Cependant la responsabilité de facteurs de risque d infection liée au patient lui-même, reste débattue face à la notoriété du risque «nosocomial» acquis à l hôpital. Ainsi, reconnaître les facteurs de risques infectieux liés au patient et surtout en apprécier l importance doit se faire de façon précoce dès l indication chirurgicale posée afin d en corriger éventuellement l impact sur l évolution per opératoire. Le rôle central d une équipe pluridisciplinaire médico chirurgicale s impose dès cette étape. La coordination de cette enquête lors de sa consultation par le médecin Anesthésiste-Réanimateur renforce la qualité de suivi médical péri opératoire et la correction de ces facteurs favorisant l infection. Enfin l information du patient sur le risque infectieux qu il apporte doit s associer à la discussion sur le bénéfice/risque de l intervention tel qu il est habituellement présenté par le Médecin Anesthésiste Réanimateur à son patient. The orthopaedic surgery patient is becoming increasingly aware of the risk of infection and its severe impact on the results of an operation that, more often, is functional. Yet the part played by risk factors linked to the patient himself is still debated owing to the widely publicised «nosocomial» risk in hospitals. Identifying the risk factors linked to the patient, and above all, assessing their importance, must take place early on, as soon as surgery is recommended, in order to correct their possible impact during and after surgery. A multidisciplinary, medical/surgical team plays a key part as of this early stage. Coordination of assessment by the anaesthetist reinforces the quality of pre- and post-operative medical care and correction of these infection-inducing factors. Informing the patient of the patient-related risk of infection must be part of the overall discussion of the benefitrisk of the operation during the customary appointment between patient and anaesthetist. Nous disposons actuellement d agents anesthésiques s éliminant vite et de façon très fiable, c est à dire sans effet résiduel significatif. L administration répétée de protoxyde d azote sur de courtes périodes peut être à l origine d une toxicité neurologique et hématologique et interférer négativement avec les processus de cicatrisation. Des données récentes font état d interactions possibles entre certains agents d anesthésie et le fonctionnement cérébral à long terme, particulièrement aux âges extrêmes de la vie. L organisation de ces anesthésies répétées est un enjeu Important : quid de la consultation pré-anesthésique? Quel site approprié? Quel personnel nécessaire? Quelle surveillance post-anesthésique? The repeated administration of nitrous oxyde for short periods may be neurologically and hemotologically toxic and impair healing processes. Recent data highlight possible interactions between certain anesthetic products and the long-term functioning of the brain, particularly among the very old or very young. It is of paramount importance carefully to organise iterative anesthesia (What about the pre-anesthesia consultation? Which is the right site? What personnel is required? Tous les tissus anatomiques peuvent être affectés. La peau est le siège de cicatrices itératives inesthétiques, d ulcérations chroniques ou de lambeaux disgracieux. Les muscles et tendons peuvent être atrophiés ou rompus, les nerfs paralysés et la vascularisation altérée. Les articulations s enraidissent ou deviennent instables. Les os se désaxent, sont allongés ou raccourcis, parfois amputés. Ces séquelles douloureuses, et souvent irréversibles, contribuent à altérer le résultat fonctionnel des patients. Elles sont d autant plus importantes que les patients sont en situation d échec et qu ils ont subi des chirurgies itératives. Ces situations d échecs infectieux sont une source d inquiétude, d incompréhension et de défiance pour le patient : comment un patient qui s est confié au corps médical et qui acquière une infection au cours d une chirurgie à visée fonctionnelle va-t-il pouvoir se confier de nouveau à la médecine dans l espoir de guérir? Du côté médical, l introspection est nécessaire pour progresser. La communication reste, quant à elle, l élément clé pour rétablir un lien de confiance avec le patient permettant de conduire, ensemble, un programme thérapeutique efficace. Anatomical and psychological sequelae of infections The sequelae of infections spring from the process of infection itself in addition to the curative surgery. All the anatomical tissues may be affected. The skin is impaired by iterative, unsightly scars, chronic ulcerations or ugly flaps. The muscles and tendons may be atrophied or torn, the nerves paralysed and vascularisation altered. The joints may become stiff or unstable. Bones go off-axis, are lengthened or shortened, sometimes amputated. These painful, often irreversible sequelae alter the functional result for the patient. They are particularly important when patients have experienced failures and undergone iterative surgery. These failures owing to infection