PRISE EN CHARGE A LA PHASE AIGUE : AVC H
«Les patients ayant une hémorragie cérébrale spontanée de moins de 6 h, et une PAS de 150 à 220 mmhg, peuvent bénéficier d une baisse rapide de la PAS, avec une PA cible < 140 mmhg en moins de 60 min. Cette baisse est réalisable, bien tolérée et probablement efficace sur le pronostic fonctionnel». «Cette baisse doit être précoce, et la réduction du niveau de PA (<140mmHg) obtenue dans l heure suivant la mise en place du traitement et maintenue durant au moins une semaine».

AHA/ASA 2014 40 The available data indicate that diuretics or the combination of diuretics and an angiotensin-converting enzyme inhibitor is useful (Class I; Level of Evidence A). The choice of specific drugs and targets should be individualized on the basis of pharmacological properties, mechanism of action, and consideration of specific patient characteristics for which specific agents are probably indicated (eg, extracranial cerebrovascular occlusive disease, renal impairment, cardiac disease, and DM) (Class IIa; Level of Evidence B).

CONCLUSION 47 Abaisser la PA est très efficace pour réduire le risque primaire et secondaire d'avc et pour en réduire la gravité. La CAT diffère selon le type d AVC Les combinaisons IEC/ARA2 et/ou Inhibiteur calcique et/ou Diurétique thiazidique ou apparentés ont le plus fort niveau de preuve Observance!
