1 CUTAAN LASER CENTRUM Medische lasers zijn toestellen die licht produceren van één bepaalde golflengte in coherent geordende golven. Door een aangepaste pulsduur en energiefluence te kiezen, kunnen heel specifiek bepaalde doelstructuren in de huid vernietigd of beïnvloed worden, zodat een dermatologische of esthetische verbetering kan bekomen worden zonder beschadiging van de normale huidstructuren. Sinds een tiental jaar is er een enorme evolutie in deze apparatuur. Jaarlijks komen er nieuwe toestellen bij die ofwel een verbetering zijn van bestaande, ofwel gebruikt kunnen worden voor nieuwe toepassingen. Het Cutaan Laser Centrum (CLC) heeft de ambitie om voor elke indicatie over het meest performante systeem te beschikken. Naast echte lasers zijn er de laatste jaren ook toestellen met intense pulsed light, infrarode straling, radiofrequency en ultrasone golven die gebruikt worden voor specifieke toepassingen. Binnen het CLC is er continuiteit in de opleiding voor deze nieuwe behandelingen. Hoe eenvoudig sommige behandelingen ook mogen lijken, de keuze van de geschikte golflengte, pulsduur en fluence vereist een grondige kennis van de dermatologie en de laserfysica. Bij onoordeelkundig gebruik van dergelijke toestellen kunnen verwondingen, littekens, pigmentbeschadiging, oogproblemen, ontstaan. Ook vereist de opvolging en de naverzorging na een dergelijke behandeling een degelijke kennis van de genezingsreacties en soms een specifiek geneeskundig voorschrift. Een uitspraak van de Koninklijke Academie voor Geneeskunde van België stelt ondubbelzinnig dat deze behandelingen derhalve thuishoren in een medische praktijk, onder de supervisie en de verantwoordelijkheid van een arts en niet in een schoonheidssalon. Onder de lasers die gecommercialiseerd worden voor de verschillende indicaties bestaat heel veel kwaliteitsverschil. De meest kwalitatieve lasers zijn meestal ook de duurste in ontwikkeling en in aankoop. De kwaliteitsverschillen kunnen zich uiten in zowel de efficiëntie als in pijnlijkheid en wijze van genezingsreactie. Vooral op dat laatste vlak zijn er de laatste jaren belangrijke evoluties, waarbij gepoogd wordt zelfde of betere resultaten te bereiken met minder ongemakken, zodat na behandeling zo snel mogelijk terug de normale professionele of sociale activiteiten kunnen hervat worden. Elk van deze lasers vraagt een specifieke opleiding met als doel zo goed mogelijke resultaten te bereiken in een minimum aantal behandelingen en met verwaarloosbare risico s. Deze opleiding wordt steeds binnen het CLC aan de artsen aangeboden. Tijdens de behandelingen verricht door Uw arts, is Dr B. Crevits ook steeds in het CLC aanwezig voor het geval er assistentie (theoretisch of praktisch) zou gewenst zijn. Laserchirurgie kan onder de noemer spitsgeneeskunde ingedeeld worden. Continu zijn er evoluties die door ons opgevolgd worden. Binnen het CLC worden geen experimentele behandelingen verricht. Nieuwe toestellen worden pas aangekocht wanneer ze hun efficiëntie bewezen hebben en wanneer ze een wezenlijke verbetering vormen ten opzichte van het reeds beschikbare laserarsenaal. De eerste generaties lasers werden reeds in de jaren 60 geproduceerd. Deze lasers hadden steeds een zeker verbrandend effect zodat er meestal enige mate van verlittekening veroorzaakt werd. Pas sinds in 1983 door Anderson en Parrish de theoretische principes van selectieve photothermolysis geformuleerd werden, zijn er voor de verschillende indicaties selectieve lasers ontwikkeld. Op basis van de wetenschappelijke studies en de ervaring, kan beweerd worden dat laserchirurgie reeds meer dan volwaardig zijn plaats in het therapeutisch arsenaal ingenomen heeft. Belangrijke complicaties (verlittekening, degeneratierisico, ) zouden bij lege artis behandelingen niet gevreesd moeten worden. Toch blijft het belangrijk dat de verschillende laserbehandelingen met professionele kennis en verantwoordelijkheid opgevolgd worden. Graag maak ik de vergelijking tussen laserbehandeling en behandeling met medicatie: zelfs op de bijsluiter van het meest onschuldige pilletje staan zeldzame complicaties vermeld, die wanneer ze zich voordoen belangrijke gevolgen voor de patiënt kunnen hebben. Complicaties zijn echter bij esthetische behandelingen absoluut ongewenst. Toch kan een individueel verschil in genezingsreactie (op basis van stoornis in ontstekingsreacties, infecties, allergieën, voorafgaande zonblootstelling, ) zich zelden eens voordoen. Op dat moment is het belangrijk dat uw arts met kennis van zaken de juiste diagnose stelt en de specifieke behandeling voorschrijft. Lasergeneeskunde is en blijft geneeskunde. Dit betekent dat een natuurlijke evolutie (in dit geval geen ziekte maar wel een natuurlijke aanleg of natuurlijke veroudering) aangepakt wordt door een medische handeling, dus door een vorm van medische beschadiging. Vooral voor esthetische indicaties blijft het belangrijk dat de patiënt en zijn/haar arts zich hiervan bewust blijven. Esthetische behandelingen zijn in tegenstelling tot de klassieke curatieve geneeskunde niet noodzakelijk. De patiënt heeft dus bij dit type van behandelingen aan zijn/haar arts de toestemming om de huid tijdelijk te beschadigen zonder dat dit medisch noo zakelijk was, met het doel een uiteindelijke esthetische verbetering te bekomen. De artsen die in het CLC behandelingen verrichten, beloven hierbij om met professionele kennis hun uiterste best te doen om de verwachte resultaten te behalen, liefst in zo weinig mogelijk behandelingen en met verwaarloosbaar risico.

