2 CHAPTER 7 Packaging and Labelling Annex H STATEMENT OF COMPLIANCE FOR PRINTERS OF PACKAGING MATERIALS AND LABELS BEARING THE MEAT INSPECTION LEGEND AND MANUFACTURERS OF STAMPS BEARING THE MEAT INSPECTION LEGEND I,.. (name)..... (position in Printing Company) request permission on behalf of (name of Printing Company) or (name and number of registered establishment doing the printing) (address - street, city, province, postal code) to reproduce the meat inspection legend. I understand that: the meat inspection legend is a national trade mark and that unauthorized use of that national trade mark is subject to prosecution under section 21 of the Meat Inspection Act; I shall deliver exclusively to establishments registered under the Meat Inspection Act, to the attention of the Veterinarian/Inspector In Charge, all labels, containers and other items bearing the meat inspection legend; in the case of stamps* and export stickers**, they will be delivered directly to the appropriate Area Executive Director unless there is a specific agreement to deliver them directly to the Veterinarian/Inspector in Charge; that the permission for the above firm to reproduce the meat inspection legend may be withdrawn for non-compliance with the prescribed requirements; and I shall inform the CFIA if I change my company s name, my address or if I close the Printing Company listed above. Please check as applicable which of the following product(s) you will be manufacturing: 1. Printers (legend reproduced on labels, containers, packaging material, etc.) 2. (nom) (titre dans la société) demande l'autorisation au nom de (nom d entreprise d impression) ou (nom et numéro de l établissement agrée qui imprime) (adresse - rue, ville, province, code postal), de reproduire l'estampille réglementaire de l'inspection des viandes. Je reconnais que : cette estampille est une marque de commerce nationale et que toute personne qui l'utilise sans autorisation est passible de poursuite aux termes de l'article 21 de la Loi sur l'inspection des viandes; je m'engage à envoyer toutes les étiquettes, tous les contenants et tout autre article portant l'estampille exclusivement à des établissements agréés en vertu de la Loi sur l'inspection des viandes, aux soins au vétérinaire/'inspecteur de service; les étampes* et les étiquettes d'exportation** seront envoyées directement au directeur exécutif du centre opérationnel approprié, à moins qu'une entente particulière ait été conclue pour qu'elles soient envoyées directement au vétérinaire/inspecteur de service; l'autorisation de reproduire cette estampille accordée à l'entreprise mentionnée ci-haut peut lui être retirée si elle ne respecte pas les exigences prescrites; et je m engage à informer l ACIA si je fais des changements de nom, d adresse ou si je ferme l entreprise mentionnée ci-haut.
