Bart Criel et Monique Van Dormael Département de Santé Publique, Institut de Médecine Tropicale, Anvers, Belgique Introduction Depuis quelques années il y a en Afrique sub-saharienne un intérêt croissant pour des systèmes d assurance-maladie volontaires, développés et gérés au niveau périphérique du système de soins. La crise dans le financement public des systèmes de soins Africains y est pour quelque chose. Une réponse pragmatique à cette crise a été l introduction généralisée dans les années 80 et 90 de politiques de paiements directs. Ceux-ci ont connu des succès certains en témoignent les évaluations de l Initiative de Bamako faites au Bénin et en Guinée (Levy-Bruhl et al. 1997) mais leurs limites sont claires. Faire payer les patients au moment de l utilisation crée un problème d accessibilité pour les plus pauvres. C est ainsi que des mécanismes de paiement anticipé, basés à la fois sur un principe d assurance et un principe de solidarité, constituent une alternative intéressante aux paiements directs. L assurance-maladie n est pas quelque chose d inconnu en Afrique. Des systèmes d assurance-maladie Bismarckiens ont été introduits dans la majorité des jeunes Etats Africains juste après les indépendances. Il s agit cependant d un modèle copié du colonisateur qui n est pas le produit d une culture et d une histoire sociale locales (Ron et al. 1990). Ce modèle est d ailleurs demeuré peu performant: sa couverture est toujours resté limitée à la petite population de salariés du secteur formel qui, bien souvent, constitue déjà un groupe de population privilégié. Il y a un consensus aujourd hui que ce modèle d assurancemaladie, financé par des contributions de sécurité sociale payées par employés et employeurs, ne constitue pas une option valable pour la majorité des Africains, c est-à-dire les paysans et les ménages vivant du secteur informel. La mutuelle de santé, par contre, est un modèle plus attrayant. Une mutuelle est définie comme une association facultative de personnes, à but non lucratif, et dont la base de fonctionnement est la solidarité entre tous les membres. Au moyen de la cotisation de ses membres et sur base de leurs décisions, la mutuelle mène une action de prévoyance, d entraide et de solidarité dans le domaine des risques sociaux (ANMC et al. 1996). Une mutuelle de santé se concentre sur la prise en charge de problèmes de santé. Une dynamique mutualiste semble émerger en Afrique surtout en Afrique francophone (Brouillet et al. 1997; Atim 1998; Bennett et al. 1998). Dans cet article, nous proposons de comparer brièvement les principaux éléments qui ont caractérisé l environnement politique, social, économique et technique dans lequel le mouvement mutualiste est né en Europe occidentale dans le 19ème siècle avec ceux qui caractérisent l environnement actuel dans lequel se développent les mutuelles Africaines. Les différences entre les deux contextes nous semblent suffisamment importantes pour les expliciter plus clairement. Nous pensons qu elles sont telles qu elles rendent peu plausible l hypothèse (implicite) selon laquelle la dynamique mutualiste en Afrique évoluera progressivement vers des systèmes nationaux d assurance-maladie obligatoire comme cela s est passé dans plusieurs pays Européens à la fin du 19ème et au début du 20ème siècle. En Europe L origine des mutuelles en Europe est intimement associée à l histoire sociale européenne du 19ème siècle. La révolution industrielle en Europe a entraîné une augmentation considérable du nombre de salariés, concentrés dans les nouveaux centres industriels et confrontés à une énorme misère sociale. Les revendications croissantes du mouvement ouvrier et la montée du socialisme ont alimenté les inquiétudes au sein de l establishment (politique, économique et même religieux) quant aux risques de dislocation de la société, et ont obligé l Etat à reconsidérer son rôle et à se transformer en puissance protectrice, l Etat-Providence. Les premières mutuelles se sont développées dans la première moitié du 19ème siècle, à l initiative de groupes d ouvriers ou de patrons ayant des préoccupations sociales: sociétés de secours mutuels en Belgique et en France, 1998 Blackwell Science Ltd 155
2 B. Criel et M. Van Dormael Mutuelles de santé en Afrique friendly societies au Royaume-Uni. Les principaux risques encourus par les ouvriers n étaient pas les éventuels frais médicaux les soins disponibles étaient très restreints et techniquement peu efficaces à l époque mais bien la perte de revenus du ménage en cas de décès, d accident, de maladie prolongée ou de licenciement. Mais les insuffisances des mutuelles sont rapidement devenues manifestes: les cotisations restreintes, la petite taille de la plupart de ces associations, et le manque d expertise gestionnaire ne permettaient pas aux mécanismes de solidarité locale d assurer une réelle protection aux adhérents (de Swaan 1988). Si l origine de ces caisses de secours mutuels provient du mouvement ouvrier