2 Een ander belangrijk kenmerk van laserchirurgie is dat er geen histologisch onderzoek gebeurt (histologie: onderzoek onder microscoop, waarvoor een biopsie noodzakelijk zou zijn welke telkens wel in een litteken zou resulteren). Hiervoor is het belangrijk dat de arts voldoende klinische kennis heeft om huidletsels te beoordelen. Daartoe zijn dermatologen opgeleid. Bij twijfel over de natuur van een huidletsel waardoor een medisch risico zou kunnen bestaan (bv bij twijfel over de aard van een gepigmenteerd letsel, waar bv een beginnend melanoom niet zou kunnen uitgesloten worden) zal nooit laserbehandeling gebeuren en zal aangedrongen worden op chirurgische wegname. Honderd procent feilloosheid in klinische beoordeling bestaat niet. Elke dermatoloog wordt af en toe eens verrast door een resultaat van een histologisch onderzoek. In de overgrote meerderheid van de gevallen zou dit voor de laserbehandeling geen medische consequenties hebben. Toch kan dit nooit 100% uitgesloten worden. Wie dit heel klein risico onaanvaardbaar vindt, kan vrijwillig beslissen om nooit een huidletsel met laser te laten verwijderen (op een littekenvrije manier), maar zal opteren voor de chirurgische wegname die wel in littekens resulteert, waarbij een histologisch onderzoek kan doorgaan. Géén risico bestaat in geen enkele vorm van geneeskunde. Géén risico bestaat zelfs niet bij therapeutische onthouding, want bepaalde huidletsels moeten behandeld worden om evolutie naar een belangrijk medisch probleem te voorkomen. Filosofisch kan ik echter beweren dat het grootste risico bij een behandeling in het CLC zou kunnen resulteren uit een verkeersongeval op weg naar of vanuit het CLC! Voor laserbehandelingen bestaat geen RIZIV-nomenclatuur (uitzondering aangeboren wijnvlekken) en dus ook geen terugbetaling. In het beste geval kan voor sommige indicaties via niet-specifieke nomenclatuur een klein deel gerecupereerd worden (bv voor de UltraPulse CO2 laser). De honoraria voor de behandelingen in het CLC zijn vastbepaald. Dit betekent dat voor elke patiënt het honorarium voor een bepaald type van behandeling hetzelfde is bij elke arts die in het CLC behandelt. Deze honoraria zijn vastgelegd op basis van de eigenlijke kosten (kostprijs laser, verbruikskosten laser, verpleegkundige kosten, afbetaling infrastructuur, ) en zijn voor elke type laserbehandeling verschillend. Zo zijn er laserbehandelingen waarbij er een vast forfait per behandeling bestaat, bij andere lasertypes wordt de prijs bepaald door het aantal pulsen (afhankelijk van te behandelen oppervlak) of door het aantal huidletsels die behandeld worden. 3 CUTANEOUS LASER CENTER Medical lasers are devices producing light of one specific wavelength, according to coherently ordered waves. By choosing an appropriate pulse duration and energy fluency, very specific target structures of the skin can be destroyed or affected. This way, a dermatological or esthetical improvement can be obtained, without damaging the normal skin structures. Since the last ten years, there is a huge evolution going on in these devices. Each year, new devices are created: they are improved versions of earlier created devices or they can be used for new applications. The Cutaan Laser Centrum (CLC) has the ambition to dispose of the best-functioning system for every indication. Apart from real lasers, devices with intense pulsed light, infrared radiation, radiofrequency and ultrasonic waves have been developed in recent years and they are used for specific applications. The CLC continuously provides trainings for these new treatment facilities. Although some treatments may seem very easy to execute, a thorough knowledge of dermatology and laser physics is needed to make the right choices concerning the appropriate wavelength, pulse duration and fluency. An improper use of these devices can cause injuries, cicatrization, pigment damage, eye problems. Moreover, the follow-up and aftercare after such treatments require a solid knowledge of the healing reactions and sometimes a specific medical prescription. A decision by the Belgian Royal Academy of Medicine explicitly states that these treatments should be performed in a medical practice, under the supervision and responsibility of a physician, and not in a beauty parlor. The lasers commercialized for the different indications, show huge quality differences. The best