Tous les frais liés à ce déplacement lui seront remboursés à l'aide du formulaire C disponible sur le site du Service Financier The services bought for visitor not belonging to the EPFL has to be paid by the private credit card of the traveller. All expenses linked to the travel will be reimbursed using the Form C available on the Financial Service web site 7

20 Dupliquer une demande de voyage / Duplicate a travel request Lorsque plusieurs personnes font le même déplacement, utiliser l'option dans le menu déroulant Dupliquer la demande de voyage et inscrire dans les commentaires les autres voyageurs. When several people are due to travel together, use the option in the scrolling menu Dupliquer la demande de voyage and write in the final comments the name of the other travellers. 20

23 Modification de la demande / Modify a travel request Dès que la demande a été envoyée à CWT à l aide du bouton Envoyer une demande d offre la date de l envoi apparaît, si des modifications sont faîtes (demandes complémentaires) il est à nouveau possible d envoyer la demande à CWT As soon as the mail request has been sent to CWT the sending date appears next to Demande d offre envoyée, if some changes are necessary you cansend again the request to CWT Nouveau motif de voyage / New travel purposes Nouvelle prestations demandées New services requested 23

24 This document, once duly completed with the prices given by the travel agent and choosen by the traveller must be signed by the traveller and his superior, must be faxed to the travel agent in order to release the travel documents Ce document une fois complété avec les informations de prix données par l agence de voyage et les prestations choisies par le voyageur doit être signé par le voyageur et son supérieur pour être ensuite faxéàl agent de voyage afin de permettre l émission des documents de voyages. 24
