1 Éducation du patient et de sa famille Hibiclens Bathing and Antibiotic Ointment Instructions / French Instructions relatives au lavage au savon Hibiclens et à l utilisation de pommade antibiotique Suivez les instructions ci-contre pour aider à prévenir les infections du site opératoire. Que faisons-nous pour prévenir les infections du site opératoire? De nombreuses précautions sont prises pour prévenir les infections, y compris: tout le personnel de la salle d opération doit se laver les mains et les bras jusqu aux coudes juste avant l opération ; des antibiotiques sont administrés à votre enfant avant l opération, pour certaines interventions ; la peau de votre enfant sur le site opératoire est nettoyée avec un savon spécial qui tue les germes. Comment puis-je contribuer à prévenir les infections du site opératoire? La peau de votre enfant doit être aussi exempte de germes (bactéries) que possible avant l opération ou l intervention. En lavant sa peau avec un savon spécial appelé Hibiclens (gluconate de chlorhexidine), vous pouvez grandement réduire le nombre de germes qui y résident. Cela peut contribuer à prévenir les infections. Il est possible que certains patients doivent utiliser une pommade antibiotique appelée Bactroban (mupirocine) dans les narines. Hibiclens et Bactroban sont des médicaments. Veuillez ne les utiliser que conformément aux instructions de votre médecin. Suivez les instructions cochées ci-dessous. Lavage au savon Hibiclens le soir précédant l opération Aidez votre enfant à se doucher ou à se baigner au savon Hibiclens le soir précédant l opération. Suivez les instructions figurant sur ce document. Bain ou douche à donner le (jour) Lavage au savon Hibiclens et utilisation de pommade antibiotique Cinq jours avant l opération, appliquez la pommade Bactroban (mupirocine) dans les narines 2 fois par jour pendant 5 jours. Suivez les instructions figurant sur ce document. Deux jours avant l opération, aidez votre enfant à se doucher ou à se baigner au savon Hibiclens chaque jour. 2 Instructions relatives au lavage au savon Hibiclens et à l utilisation de pommade antibiotique Lavage au savon Hibiclens et utilisation de pommade antibiotique (en cas d antécédents d infection au SARM) Aidez votre enfant à se doucher ou à se baigner au savon Hibiclens chaque jour durant les 5 jours précédant l opération. Suivez les instructions figurant sur ce document. Appliquez la pommade Bactroban (mupirocine) dans les narines 2 fois par jour pendant les 5 jours précédant l opération. Suivez les instructions figurant sur ce document. Bains ou douches et applications de pommade à commencer le (jour) Étapes à suivre pour le bain ou la douche Hibiclens est le nom de marque d un savon appelé gluconate de chlorhexidine (GCH). N utilisez pas Hibiclens si votre enfant y a déjà été allergique (s il a présenté une irritation cutanée ou un œdème). Si vous égarez votre savon Hibiclens, vous pouvez vous en procurer dans la plupart des grandes pharmacies. Veuillez suivre les instructions suivantes pour nettoyer la peau de votre enfant : 1. Dans la douche ou la baignoire, commencez par un lavage du corps à l eau et au savon habituel. Lavez les cheveux comme d habitude avec le shampoing habituel. 2. Rincez abondamment le corps et les cheveux pour enlever toute trace de savon et de shampoing. Centre chirurgical 2 sur 4

Videz la baignoire ou sortez votre enfant de l eau. 4. À l aide d un gant de toilette propre, appliquez le savon Hibiclens sur tout le corps de votre enfant en commençant par le cou. N utilisez pas Hibiclens au-dessus du cou, en particulier sur le visage, à proximité des yeux ou des oreilles, car cela pourrait entraîner des lésions permanentes dans ces zones. 5. Savonnez délicatement le corps de votre enfant pendant 1 minute en faisant particulièrement attention à la zone sur laquelle aura lieu l intervention ou l opération (sauf si votre enfant est opéré à la tête). Lavez toute la zone grisée. N appliquez pas de savon au-dessus du cou. 6. Rouvrez le robinet et rincez abondamment le corps de votre enfant. Ne relavez pas votre enfant au savon habituel après avoir utilisé le savon Hibiclens. Centre chirurgical 3 sur 4

