2 AIDE-MÉMOIRE Nous avons bien hâte d accueillir les enfants à notre camp d été 2014! L été s annonce comme une saison remplie d aventures qui laisseront les jeunes comblés et en pleine forme. Les enfants seront transportés au travers différents thèmes à chaque semaine et auront l occasion de pratiquer le sport de la gymnastique au moins 2 heures par jour! Notre camp d été s adresse aussi bien aux débutants qu aux gymnastes déjà inscrits dans cette discipline. Le but du camp est d offrir aux enfants le plaisir de pratiquer la gymnastique dans un cadre social et éducatif et de les familiariser avec les mouvements gymniques de base. 4 CHECKLIST We are excited to welcome the kids to this year s summer camp! Summer is looking to be a season full of adventures sure to leave kids fulfilled and in great shape. Kids will be transported through different themes each week and will have the opportunity to practice the sport of gymnastics at least to hours each day! Our summer camp is focussed on beginners as well as gymnasts who have experience with the sport. The objective of the camp is to offer kids the joy of practicing gymnastics in a social and educational environment as well as familiarize them with gymnastic movements. Here are a few important details to remember in order to ensure a great week at camp: General Daycare and one special activity included Qualified coaches Bilingual environment Relevé 24 for tax purposes (SIN number required) Nuts and peanuts are prohibited Hours of Operation Daycare (included in the price): 7 a.m. to 9 a.m. and 4 p.m. to 5:30 p.m. Camp: 9 a.m. to 4 p.m. What to Wear Comfortable clothing Gymnastics suit (optional) * Gymnastics suits are on sale on site. If so, details will be provided on the Monday. Plan for a bathing suit and towel for this activity. Je demande, par la présente, que l EpiPen soit administré. Il est aussi convenu que l enfant portera le médicament sur sa personne. C est la responsabilité du parent ou de la personne qui a la tutelle légale de l enfant de renseigner les animateurs sur l endroit où se trouve l EpiPen sur la personne de l enfant. J accepte de fournir au Camp de jour d Unigym Gatineau une déclaration médicale mise à jour quand le médecin modifiera ses instructions relativement au médicament. Par la présente, je libère le Camp de jour d Unigym Gatineau, ses employé(e)s et représentant(e)s de toute action en justice, perte, dommages ou blessures, peu importe les circonstances, qui pourraient résulter de l administration de l auto-injecteur d EpiPen ou du médicament, tel que demandé dans le présent document. I also agree that the child keep the EpiPen on his person. It is the responsibility of the parent or person who has legal guardianship of the child to inform the animators where the EpiPen is located. I release the Unigym Gatineau summer camp, its employees and representativess of any lawsuit, loss, damage or injury, regardless of circumstances, which could result from the administration of the auto-injector EpiPen or medication, as requested in this document. I agree to indemnify said Unigym Gatineau summer camp, its employee or representative for any loss or damage arising from any action or judicial proceedings against them by me or my child or any relative or guardian of the child above.
