Intrathecal Trastuzumab HER2 is overexpress in 30% of primary BC 29% of patients with HER2 + BC will relapsed in the brain, but LM involvement still a relatively rare manifestation of HER2 (3-5%). (Nam 2008, Bendell 2003, Brusky 2008Lin 2010) High level of concordance in HER2 status between primary tumors and metastatic cancer cells in the CSF (Park 2010 ) The CSF/serum trastuzumab ratio increased from 0.0023 prior to brain radiotherapy to 0.013 after completion of radiotherapy and was as high as 0.02 with concomitant LM after radiotherapy. IT trastuzumab has been successfully tested on several animal models. A limited number of administrations in humans have been described in the literature, with weekly doses up to 100 mg. Rationnel de la CT IT Généralement les agents systémiques ont une mauvaise diffusion méningée et n atteignent pas des taux IT thérapeutiques Quelques produits administrés à forte dose en systémique (dont le MTX, la cytarabine ou le thiotepa) peuvent avoir des taux IT suffisants, mais au prix d une toxicité importante et d une difficulté à les associer à un traitement systémique du primitif
