Bennani Fillette de huit ans à présent, porteuse d anomalies vertébrales, l une majeure en Th6, l autre moins accusée en L5. Sous corset est apparue une vertèbre Th6 presque normale d aspect. La petite scoliose initiale et l encoche importante de la vertèbre sus-jacente sont actuellement presque résorbées. Le léger déséquilibre gauche constatable ici était dû à l inexpérience de perfectionnements récemment apportés à notre système. Actuellement (et après correction pour cette patiente), nous l évitons en prolongeant obliquement la coque gauche d environ quatre centimètres vers le bas. Ora (e dopo correzione per questa paziente), lo evitiamo prolungando oblicamente la cocca sinistra di quasi 4 centimetri verso il basso. This eight years old girl has vertebral anomalies. One of them was a tiny wedged sixth thoracic vertebra, the lower one being a minute wedging of L5. After two years of bracing has appeared an almost whole sixth thoracic vertebra. The little scoliosis which did exist at six as well as the notch on Th5 have almost been rubbed out. Notice a remaining little overhang towards left. It was a result of a certain lack of experience concerning recent progresses in our system. For this time, we build an oblique, some 4 centimeters longer brace wall below at left side. 2 Il y a très peu de témoignages sur la normalisation sous corset de vertèbres déformés, que ce soit dans le plan antéro postérieur, comme dans la maladie de Scheuermann ou dans un plan oblique, ce qui se passe dans la scoliose grave. A ce sujet, nous avons présenté à Viersen, Berlin, Bad Sobernheim (Allemagne) Prague, Budapest, Katowice (Pologne), Beijing (Chine) une série dans laquelle, entre autres facteurs pathologiques, la cunéisation a diminué de plus de 60% en moyenne après traitement par corset, sans qu aucun cas ne se soit aggravé. Mais celui que nous présentons ici est plus étonnant encore et concerne un moignon de vertèbre congénital. Esistono pocche testimonianze sulla normalizzazione sotto corsetto, di vertebre deformate, che sia nel piano antero-posteriore, come nella malattia di Scheuermann o nel piano obliquo, cio che soppraviene nelle scoliosi gravissime. A questo proposito, abbiamo presentato a Viersen, Berlin, Bad Sobernheim (Germania), Prague, Budapest, Katowice (Pologna), Beijing (Cina) una serie nella quale, tra altri fattori patologici, la cuneiformizzazione è diminuita di più del 60% in media, dopo trattamento con corsetto, senza che nessun caso si sia peggiorato. Ma quello che presentiamo qui è più stupendo ancora e riguarda un moncone di vertebra congenita. R.E.R. Volume 13, N 39 p One can hardly find a report on the normalization of wedged vertebrae, in the antero posterior plane as in the Scheuermann s disease and in what concerns lateral wedges of severe scoliosis. Concerning this matter, we already showed in Prag, Budapest and Katowice (Poland) and Beijing (China) a series in which wedging has decreased in an average percentage of more than 60% after a treatment with brace, without a single worsening (Figure 1). What we should like to show here, we never could find a similar case in the world literature. It concerns a congenital scoliosis. Figure 1. A l étage thoracique, elle présente un moignon vertébral composé du pédicule droit entouré de un à deux millimètres de tissu osseux. A l étage lombaire, c est L5 qui est cunéisée à droite. In the thoracic region, the sixth vertebra is a tiny wedge consisting in a right pedicle surrounded by some 1_2 millimeters of bony tissue. This small piece of vertebra is separated from the over- and underlying vertebrae by a distance which is similar to the thickness of 2A 2B

3 R.E.R. Volume 13, N 39 p the neighbouring intervertebral spaces. In the lumbar area, the fifth vertebra is right wedged. Two slight scoliotic curvatures underline both anomalies. A brace is given, the goal of which being a preparation to the surgical taking off of the wedge. As we ought to do it, the brace has wide and deep concave sided expansion spaces. The only difference with the braces aiming at correcting idiopathic scoliosis is the absence of any derotative action. There is no rotation in congenital scoliosis. 