naissant ou de mouvements philanthropiques, et non des pouvoirs publics, il est essentiel de noter que c est seulement après l intervention de l Etat que ce type d association est devenu viable. La manière dont l Etat Providence est intervenu dans ce qui s est appelé ultérieurement la sécurité sociale a varié d un pays à l autre. Là où l Etat est intervenu directement, comme au Royaume-Uni, les mutuelles ont progressivement perdu leur raison d être. Dans d autres pays comme la Belgique, les mutuelles ont subsisté en s intégrant dans le cadre de l assurance-maladie obligatoire. En Allemagne, le nom du chancelier Bismarck est associé au premier système de sécurité sociale organisé par l Etat, avec la création de l assurance maladie en 1883 financée par des cotisations patronales et ouvrières ainsi que par l Etat. Pour Bismarck, la mise en place d un système de sécurité sociale obligatoire, subsidié par l Etat (et donc dépendant de lui), était un instrument de contrôle social et politique: plus d égalité sociale afin de garantir une loyauté des travailleurs vis-à-vis de l Etat. L intention était de contrer l influence politique des socialistes et d endiguer les risques d émeutes et de révolution, en répondant à certaines exigences du mouvement ouvrier (Rimlinger 1971). L émergence de l Etat-Providence à la fin du 19ème siècle apparaît donc comme une réponse face au péril constitué par des masses populaires paupérisées mais de plus en plus organisées et capables de se faire entendre dans la presse, dans la rue, voire dans les instances parlementaires. L intervention de l Etat était censée maintenir et non pas changer les fondements de la société capitaliste (Vanthemsche 1997). Pour réaliser cette fonction de protection sociale, il fallait cependant que l Etat dispose de l autorité nécessaire pour imposer et faire respecter le caractère obligatoire du système d assurance, d une administration efficace pour le gérer, et finalement des ressources financières nécessaires pour le subsidier. L industrialisation avait provoqué un enrichissement matériel considérable des Etats européens même si cet enrichissement ne profitait pas à tous. La colonisation augmenta encore le réservoir de richesses, assurant à l Etat Providence les moyens de sa politique. En Afrique sub-saharienne La dynamique mutualiste africaine présente un certain nombre de spécificités qui la distinguent nettement de la situation Européenne de la fin du 19ème siècle. Ceux qui promeuvent le concept de mutuelles et qui apportent l appui institutionnel et technique nécessaire à leur développement sont souvent extérieurs à la société africaine. La dynamique est donc largement exogène et les liens éventuels avec les systèmes endogènes d entraide ont encore été très peu étudiés (Criel et al. 1998). La série de réunions internationales organisées au cours de l année témoigne de l intérêt que des organisations de coopération, des instituts de recherche (non africains) et des institutions mutualistes européennes (surtout belges et françaises) portent pour le sujet. Le développement de mutuelles de santé cadre d ailleurs bien avec le climat international actuel favorable à une plus grande implication du secteur non-étatique dans l offre et le financement des soins. Ainsi, la promotion de systèmes locaux d assurancemaladie volontaires en général, et de mutuelles de santé en particulier, est aujourd hui politiquement correct. Cependant, la dynamique mutualiste ne va pas de pair avec un mouvement politique organisé, ouvrier ou paysan. Elle trouve sa justification dans les dysfonctionnements des systèmes de soins de santé africains: d une part le problème de l exclusion financière des soins de santé, et d autre part le manque de dialogue et de négociation dans les relations entre usagers et les techniciens de santé. Les attentes sont considérables: un meilleur accès aux soins de santé pour ceux qui ont en besoin, plus de solidarité sociale vis-à-vis de la maladie, et des nouveaux rapports de force entre population et services de santé. L apparition de mutuelles, en l absence d une polarisation politique, ne semble pas constituer une menace quelconque ni pour l Etat, ni pour l establishment économique Africain. C est peut être même l inverse qui est vrai; les mutuelles seraient les bienvenues ne fût-ce que pour remédier à l insuffisance du financement public des soins de santé. 