CONSIDÉRATIONS GÉNÉRALES
OUI BIOPSIE de la récidive métastatique IB 98 %
Longues survivantes ABC durablement stable, une imagerie mammaire devrait continuer à être réalisé de manière régulière. Avis d experts OUI 53% L ablation de la tumeur primitive chez les patientes ayant un cancer du sein stade IV de novo n a pas montré un prolongement de la survie, à l exception des patientes ayant des métastases osseuses exclusives 1B 70% Une partie non négligeable des patientes ayant un ABC, comme par exemple celles ayant une maladie oligo-métastatique ou une masse tumorale faible très sensibles aux traitements systémiques, peut être mise en rémission complète et avoir une survie longue. Une approche pluridisciplinaire, comprenant des traitements locorégionaux à visée curative, devraient être envisagés pour ces patientes. MALADIE OLIGOMETASTATIQUE La maladie oligométastatique est définie comme une maladie avec un faible volume et un nombre limité de lésions métastatiques (jusqu à 5 et pas nécessairement dans le même organe), potentiellement éligible pour un traitement local destiné à obtenir une rémission complète. Visceral crisis VISCERAL CRISIS is defined as severe organ dysfunction as assessed by signs and symptoms, laboratory studies, and rapid progression of disease. Visceral crisis is not the mere presence of visceral metastases but implies important visceral compromise leading to a clinical indication for a more rapidly efficacious therapy, particularly since another treatment option at progression will probably not be possible. Patientes préménopausées Chez les patientes préménopausées, l hormonothérapie ajoutée peut être IA ou tamoxifène en fonction de la durée et du type d hormonothérapie adjuvante. Un IA nécessite absolument une suppression ovarienne. Le fulvestrant est également une option valable, mais exige également une suppression ovarienne. ER+/HER2- metastatic breast cancer The preferred 1st line ET for postmenopausal patients is an aromatase inhibitor or tamoxifen, depending on type and duration of adjuvant ET. PALBOCICLIB L addition du palbociclib, inhibiteur CDK 4/6, à un IA en 1ère ligne chez les patientes ménopausées apporte un bénéfice important en PFS dans une étude randomisée de phase II. Les résultats de l étude de phase III sont attendus avant qu il puisse être considéré comme une option thérapeutique recommandée. OUI 51,2 %

Bevacizumab Bevacizumab combined with a chemotherapy as 1st or 2 nd line therapy for MBC provides only a moderate benefit in PFS and no benefit in OS. The absence of known predictive factors for bevacizumab efficacy renders recommendations on its use difficult. Bevacizumab can only therefore be considered as an option in selected cases in these settings and is not recommended after 1 st /2 nd line. (LoE: 1 A).

HER2- metastatic breast cancer In patients pre-treated (in the adjuvant or metastatic setting) with anthracycline and taxanes and who do not need combination CT, capecitabine, vinorelbine or eribulin single agent are the preferred choices. Additional choices include gemcitabine, platinum, a taxane, liposomal anthracyclines. Decision should be individualized and take into account different toxicity profile, previous exposure, patient preferences. (LoE: 1 B)

CARBOPLATINE et TNG Chez les patientes TN (quel que soit le statut BRCA) préalablement traitées par anthracyclines et taxanes en situation (néo)adjuvante, le carboplatine montre une efficacité comparable et un meilleur profil de tolérance que le docétaxel, et représente une option thérapeutique. OUI 1A 90% PLATINE et BRCA Chez les patientes ayant un cancer du sein muté BRCA triplenégatif ou hormonorésistant préalablement traité par une anthracycline et un taxane (en adjuvant ou en métastatique), un schéma avec un sel de platine est l option préférentielle si la patiente n en a jamais reçu et qu aucun essai clinique adapté n est disponible.
