La qualité de l os spongieux et l indice cortical sont déterminants dans le choix de la tige. La tige fémorale VECTRA convient à : - une implantation de première intention, - une implantation de reprise, lorsque le capital osseux de la partie proximale est suffisant. Indications VECTRA Femoral Stems are available in various shapes, sizes, materials and coatings, to address a variety of applications. Quality of cancellous bone and cortical index are crucial factors in selection of the appropriate stem. VECTRA Femoral Stems are indicated for : - primary cases - revision cases with sufficient proximal bone stock Contraindications Revision cases with extensive metaphyseal bone loss. 3

4 Description du système Fact sheet La tige fémorale VECTRA a été développée en 1987 à partir de la tige fémorale droite autobloquante de Maurice E. Müller. Sa géométrie améliorée permet un excellent ancrage dans la partie métaphysaire et le comblement optimal du canal médullaire. La tige VECTRA peut être implantée avec ou sans ciment. The VECTRA Femoral Stem was developed in 1987; it was modeled after the self-locking straight stem designed by Maurice E. Müller. Its improved geometry provides strong anchorage in the metaphysis and optimal fit and fill of the femoral canal. The VECTRA Femoral Stem can be implanted with or without cement. Excellent ancrage dans la partie métaphysaire La tige VECTRA se distingue par sa forme conique proximale prononcée dans les plans frontal et sagittal. Grâce à elle, l'ancrage de la tige intervient principalement dans la métaphyse. De plus, les efforts extérieurs sont transmis sous forme de contraintes de compression et non de cisaillement. La transmission des forces se fait donc de manière physiologique. Afin d'éviter tout risque de blocage diaphysaire et donc de stress shielding, la partie distale de la râpe est légèrement surdimensionnée. On obtient ainsi un espace entre la tige et l'os cortical. NB : Dans la version cimentée, le ciment assure uniquement un rôle de remplissage. Il ne modifie pas le positionnement de la tige, ceci évitant les risques de mauvais positionnement. Strong metaphyseal anchorage The proximal double taper (in the coronal and sagittal planes) of the VECTRA Femoral Stem provides superior fixation in the metaphyseal region, and allows conversion of forces into compressive loads (not shear stresses). This results in a more physiological transfer of loads. The distal part of the rasp is slightly oversized. The tiny gap created between the cortical bone and the stem prevents distal jamming and its resulting stress shielding phenomenon. Note : In the cemented design, cement is just a filling material. It does not influence the position of the femoral stem, which avoids the potential risk of malpositioning. Adaptation idéale à la morphologie fémorale La tige fémorale VECTRA existe en version standard et en version latéralisée avec un offset supérieur de 7mm, ce qui permet une bonne adaptation à l'angle CCD (cas de coxa vara ou de fémur de grande taille). En outre, la version latéralisée offre la possibilité de rétention musculaire, réduisant le risque de luxation post opératoire. Addressing a wide morphological spectrum The VECTRA Femoral Stem is available in both standard and lateralized versions (high offset +7mm, allowing optimal adjustment to the neck-shaft angle in patients with coxa vara or large femurs). Furthermore, the lateralized version permits muscle tightening, thus reducing the potential for postoperative dislocation. 4

5 Grande amplitude de mouvement La conception affinée du col améliore l'amplitude de mouvement et diminue le risque de conflit entre le col et l'implant cotyloïdien. Increased range of motion Thinner neck results in increased range of motion and reduced potential for impingement between the prosthetic neck and the acetabular component. Planification préopératoire simple L'échelonnement des tailles permet un ajustement précis de la tige à l'os. La longueur de col, la conicité distale de 5 et la distance de la corticale médiale au centre de tête fémorale sont constantes. Simple preoperative planning Progressive increase in sizes allows the surgeon to select the stem that best fits the patient's femur. Neck length, 5 distal taper, and distance from the medial cortex to the centre of the femoral head are the same for all stem sizes. Grande flexibilité per-opératoire Selon la qualité du stock osseux, le chirurgien pourra choisir pendant l'opération d'implanter une tige VECTRA cimentée ou non cimentée. De plus, le jeu de râpes est commun aux tiges non cimentées et cimentées, l'implant cimenté étant légèrement sous dimensionné. Les râpes peuvent servir également d'implants d'essai. L'instrumentation est réduite au minimum. High intraoperative flexibility The surgeon can decide intraoperatively according to the quality of bone stock, whether a cemented or uncemented stem should be used. Rasps are the same for cemented and uncemented stems, since the cemented implant is slightly undersized. Rasps can also be used as trial stems. The number of instruments is reduced to a minimum. Grande stabilité opératoire Les ailettes de la partie proximale augmentent la surface de contact : Implant / os dans le cas d'un implant non cimenté, Implant / ciment dans le cas d'un implant cimenté. High intraoperative stability Proximal ridges result in increased contact area : between the bone and the implant for a cementless stem between the cement and the implant for a cemented stem. 5

