilisé en chirurgie des fléchisseurs, permet de réaliser un nœud coulant simple et efficace, pour un amarrage fiable des tubérosités. Des tests cadavériques (24 épaules - 12 cadavres) ont été réalisés avec fabrication de boucle pour réaliser les sutures de tubérosités. A chaque test, il a été vérifié la rapidité de la technique, sa reproductibilité, l intérêt de la boucle et la résistance du montage. Trois systèmes de 2 fils boucles nous semblent importants : - L un pour amarrer et tracter les tubérosités, l autre pour les plaquer à l implant, le dernier groupe de 2 fils pour un système de haubanage vertical (fig. 1 et 2). Par ailleurs, le dessin de la tige au niveau métaphysaire risque de minimiser la stabilité primaire, les tubérosités pouvant glisser en capotant avec la tension de suture. - L autre intérêt est de réaliser un nœud très simple autostable (autorisant une mobilisation de la fracture fixée autour de l implant, et facile à modifier, à défaire et à refaire. Ce nœud est le nœud de tête d alouette (fig. 3 à 10). Ce nœud est connu et utilisé depuis longtemps mais pas en chirurgie. Utilisé avec le système de fils de plaquage des tubérosités à l implant (fils bleus) un cliché radiographique permet de vérifier la bonne position des tubérosités. Osteosuture remains the essential tool for anchoring tuberosities. The looped thread principle This fracture is the only one that can be fixed with osteosutures. Ideally a looped thread can advantageously replace single threads. The benefit of the loop is double With this being tendon surgery, the quantity of suture is known to be an element of resistance, but a looped thread, used in flexor surgery, allows us to make a series of running knots, which are simple and effective, for reliable anchoring of the tuberosities. Cadaver studies (24 shoulders - 12 cadavers) have been carried out with constructed loops to perform tuberosity suturing. For each test, the speed of the technique, its reproducibility, the reliability of the loop and the strength of the mounting were checked. Three systems of 2 looped threads seem valuable to us: - The first to anchor and draw the tuberosities, the next to press them onto the implant, and the final group of two threads to create a vertical tie-down system (fig. 1 and 2). Moreover, the design of the stem at the metaphyseal level risks reducing the primary stability, and the tuberosities may slide «turning over with the tension of the suture. - The other advantage is to create a very simple self-stabilising knot, allowing mobilisation of the fracture fixed around the implant, and which is easy to change, undo and re-tie. This knot is the lark s head hitch knot (fig. 3 to 10). This is a wellknown knot that has been used for a long time just not in surgery. Used with a threading system to press the tuberosities onto the implant (blue threads), an x-ray image can be used to check that the tuberosities are well-positioned. 45

Avant serrage, on peut encore positionner le nœud où bon nous semble (5). On écarte les brins pour serrer le nœud (6). On coupe un des brins (7) on peut bloquer le nœud, soit par des demi-clés qui fixent la tête d alouette (8). Soit en passant sous la boucle avec le blocage par demi-clés ensuite (9 et 10).