lasers are mostly the most expensive ones, as far as their development and acquisition are concerned. Quality differences can be noticed in the efficiency of the treatment, the painfulness and the healing reaction. Especially in this last field, there have been some important evolutions over the past few years, according to which it is being tried to achieve the same - or even better - results with less discomfort, in order to allow the patient to continue his/her normal professional or social activities as soon as possible after the treatment. For each of these lasers, a specific training is required in order to obtain the best possible results in a minimum number of treatment sessions and with negligible risks. This training is organized for physicians, always in the CLC. During the treatment sessions performed by your physician, Dr B. Crevits will always be present in the CLC to provide (theoretical or practical) assistance if you want so. Laser surgery can be considered as advanced medicine. We permanently follow the evolutions, but we don t perform any experimental treatment in the CLC. We only acquire new devices after they have proved their efficiency and if they are a substantial improvement compared to our existing laser equipment. The first generations of lasers have already been created in the sixties. These lasers always had a certain burning effect, causing a certain degree of cicatrization. Only after Anderson and Parrish had formulated the theoretical principles of selective photothermolysis in 1983, selective lasers have been developed for the different indications. On the basis of scientific research and our own experience, we may state that laser surgery has obviously earned its place among the therapeutic treatments. Important complications (cicatrization, risk of degeneration ) shouldn t be feared if the treatment is executed according to the rules. It still remains important to follow-up the different laser treatments with professional knowledge and responsibility. I d like to compare laser treatments with treatments by means of medication: rare complications are even mentioned on the instruction leaflet of the most innocent little pill, and when they occur, they can have serious consequences for the patient. However, complications during or after esthetical treatments are absolutely undesired. Still, it is possible that an individual difference of healing reactions (based on a disturbance in the inflammation reactions, infections, allergies, preceding sun exposure ) seldom occurs. If it does, it is important that your physician makes the correct diagnosis and prescribes the specific treatment you need. Laser medicine will always be medicine. This means that a natural evolution (in this case not a disease, but a natural predisposition or natural ageing) is tackled by a medical action, thus a kind of medical damaging. It remains important, especially for esthetical indications, that the patient and his/her physician keep that in mind. Contrary to curative treatments, esthetical treatments are not necessary. For this type of treatments, the patient allows his/her physician to temporarily damage his/her skin without medical necessity, in order to finally obtain an esthetic improvement. The physicians performing treatments in the CLC, promise to do everything they can to obtain the expected result, with professional knowledge, after as less treatments as possible and with negligible risks. Another important characteristic of laser surgery is that no histological tests are performed (histology: examination by means of a microscope, for which a biopsy is required, resulting in cicatrices). Therefore, it is important that the physician disposes of adequate clinical knowledge to evaluate the skin lesions. Dermatologists have been trained for this purpose. Laser treatment will never be performed when the nature of a skin lesion is unsure, due to which a medical risk might occur (e.g. doubts about the nature of a pigmented skin lesion, in which case an arising melanoma couldn t be excluded). In that case, a surgical removal will be recommended. A faultless clinical evaluation (with 100% certainty) does not exist. Every dermatologist is sometimes surprised by the result of a histological test. In most cases, it would not have any consequences for the laser treatment, but this can never be excluded for 100%. If you consider this very small risk as unacceptable, you can voluntarily decide to never have a laser removal