7 Planification opératoire Preoperative planning 1 Préparation Dessiner sur la radiographie la ligne de référence reliant les extrémités distales des figures lacrymales ( U radiographique) ainsi que l axe vertical du bassin. Pour déterminer le modèle et la tige, poser le calque comme suit sur la radiographie du fémur à opérer : - Faire correspondre la partie médiale du contour de la tige et la partie médiale de l os cortical (a). - Aligner l axe de la tige parallèlement au fémur de sorte que la conjuguée T correspondant au centre de la tête fémorale affleure le sommet du grand trochanter (b). - Choisir la tige représentée sur le calque qui est tangente à la partie latérale de l os cortical fémoral (c). 1 Preparation Draw a reference line on the x-ray, that connects the inferior margins of the teardrop ; then draw the vertical axis of the pelvis. To determine the appropriate version and size of the implant, overlay the femoral template on the x-ray of the affected femur as follows : - align the medial border of the stem with the medial cortex (a), - align the centre line of the implant with the long axis of the femur such that line T that passes through the centre of the femoral head, lies at the top of the greater trochanter (b), - with the template properly overlaid on the x-ray, select the stem which lateral border is tangential to the lateral cortex (c). Use a tracing paper to draw the outlines of the selected implant, line T, and resection line R. 2 Choix de la cupule et de la tête fémorale La planification se fonde sur une longueur de col avec tête fémorale Ø28mm col moyen CM, avec possibilité de passer au col court CC ou au col long CL pendant l opération. 2 Cup choice and femoral head choice Preoperative planning is based on the use of a femoral head Ø28 with a standard neck length (CM); however, neck lengths CC or CL can be selected intraoperatively. 7

8 3 Résultat de la planification Le type de tige et la taille de la tige La distance entre la ligne de résection R et la base supérieure du petit trochanter La conjuguée T entre le centre de rotation et la pointe du grand trochanter. Le dessin final complet devra coïncider avec la radiographie post-opératoire. 3 Planning result Reference the type and size of the selected stem on the tracing paper. Note down the distance from the resection line R to the upper end of the lesser trochanter, and draw line T (that joins the centre of rotation to the tip of the greater trochanter). Once completed, the drawing should match the postoperative x-ray. 8

9 Technique opératoire Surgical technique La tige fémorale VECTRA peut être implantée quelle que soit la voie d abord. La technique opératoire suivante part de l exemple d un abord latéral. The VECTRA Femoral Stem can be inserted through any approach. This document describes a procedure using a lateral approach. 1 Réséquer le col du fémur en respectant un angle de 45 par rapport à l axe fémoral Resect the femoral neck at 45 to the long axis of the femur. 2 Extraire la tête fémorale au moyen d'un extracteur pour tête fémorale à poignée en T. Le cas échéant, recourir à une luxation de la tête fémorale au moyen du levier de luxation pour faciliter l extraction. Remove the femoral head with a T-handle femoral head extractor. If necessary, dislocate the femoral head using a dislocation lever to facilitate removal. 3 Implanter le cotyle. Insert the acetabular component. 4 Exposer la surface de résection par une manœuvre combinée associant une rotation extrême de la jambe et une adduction. Mesurer la distance entre le petit trochanter et la surface de résection, afin de déterminer la distance de résection conformément à la planification préopératoire. Le cas échéant, pratiquer une résection supplémentaire. Expose the proximal femur by placing the leg in extreme rotation and adduction. Measure the distance between the lesser trochanter and the resected surface to check conformity with the templated osteotomy level. It may be necessary to recut the femoral neck. 9