are a source of concern, incomprehension and mistrust for the patient. How can a patient who trusted the medical establishment and acquired an infection during functional surgery place his trust once again in medicine in the hope of achieving a cure? On the medical side, introspection is necessary in order to progress. Communication remains the key to re-establishing a trusting relationship with the patient in order, together, to implement an effective therapeutic programme. MA1.7 Le coût des infections The cost of infections Joseph Hajjar, Médecin Hygiéniste, CH Valence, France L infection du site opératoire (ISO) est la complication la plus fréquente en chirurgie, parmi les infections et parmi les complications. L ampleur du problème peut être résumé en 2 chiffres : 6,5 millions d opérés par an en France, 2 % d ISO toutes chirurgies confondues. Les répercussions de ces ISO peuvent s apprécier sur un triple plan : celui de l opéré (et de son entourage), celui du professionnel (et de l établissement), celui de la société. Agenda : Surgical site infections (SSI) are the most common complications in surgery. The magnitude of the problem can be summed up in 2 figures: 6.5 million patients are operated on every year in France, with an overall SSI rate of 2%. The repercussions of these SSIs can be assessed on three levels: the patient (and entourage), health professional (and health establishment), and society. Our skin : beware of bacteria! Ludovic RICHARD La préparation cutanée est probablement une des étapes les plus importantes de la lutte contre les infections. Tous les acteurs doivent être, en effet, préparés avec la même rigueur. Elle est totalement transversale nous impliquant tous et nous laissant tous égaux. Bien sûr le patient est au centre du dispositif, mais les aides-soignantes et infirmières doivent respecter les règles d hygiène de leur peau afin que la préparation cutanée du patient ait toute son efficacité. De même les panseuses, aide opératoires, anesthésistes et chirurgiens doivent aussi se préparer pour pouvoir préparer le patient et l endormir puis l opérer ainsi en tout respect. Car si un patient qui ne respecte pas les règles peut être considéré comme inconscient ou mal informé ; un acteur de soin, lui, devient négligent et fautif. En chirurgie programmée, la première étape de la préparation cutanée est d informer le patient en consultation sur la façon de se préparer. Le patient doit savoir ce qu il ne peut pas faire en lui expliquant le rôle de la peau et du développement des staphylocoques. Ainsi il épilera ou tondra la région à opérer mais surtout il ne la rasera pas, il se coupera les ongles mais il n arrachera pas la peau, et bien sûr il retirera tous ses piercing! Of course, the patient lies at the heart of this procedure; however, orderlies and nurses must subject their own skin to the same rules of hygiene if patient skin preparation is to be truly effective. Similarly, chief operating room nurses, auxiliary nurses, anaesthetists and surgeons also have to prepare properly before preparing the patient, putting him to sleep and operating. While a patient who fails to respect the rules can be viewed as irresponsible or poorly informed, a health professional is negligent and culpable. In elective surgery, the first stage in skin preparation consists in informing the patient how to prepare for the operation. The patient must know what he cannot do, by explaining to him the role of the skin and the development of staphylococci. He must therefore pluck or sheer the region to be operated on, but not shave it under any circumstances. He must cut his nails but not tear the skin. An original research has been performed in our Institute to show how a patented anti-bacterial finishing of commonly used hospital gowns and textiles may significantly prevent multi-resistant nosocomial bacterial contamination of hospital textiles both in vitro and in the clinical setting. This low-cost, effective technology may open new pathways to reduce the impact of nosocomial infections. Staphylococcus aureus (SA) is both a commensal and a pathogen responsible for various anatomical site infections in man. The link between nasal SA carriage and infection was first described by Danbolt in 1931 in a patient with furunculosis. In the last 20 years, it has been proven that high nasal SA carriers have a three to six times higher risk of infection than non-carriers or low level carriers, particularly dialysed patients. In surgical patients, SA is the pathogen most often responsible for surgical site infections (SSIs), particularly in clean surgery. Polyethylene and titanium discs covered with biofilm formed by isolates of Pseudomonas aeruginosa and Staphylococcus aureus were treated with sonication, scraping, dithiothreitol and N-acetylcysteine, in order to remove adhered bacteria. The overall data demonstrated that the treatment with dithiothreitol was significantly more efficient than sonication, scraping and N-acetylcysteine in removing bacteria within biofilm (p<0.05). The in