4 (without cicatrices) executed, but you can choose for surgical removal, resulting in cicatrization, but allowing a histological test. Every type of medicine implies risks. Even in case of therapeutic abstinence, there are risks because some skin lesions have to be treated in order to prevent the evolution towards a considerable medical problem. Philosophically speaking, I could state that the biggest risk related to a treatment in the CLC, would result from of a traffic accident on your way to, or coming back from, the CLC! For laser treatments, there is no nomenclature by the National Institute for Sickness and Invalidity Insurance (except for the treatment of congenital port-wine naevi). Consequently, laser treatments aren t refunded. In the best case scenario, you can recuperate a small part of the costs through a non-specific nomenclature for certain indications (e.g. for the UltraPulse CO2 laser). The fees for the treatments in the CLC are determined. This means that, for all patients, the fee for a particular type of treatment performed by any physician in the CLC, will be the same. These fees are determined on the basis of the real costs (cost price of the laser, costs for the use of the laser, medical costs, payments for the infrastructure ) and differ according to the type of laser treatment. For some types of laser treatments, we use a fixed rate per treatment; for other laser types, the price depends on the number of pulses (according to the surface that needs to be treated) or on the number of skin lesions being treated. 5 CENTRE LASER CUTANE Les lasers médicaux sont des appareils qui produisent de la lumière d une seule longueur d onde particulière, selon des ondes ordonnées de manière cohérente. En choisissant une durée d impulsions et une fluence d énergie adaptées, nous pouvons très spécifiquement détruire ou affecter certaines structures cibles dans la peau, de sorte que nous puissions obtenir une amélioration dermatologique ou esthétique, sans détériorer les structures normales de la peau. Depuis une dizaine d années, ces appareils ont énormément évolué. Chaque année, de nouveaux appareils sont introduits; soit ils sont une version améliorée de la version précédente, soit ils peuvent être utilisés pour de nouvelles applications. Le CLC a l ambition de disposer du système le plus performant pour chaque indication. A part les vrais lasers, des appareils avec une lumière intense pulsée, un rayonnement infrarouge, une radiofréquence et des ondes ultrasonores ont été introduits les dernières années. Ils sont utilisés pour des applications spécifiques. Le CLC assure la continuité des formations pour ces nouveaux traitements. Même si certains traitements paraissent simples, le choix de la longueur d onde adéquate, de la durée d impulsions et de la fluence nécessite des connaissances approfondies en matière de dermatologie et de physique du laser. Une mauvaise utilisation de tels appareils peut provoquer des blessures, des cicatrices, la détérioration des pigments, des problèmes oculaires. En outre, le suivi et les soins ultérieurs après un tel traitement nécessitent de bonnes connaissances des réactions de guérison et dans certains cas, une ordonnance médicale spécifique. Selon l Académie royale de Médecine de Belgique, ces traitements doivent dès lors être effectués dans un cabinet médical, sous la supervision et la responsabilité d un médecin, et non pas dans un institut de beauté. Les lasers commercialisés pour les différentes indications, présentent beaucoup de différences en ce qui concerne leur qualité. Les meilleurs lasers sont souvent les plus chers au niveau du développement et de l achat. Les différences de qualité peuvent se manifester au niveau de la douleur ressentie et de la réaction de guérison. Surtout sur ce dernier plan, d importants progrès ont été constatés les dernières années. On essaie d obtenir les mêmes résultats, ou de meilleurs résultats, avec moins d inconvénients, pour que les patients puissent reprendre leurs activités professionnelles et sociales le plus vite possible après le traitement. Chacun de ses lasers nécessite une formation spécifique, ayant pour but d obtenir des résultats optimaux après un nombre minimal de traitements et avec des risques négligeables. Cette formation est proposée aux médecins, toujours au sein du CLC. Lors des traitements effectués par votre médecin, le Dr. B. Crevits sera toujours présent au CLC, au cas où vous désireriez son assistance (au niveau théorique ou pratique). La chirurgie laser peut être considérée comme médecine de pointe. Nous suivons certaines évolutions en permanence. Au sein du CLC, aucun traitement expérimental n est effectué. Nous achetons de nouveaux appareils uniquement si leur efficacité a été prouvée et s ils constituent une amélioration réelle par rapport aux appareils laser dont nous disposons déjà. Les premières générations de laser ont déjà été fabriquées dans les années 60. Ces lasers engendraient toujours un certain effet de brûlure de sorte que, dans la plupart des cas, ils causaient des cicatrices à un degré divers. Ce n est qu après qu Anderson et Parrish aient formulé en 1983 les principes théoriques de la photothermolyse sélective, que des lasers sélectifs ont été développés pour les différentes indications. Sur base des études scientifiques et de notre expérience, nous pouvons affirmer que la chirurgie laser a plus que largement mérité