Cost-effectiveness of Cetuximab (Erbitux ) in metastatic colorectal cancer after failure of irinotecanincluding cytotoxic therapy in Belgium
Introduction The approval of cetuximab in mcrc after failure of irinotecan-including was based on a phase II study comparing the combination of cetuximab plus irinotecan vs. cetuximab monotherapy* Cetuximab plus irinotecan is the first treatment to offer significant clinical benefit to patients with mcrc failing irinotecan-based therapy The cost-effectiveness analysis requires the comparison of cetuximab and irinotecan with the therapy that is most likely to be replaced by this combination Before cetuximab was introduced in clinical practice, the only therapy option for this patient group was standard care Therefore modelling techniques had to be used to compare cetuximab plus irinotecan vs. standard care in terms of survival and costs
Methods (1) Survival and resource use data were collected for two different groups of patients: 1) Patients receiving cetuximab plus irinotecan from the complete Bond population (for cetuximab, irinotecan and EGFR testing costs) and from the records of Belgian patients who had participated in the BOND trial (n=43) (other costs) 2) Patients receiving standard care- data from a review of treatment received by patients in the three largest BOND study centres (Belgium, France, Italy) who met the eligibility criteria for inclusion in the BOND trial but who fell outside the recruitment period for the trial (matched Bond population), (n=66) This group was further divided in those patients who received 2a) Active chemotherapy (ASC) 2b) Best supportive care (BSC; no chemotherapy or only very low doses of chemotherapy for palliation, with relative dose intensity <50%)
Methods (2) To enable optimal use of health care resources by selecting those patients who are most likely to benefit from treatment, two scenarios were tested for cetuximab: 1) 6-week rule; to assess tumour response (by means of a computed tomography [CT] scan) at 6 weeks and then to continue treatment only in patients with complete or partial tumour response 2) 12-week rule; to follow the same algorithm, but to assess tumour response at 12 weeks only Adjustment factors were applied to the Bond data to derive the survival for both treatment rules Belgian unit cost data were applied to all patients

Conclusion Cetuximab plus irinotecan is the only treatment to offer significant clinical benefit to patients with mcrc failing irinotecan-based therapy The incremental cost per life year gained of cetuximab plus irinotecan compared to standard care or best supportive care is well below currently accepted thresholds* for costeffectiveness when a 6-weeks or 12 weeks continuation rule is applied Therefore, the use of cetuximab and irinotecan in patients with mcrc failing irinotecan-based therapy represents cost-effective use of health care resources in Belgium *usually considered to be around 50,000/LYG gained