3 Conclusion Les conditions générales qui prévalent en Afrique sub- Saharienne diffèrent considérablement de celles qui régnaient en Europe au siècle dernier. De là à prédire le futur, il est un pas que nous ne franchirons pas. Cette analyse nous apprend, cependant, qu il est légitime de douter que la dynamique mutualiste africaine évolue de façon semblable à celle d Europe. Le caractère exogène du concept mutualiste ne constitue pas (nécessairement) un obstacle à son appropriation par les communautés africaines, mais mènera probablement à une variété d expressions adaptées, nous l espérons, à des environnements sociaux et culturels distincts. Cependant, si les mutuelles de santé se veulent être une réelle contribution à la création d une société plus solidaire avec moins d exclusion sociale, alors, à terme, l intervention d une autorité publique semble incontournable. La dynamique mutualiste africaine se verra donc confrontée, tôt au tard, à la problématique du rôle et de la performance de l Etat. Editorial: Mutual Health Organizations in Africa and social health insurance systems: will European history repeat itself? Introduction In recent years there has been growing interest in locally developed and managed voluntary health insurance schemes in subsaharan Africa, being partially the result of the financing crisis of African health systems. General introduction of user fees in the 1980s and 1990s was a pragmatic answer to this crisis. This policy has yielded successes as indicated by the evaluations of the Bamako Initiative in Benin and in Guinea (Levy-Bruhl et al. 1997) but its limitations are obvious. Charging fees at the time of health care utilization creates an accessibility problem for the poor. Prepaid health care financing arrangements, based on insurance and solidarity therefore are an interesting alternative. Health insurance, however, is not an entirely new concept in Africa. Bismarckian social health insurance systems were introduced in most African states just after their independence. But these systems were copied from the former coloniser, and by no means the expression of local culture and history (Ron et al. 1990). The effectiveness of the imported model of health insurance remained, on the whole, disappointing in Africa: its coverage was, and still is, confined to the small proportion of wage earners who constitute a relatively privileged group within African society. Today there is consensus that these social health insurance systems, financed by employee and employer contributions, are not a solution for the majority of the African citizens, i.e. self-employed farmers and households making a living in the 2 Mutuelles in French. informal sector. Mutual aid organizations 2 or mutuals, on the other hand, constitute a more attractive model. A mutual aid organization is a voluntary association of people, without lucrative purpose, which is based on solidarity between all its members; through the contribution paid by its members and on the basis of decisions taken by the members themselves or by their management structures, it takes action to promote mutual help between members in the light of social risks they face (ANMC et al. 1996). A Mutual Health Organization (MHO) specifically addresses the management of health problems 3. A dynamic of MHOs seems to emerge in Africa, especially in the francophone countries (Brouillet et al. 1997; Atim 1998; Bennett et al. 1998). In this paper, we compare the main features of the political, social, economical and technical environment in which the MHO movement was born in Europe in the 19th century with elements characterising the current context of developing African MHOs. The differences between the two situations are sufficiently important to be clearly stated, and in our view, oppose the (implicit) hypothesis that the MHO dynamic in Africa would gradually transform into national mandatory health insurance systems just as was the case in several European countries at the end of the 19th and beginning of the 20th century. 3 Attempts to translate the French term mutuelle de santé into English have always been dogged by the lack of any clear recognizable equivalent, perhaps illustrating the fact that the mutuals reality is different in English speaking African countries. The term Mutual Health Organization, however, has recently come to be used in the discourse to describe organizations of this kind (Atim 1998) Blackwell Science Ltd 157
4 Europe The origin of European mutuals is closely linked to 19th century European social history. The industrial revolution led to a dramatic increase in salaried workers who lived in new industrial centres, often under very harsh conditions. Growing social claims of the working class movement and the rise of socialism fuelled fears amongst the political, economical and even religious establishment of a dislocation of society. The state was forced to dramatically reconsider its role and to change into a protective power, the Welfare State. The first mutuals developed in the first half of the 19th century upon the initiative of workers or employers willing to improve the living conditions of their workforce: sociétés de secours mutuels in Belgium and France, friendly societies in the United Kingdom. The main risks covered were not expenses for health care available care was very limited and fairly ineffective but rather loss of income in case of death, accident, chronic disease or unemployment. The limitations of these organizations became rapidly apparent. The modest size of contributions, the small scale