sa place au milieu des techniques thérapeutiques. On ne devrait pas craindre d importantes complications telles la cicatrisation, le risque de dégénérescence si les traitements sont effectués selon les règles de l art. Toutefois, il est important que les différents traitements laser soient suivis par des professionnels compétents et responsables. Permettez-moi de comparer le traitement laser au traitement à base de médicaments: même sur la notice d une petite pilule tout à fait innocente, des complications rares sont mentionnées. Elles peuvent avoir de lourdes conséquences pour le patient si elles se produisent. Toutefois, dans le cas des traitements esthétiques, les complications sont absolument indésirables. Pourtant, en cours de guérison, une réaction différente peut se produire rarement (lors d un trouble au niveau des réactions d inflammation, des infections, des allergies, de l exposition au soleil ). A ce moment-là, il est important qu en parfaite connaissance de cause, votre médecin fasse le bon diagnostic et prescrive le traitement spécifique approprié. La médecine laser restera toujours de la médecine. Ceci signifie qu une évolution naturelle (dans ce cas-ci, il ne s agit pas d une maladie, mais d une prédisposition naturelle ou du vieillissement naturel) est influencée par un traitement médical, donc par une sorte de destruction médicale. Surtout en ce qui concerne les indications esthétiques, il est important que le patient et son médecin en restent conscients. Contrairement à la médecine curative traditionnelle, les traitements esthétiques ne sont pas indispensables. Pour ce type de traitements, le patient donne l autorisation à son médecin d endommager temporairement sa peau, sans que ceci soit nécessaire d un point de vue médical, en vue d obtenir une amélioration esthétique au final. Dans ce contexte, les médecins effectuant des traitements au sein du CLC, promettent de se surpasser au niveau professionnel pour obtenir les résultats attendus, de préférence en effectuant le moins de traitements possible et avec un risque négligeable. Une autre caractéristique importante de la chirurgie laser consiste à ce qu aucun examen histologique ne doive être effectué (l histologie est un examen à l aide d un microscope, qui nécessiterait une biopsie, ce qui provoquerait des cicatrices). A cet effet, il est donc important que le médecin dispose de suffisamment de connaissances cliniques pour pouvoir évaluer les lésions

6 cutanées. Les dermatologues ont été spécialement formés en la matière. En cas de doute sur la nature d une lésion cutanée, qui pourrait comporter un risque médical (par exemple, en cas de doute sur la nature d une lésion pigmentée, où un nouveau mélanome ne pourrait être exclu), nous n effectuerons jamais de traitement laser et nous insisterons sur l enlèvement chirurgical. Il est impossible de faire des évaluations cliniques entièrement infaillibles. Tout dermatologue est parfois surpris par le résultat d un examen histologique. Dans la grande majorité des cas, ce dernier n aurait pas de conséquences médicales pour le traitement laser. Pourtant, on ne peut jamais exclure ceci à 100%. Si vous trouvez que ce risque minime est inacceptable, vous pouvez décider délibérément de ne jamais faire enlever une lésion cutanée au laser (manière selon laquelle vous n auriez pas de cicatrices). Vous pouvez alors opter pour l enlèvement chirurgical, qui provoquera des cicatrices et pour lequel un examen histologique pourra être effectué. Tout type de médecine contient des risques. Même l abstinence thérapeutique n est pas sans risques, car certaines lésions cutanées doivent être traitées afin de prévenir l évolution vers un problème médical considérable. D un point de vue philosophique, je pourrais par contre affirmer que le plus grand risque lié à un traitement au CLC, pourrait résulter d un accident de la route lors de vos déplacements! Il n y a pas de nomenclature INAMI (à l exception des taches de vin congénitales) pour les traitements laser et ils ne sont donc pas remboursés. Dans le meilleur des cas, pour certaines indications, une petite partie des frais peut être récupérée (par exemple, pour le laser CO2 UltraPulse), par l intermédiaire de la nomenclature non spécifique. Les honoraires pour les traitements au sein du CLC sont figés, ce qui signifie que pour chaque patient, l honoraire pour un type de traitement particulier est le même, n importe quel médecin effectue le traitement au sein du CLC. Ces honoraires sont déterminés en fonction des frais effectifs (coût du laser, frais de consommation du laser, frais des soins infirmiers, paiements de l infrastructure ) et différents pour chaque type de traitement laser. Ainsi, il existe des traitements laser pour lesquels il y a un forfait fixe. Pour d autres types de laser, le prix est déterminé par le nombre d impulsions (en fonction de la surface qui doit être traitée) ou par le nombre de lésions cutanées à traiter.