Essuyez délicatement votre enfant avec une serviette propre et douce. 8. N appliquez ni lotions, ni poudres, ni huiles sur sa peau après le lavage. 9. N utilisez ni maquillage ni déodorant après le dernier lavage au savon Hibiclens la veille de l opération. Étapes à suivre pour l application de la pommade Bactroban (mupirocine) 1. Mettez une petite quantité de pommade antibiotique Bactroban (mupirocine) au bout d un coton-tige. 2. Appliquez le médicament tout autour de l intérieur d une narine sans aller trop profondément. Seule l extrémité du coton-tige doit être introduite dans la narine. 3. Mettez ensuite de la pommade sur l autre extrémité (non utilisée) du coton-tige et appliquez le médicament à l intérieur de l autre narine. Seule l extrémité du coton-tige doit être introduite dans la narine. 5 Patient and Family Education Hibiclens Bathing and Antibiotic Ointment Instructions Follow these instructions to help prevent surgical site infections. What are we doing to prevent surgical site infections? We take many precautions to prevent infections, including: Having all operating room staff clean their hands and arms up to their elbows right before surgery Giving your child antibiotics, for some procedures, before the surgery Cleaning your child s skin at the surgery site with special soap that kills germs How can I help prevent surgical site infections? Your child s skin needs to be as free of germs (bacteria) as possible before their surgery or procedure. Washing the skin with a special soap called Hibiclens (chlorhexidine gluconate) can greatly reduce the number of germs on the skin. This can help prevent infection. Some patients may need to use an antibiotic ointment called Bactroban (Mupirocin) in the nostrils. Hibiclens and Bactroban are medicines. Use them only as instructed by your healthcare provider. Follow the checked instructions below. Bathe with Hibiclens the night before surgery Help your child shower or bathe with Hibiclens soap the night before the surgery. Follow the instructions in this handout. Give the bath on (day) Bathe with Hibiclens and use antibiotic ointment Starting 5 days before surgery, apply Bactroban (Mupirocin) ointment in the nostrils 2 times a day for 5 days. 6 Hibiclens Bathing and Antibiotic Ointment Instructions Bathe with Hibiclens and use antibiotic ointment (History of MRSA infection) Help your child shower or bathe with Hibiclens soap daily for 5 days before the surgery. Follow the instructions in this handout. Apply Bactroban (Mupirocin) ointment in the nostrils 2 times a day for 5 days before the surgery. Follow the instructions in this handout. Start the baths and ointment on (day) Bathing or showering steps Hibiclens is a brand name for a soap called chlorhexidine gluconate (CHG). Do not use Hibiclens if your child has had an allergy to it such as a rash or swelling. If you misplace your packet, you can get Hibiclens soap at most large pharmacies. Please follow these instructions for cleaning your child s skin: 1. In the shower or tub, wash the body with regular soap and water first. Wash the hair as usual with your normal shampoo. 2. Rinse the hair and body really well to remove all soap and shampoo. 3. Turn water off to avoid rinsing the Hibiclens off too soon. Drain the tub or remove your child from the water. 7 Hibiclens Bathing and Antibiotic Ointment Instructions 4. Using a clean washcloth apply the Hibiclens soap to your child s entire body from the neck down. Do not use Hibiclens above the neck, especially on the face, near the eyes or ears to avoid permanent injury to those areas. 5. Wash the body gently for 1 minute, paying special attention to the area where the surgery or procedure will be done (unless your child is having surgery on their head). Wash all of the body shaded in gray. Do not wash above the neck. 6. Turn the water back on and rinse the body really well. Do not wash with regular soap after the Hibiclens is used. 7. Pat your child dry with a clean, soft towel. 8. Do not put lotions, powders or oils on your child s skin after bathing. 9. Do not use make up or deodorant after the last Hibiclens bath the night before surgery. Surgery Center 3 of 4

From outside the hospital, call the toll-free Family Interpreting Line Tell the interpreter the name or extension you need. Bactroban (Mupirocin) Ointment Steps 1. Put a small amount of Bactroban (Mupirocin) antibiotic ointment on one end of a cotton-tip swab (Q-tips). 2. Apply the medicine all around the inside of one nostril, not going too deeply. Only the cotton tip should go in the nostril. 3. Then, put the ointment on the other (unused) end of the cotton-tip swab, and apply the medicine inside the other nostril. Only the cotton tip should go in the nostril.
