Présentation Prése/ du cas clinique / Clinical case presentation Patiente de 44 ans ne présentant ni facteurs de risques ni antécédents dentaires pour la mise en œuvre d interventions chirurgicales. Les dents 26 et 27 étant détériorées, il a été décidé de les extraire et de les remplacer par des implants. Deux interventions vont être pratiquées afin de pallier à la perte osseuse. Dans un premier temps, un comblement post-extractionnel est effectué accompagnant la mise en place du premier implant. Dans un second temps, un sinus lift est réalisé pour la mise en place du second implant. ----------------------- Woman of 44 years old having no risk factors nor dental history for the implementation of surgeries. Teeth 26 and 27 are deteriorated, it was decided to extract and replace them with implants. Two surgeries will be performed in order to fill the bone loss. First, a post-extraction filling is performed with the first implant placement. Second, a sinus lift is realized with the second implant placement. 2

La dent 26 est délabrée. Au niveau de la 27 noter la présence de débris radiculaires sous muqueux. ---------------------------- Preoperative view (crowns were lost). The tooth 26 is decayed. There are submucosal root residues at the tooth 27. Prélèvement du sang à l aide d une seringue après décollement du lambeau et avant vasoconstriction. ------------------------- Blood is collected using a syringe after flap detaching and before vasoconstriction. 5

Parfaite adaptation au site. ----------------- Hydrated COVA MAX membrane is easily placed. Perfect adaptation to the site. Incision périostée du lambeau afin de gagner en élasticité puis réalisation de sutures hermétiques. ---------------------- A periosteal incision of the flap is performed to gain elasticity followed by hermetic suturing. 10
1/ Comblement alvéolaire post-extractionnel + Pose premier implant/ Post-extraction socket filling + First implant placement Radiographie panoramique postopératoire immédiate. La zone comblée semble dense et homogène (on distingue peu les traces des alvéoles). MATRI BONE a permis le rétablissement de continuité de la crête osseuse. Une élévation du plancher sinusien pourra donc être envisagée 4 mois après le comblement. --------------------- Postoperative panoramic X-Ray showing good density and homogeneity of the filled region (no visible marked sockets). Filling with MATRI BONE allowed ridge correction. A sinus-lift procedure may be plan after 4 months. 11

2/ Elevation de sinus + Pose second implant/ Sinus lift + Second implant placement Dans un premier temps, réalisation d un volet osseux au piézotome. Bonne vascularisation du site préalablement reconstruit au niveau des dents 26 et 27. Dans un second temps, éviction du volet osseux. --------------------- In the first step, a bone flap was made using piezotome. Good vascularization at the surgical site previously filled at level of teeth 26 and 27. In a second step, the bone flap was removed. 14

2/ Elevation de sinus + Pose second implant/ Sinus lift + Second implant placement MATRI BONE de petite granulométrie (30 x 30 x 6 mm, 80-200 µm) sera utilisé et préalablement imprégné avec le sang du patient avant le découpage en morceaux. --------------------- Small granulometry MATRI BONE (30 x 30 x 6mm, 80-200 µm) will be used and previously soaked with the patient s blood before being cut into pieces. 16