of most associations and lack of management expertise did not allow for economies of scale and jeopardized effective social protection (de Swaan 1988). Although these mutuals were created by the working class movement and philanthropic movements rather than a public authority, crucially they only became effective and sustainable after state intervention. This intervention of the Welfare State varied from one country to another. In the United Kingdom, for instance, the state intervened directly and made the existence of mutuals superfluous. In other countries such as Belgium, the mutuals remained and were gradually integrated into the national social health insurance system. In Germany, Bismarck introduced the first state-run social security system. A national social health insurance system, financed by employee and employer contributions and benefiting from state subsidies, was created in Implementation of a state-subsidized, state-dependent social security system was an instrument for more social and political control: greater social equality as a means to guarantee the workers loyalty towards the state. The purpose of Bismarck s social policy was to counter the political influence of the socialists and contain the risks of riots and revolution (Rimlinger 1971). The emergence of the Welfare State appears thus as an answer to the threat constituted by impoverished but increasingly well-organized popular masses that succeeded to voice their claims in the press, in the streets, and even in parliament. State intervention was meant to safeguard the foundations of the capitalist system rather than overthrow them (Vanthemsche 1997). This gradual transformation into the Welfare State was only possible because the state had the necessary authority to enforce the mandatory character of the insurance system; because it could rely on a well-functioning administrative apparatus to manage it; and because it possessed the financial resources to subsidize it. Industrialization was a major source of wealth for the European states, even if only for a minority. Colonization further increased the state s financial resources, thereby providing the means to fund its social policy. Subsaharan Africa The MHO dynamic in Africa has a number of features that distinguish it clearly from the situation in Europe at the end of the 19th century. The institutions which promote the concept of mutuals in Africa and offer technical support for their development often are outsiders to African society. The MHO dynamic is thus to a large extent exogenous and possible links with endogenous mutual aid systems have hardly been studied (Criel et al. 1998). The range of international meetings organized in 1998 on the MHO dynamic in Africa 4 indicates the interest of development agencies, non-african research institutions, and European MHO institutions (mainly Belgian and French). Furthermore, the development of MHOs follows the current trend of increasing importance of the nongovernment sector in financing and provision of health care services. Nowadays it is politically correct to promote and develop locally managed voluntary health insurance schemes in general and MHOs in particular. The African MHO dynamic, however, is not part of a larger organized political movement of workers or peasants. It finds its raison d être in the ill-functioning of African health systems: the problem of financial exclusion from health care on the one hand, and lack of dialogue and negotiation between population and health care professionals on the other hand. Expectations are high: improved access to health care for those who need it, greater social solidarity in the face of illness, and new power relationships between population and health services. The creation of MHOs in the absence of clear political polarization does not appear to constitute a (significant) threat to the state or the economic establishment in Africa. Perhaps even the reverse holds: MHOs are welcome if only because they relieve the state of its responsibility to properly fund its health system. 5 necessary monetary means to support, finance and upgrade an MHO dynamic. Conclusion The conditions currently prevailing in subsaharan Africa differ considerably from those in Europe 100 years ago. Present analysis does not permit to predict the future, but casts serious doubts on whether the MHO dynamic in Africa will evolve along the same lines as in Europe. While the exogenous character of the MHO concept does not necessarily constitute an obstacle to its appropriation by African communities, it will probably lead to a variety of expressions, hopefully adapted to different social and cultural environments. However, if MHOs are to significantly contribute to an increase in solidarity and a reduction of social exclusion, the support of a public authority will eventually be needed. MHOs in Africa will thus, sooner or later, be confronted with the problem of the role and the performance of the state.