10 5 Ouvrir le canal médullaire au moyen de l alésoir conique. Open the medullary canal with the conical reamer. Alternative : Ouvrir le canal médullaire au moyen d un emporte pièce avec une antéversion de 10 à 15 approximativement. Alternatively, the medullary canal can be opened with a punch held in 10 to 15 degrees of anteversion (approximately). 6 Elargir progressivement le canal médullaire au moyen des râpes de tailles croissantes, en commençant par la plus petite. La taille de la râpe utilisée en dernier devrait correspondre à la taille de la tige déterminée lors de la planification préopératoire. Enfoncer la râpe de la taille de la tige définitive, de telle sorte que l extrémité supérieure du corps de la râpe coïncide avec la surface de résection. Rasp sequentially upward in size, beginning with the smallest size. The final rasp size should correspond to the templated implant size. Stop rasping when the top of the rasp is flush with the resected surface. 7 Retirer la poignée modulaire, mettre en place la tête d essai et effectuer une réduction d essai. Contrôler la stabilité de l articulation, la longueur de la jambe, la tension musculaire et la mobilité. Contrôler le risque de luxation pour tous les mouvements permis de l articulation. Remove the broach handle, position the trial head, and perform a trial reduction. Assess joint stability, leg length, soft tissue tension, and range of motion. Evaluate the potential for dislocation, throughout the range of motion. Pour l implantation d une tige fémorale Vectra non cimentée, passer au point 8A de la page 11. 11 8A Implantation d une tige Vectra non cimentée Insertion of a Vectra cimentless femoral stem Extraire la tête d essai et la râpe fémorale. Introduire à la main la tige non cimentée choisie. Enfoncer soigneusement la tige fémorale à l aide d un impacteur. L extrémité supérieure des ailettes doit coïncider avec le niveau de résection. Remove the trial head and the broach. Insert the selected cementless stem into the canal by manual pressure. The stem is fully seated when the superior limit of the ridges is flush with the cut surface. 8B Implantation d une tige Vectra cimentée Insertion of a Vectra cimented femoral stem 8B 1 Extraire la tête d essai et la râpe fémorale. Introduire un obturateur médullaire universel. Remove the trial head and the broach. Insert a universal cement restrictor into the medullary canal. 8B 2 Introduire un drain puis injecter le ciment à l aide d une seringue à ciment Insert a drain, and apply cement with a syringe. 8B 3 Enfoncer la prothèse fémorale choisie dans le canal fémoral en s aidant de l impacteur semi-lunaire. Parallèlement, retirer le drain. Placer la tige fémorale dans la position définitive. Enlever l excédent de ciment et maintenir la tige fémorale dans cette position jusqu à la prise du ciment. Use the half-moon shaped stem impactor to drive the implant into place, while removing the drain. Seat the implant in its final position. Remove any excess cement, and firmly hold the implant in the correct position until cement is cured. 11

12 9 Retirer la protection du cône puis nettoyer et sécher soigneusement le cône. Mettre en place la tête prothétique choisie par un léger mouvement de rotation dans le sens des aiguilles d une montre. Remove the taper cover and carefully clean and dry the taper. Place the selected head on the neck taper with a slight clockwise twist. 10 Bloquer la tête par un léger coup sur l impacteur de tête fémorale. Impact the head with one moderate blow on the head impactor. 11 Rincer la prothèse. Effectuer la réduction et un examen fonctionnel. Poser un drain de Redon et suturer la plaie. Procéder au contrôle radiologique. Irrigate the site. Perform a final reduction, and assess function of the prosthetic joint. Place a Redon drain, and close in a routine fashion with sutures. Check correct position of the implant on a roentgenogram. Remarque : Pour extraire la prothèse, utiliser l impacteur /extracteur positionné dans le trou d extraction situé sous le col de la tige fémorale. Ne pas réutiliser explant. If extraction of the prosthesis need be, remove the prosthesis by using the extractor/ impactor positioned in the extraction hole located under the neck of the femoral stem. Once removed, the prosthesis MUST NOT BE reused. 12