vitro testings were confirmed by in retrieved, infected hip and knee implants. Dithiothreitol treatment may open a new, low cost and easy way to diagnose implant-related infections. MA5.3 Diagnostic rapide des infections ostéo-articulaires à staphylocoque Rapid diagnosis of staphylococcal bone and joint infection Frédéric LAURENT, Centre de Biologie Nord Hôpital de la Croix Rousse, Lyon, France Le but de notre étude était d évaluer le test PCR Xpert MRSA/ SA SSTI RT (Cepheid) directement sur des échantillons articulaires en periopératoire. Une analyse rétrospective de 91 échantillons choisis (SASM, n=63 ; SARM, n=9, négatif, n=19) a d abord été effectuée pour valider l utilisation du test (liquide synovial, n=24, os, n=42, biopsie tissulaire n=25). Puis 135 échantillons de 105 patients (45 infections articulaires sur implant, 3 spondylodiscites, 14 arthroses septiques aigues et 33 contrôles non infectés) ont été testés prospectivement. Dans tous les cas, les résultats moléculaires ont été comparés aux cultures microbiennes. Etude rétrospective : Se et Sp pour la detection de SA étaient respectivement de 94,4% et 100%. Tous les échantillons SARM + ont été détectés. Etude prospective : Se, Sp, valeur prédictive positive et valeur prédictive négative pour la détection de SASM, SARM et MRCNS (staphylocoques à coagulase négative résistant à la méticilline) dans les échantillons cliniques étaient respectivement de 100%, 97%, 96% et 100%. Le temps moyen de travail pour la culture standard et Xpert MRSA/SA SSTI était respectivement de 25 minutes vs 2 minutes. Le temps entre admission et notification des résultats étaient en moyenne de 72 heures vs 72 minutes. La détection de résistance à la méticilline améliore le traitement antimicrobien car la vancomycine peut ainsi être évitée dans un grand nombre de cas. Le test Xpert MRSA/SA SSTI est une méthode de détection rapide et fiable des SARM, SASM et MRCNS dans les échantillons periopératoires. Il permet une amélioration spectaculaire de la prise en charge des patients atteints d infections ostéo-articulaires. D autres études seront nécessaires pour évaluer l impact clinique et économique de ce test. The aim of our study was to evaluate Xpert MRSA/SA SSTI RT-PCR assay (Cepheid ) directly on peri-operative osteoarticular samples. A retrospective analysis was first performed on 91 selected samples (MSSA, n=63; MRSA, n=9; negative, n=19) in order to validate the use of the assay (synovial fluid, n=24; bone, n=42; tissue biopsy n=25). Then, 135 samples from 105 patients (45 prosthetic joint infections, 3 spondylodiscitis, 14 acute septic arthritis, and 33 non infected controls) were tested prospectively. It can dramatically improve the clinical management of patients suffering from osteoarticular infections. Further studies must be conducted in order to evaluate the clinical and economical impact of such an assay. En plus de sa vocation médico-économique pré éminente, il lui reste encore un rôle de description de l épidémiologie hospitalière. Il est possible de signifier, directement ou non, les infections des patients dans les résumés des séjours ainsi que le caractère nosocomial de celles-ci. Les différentes publications ayant trait aux signalements des infections par le biais du PMSI concluent à l insuffisante sensibilité du PMSI pris isolément. L impact économique de ces évènements dans le PMSI a pourtant été récemment renforcé et devrait augmenter le taux de description. Le rôle du PMSI et des personnes en charge de ce secteur peut être renforcé par couplage à d autres systèmes d information (signalement interne des infections postopératoires, résultats de bactériologie), rapprochement d organisation (Equipe opérationnelle d hygiène, COVIR, CLIN) ou de rencontre de personnes clés (DSSI, RAQ, infirmière hygiéniste). La médecine, mais aussi l automobile ou le transport aérien, fourmillent d exemples de technologies performantes développées sur des bases présentant initialement des niveaux de risques qui seraient aujourd hui inacceptables. L infection sur une prothèse totale de genou reste une situation dramatique pour le patient, plus encore que pour une prothèse de hanche. Les patients sont, en effet, plus âgés, les taux de succès inférieurs et ce quelques soient les schémas thérapeutiques proposés (excision-lavage, changement en 1 ou 2 temps, etc.), et les conséquences fonctionnelles bien plus graves. Il est donc important de mettre en œuvre d emblée, via les CRIOA, des circuits de prise en charge d emblée optimaux pour tenter de minimiser les conséquences de la survenue d une infection. L expérience de la Mayo Clinic a été rapportée par Haleem et al (CORR, 2004). 94 patients, dont deux anthroplasties bilatérales, ont été suivis pendant 7,2 ans. 50 genoux ont du être réopérés, 9 prothèses ont été enlevées suite à une réinfection, et 6 genoux ont été repris pour descellement. Le taux de survie sans réinfection est de 93,5% à 5 ans et de 85% à 10 ans. Agenda : The rate of infection after total knee has decreased over the last three decades.