2C 2D Figure 2.A. 4 individualisé, séparé des vertèbres sus- et sousjacentes (qu il encoche) par un espace de même largeur que les disques avoisinants. Dans la région lombaire, la cinquième vertèbre est cunéisée à droite. Deux courbures scoliotiques modérées soulignent les deux niveaux. Un corset est prescrit, espérant simplement limiter la petite scoliose et préparer l intervention d exérèse dans de bonnes conditions. Le corset comporte les très volumineux espaces d expansion qui sont la base de notre système. Seule différence avec les corsets de scoliose idiopathique, nous n avons pas cherché de dérotation, car dans les scolioses congénitales, il n y a pas de rotation. Deux ans après, une vertèbre d aspect presque normal s est constituée. (Figure 1 D) Les lignes des plateaux sont parallèles. La hauteur de la vertèbre est un peu moindre que celles des vertèbres normales, sus- et sous-jacentes ; l espace clair sus-jacent est lui aussi un peu moins clair et un peu moins haut. Il semble qu un pédicule se constitue symétriquement à celui préexistant au niveau du moignon. La petite scoliose s est pratiquement effacée aux deux étages (Figure 2E). Discussion. Ceci est un cas extrême (et extrêmement favorable) de cunéisation vertébrale guérie par corsets. Nous nous étonnons de voir si peu de recherches et de résultats publiés dans la littérature. Concernant le diagnostic, ces deux étages anormaux sont certainement de nature congénitale. Les symptômes et les aspects radiologiques des vertebra plana sont différents. La cunéisation serait apparue récemment. Elle s accompagnerait alors d une gibbosité angulaire, en arrière, dont le sommet serait droit. L atteinte serait unique. Rien de tout cela. On perçoit à peine la scoliose à l examen clinique. En ce qui concerne les cunéisations vertébrales observées dans différents contextes. On en rencontre dans la maladie de Scheuermann, dans les scolioses graves et, comme ici, les hémivertèbres congénitales. We would like to discuss the wedging involving various pathologic situations, as the Scheuermann s disease, severe scoliosis, and, as here, congenital half vertebrae. Concernant la maladie de Scheuermann. Les vertèbres y sont cunéisées dans le plan antéro postérieur. Au mot «Scheuermann» inscrit dans le cartouche d un moteur de recherche d internet, parmi les quelques réponses, nous en avons scruté 250. Peu d entre elles, à notre surprise, mentionnaient l usage du corset. Une seule signalait l éventualité de la diminution de la cunéisation par corset. Nous n avons trouvé aucun résultat chiffré. Nous ne nous étonnons pas d ailleurs de la modicité des résultats obtenus dans cette affection qui devrait être facilement curable. En effet, les corsets que nous voyons un dalle vertebre sotto e sopra (che intacca) da uno spazio di stessa larghezza dei dischi vicini. Nella regione lombare, la quinta vertebra è cuneiforma a destra. Due curve scoliotiche moderate sottolineano i due livelli. Un corsetto è prescritto, sperando semplicemente limitare la piccola scoliosi e preparare l intervento di exeresi in buone condizioni. Il corsetto comporta i voluminosi spazi d espansione che sono la base del nostro sistema. Unica differenza coi corsetti di scoliosi idiopatica, non abbiamo ricercato la derotazione, perchè nelle scoliosi congenite non c è rotazione. Due anni dopo, una vertebra di aspetto quasi normale si è costituita (Figura 1D). Le linee dei piatti sono parallele. L altezza della vertebra è un po minima di quella delle vertebre normali sotto et sopra ; lo spazio chiaro sopra è anche lui un po meno chiaro e un po meno alto. Sembra che un pedicolo si costituisca simmetricamente a quello pre esistente al livello del moncone. La piccola scoliosi si è praticamente cancellata ai due piani (Figura 2E). Discussione. Questo è un caso estremo (e specialmente favorevole) di cuneiformizzazione vertebrale guarita dai corsetti. Ci stupiamo di vedere cosi poche ricerche e risultati pubblicati nella letteratura. In quanto riguarda la diagnosi, questi due piani anormali sono certamente di natura congenita. I sintomi e gli aspetti radiologici delle vertebra plana sono diversi. La cuneizzazione sarebbe apparse da poco. Si accompagnerebbe allora di una gibbosità angolare, indietro, di cui l apice sarebbe dritto. L affetto sarebbe unico. Niente di tutto questo. All esame clinico, si percepisce appena la scoliosi. Per quanto riguarda le cuneizzazioni vertebrali osservate nei diversi contesti. Si incontra nel morbo di Scheuermann, nelle scoliosi gravi e, come qui, le emivertebre congenite. Per quanto riguarda il morbo di Scheuermann. Le vertebre sono cuneizzate nel piano anteroposteriore. Alla parola Scheuermann iscritta per un motore di ricerca internet, in mezzo alle qualche 4000 risposte, ne abbiamo scrutato 250. Poco entro loro, alla nostra sorpresa, menzionano l uso del corsetto. Soltanto una segnalava l eventualità della diminuzione della cuneizzazione con corsetto. Non abbiamo trovato nessun risultato colle ciffre. Non ci stupiamo neanche della modicità dei risultati ottenuti in quest affezione che dovrebbe facilmente essere curabile. In effetti, i corsetti che vediamo un po da pertutto in tutto il mondo all occasione di formazioni che ci effetuiamo, peccano spessissimo dalla presenza di appoggi in zone concave (Figura 3). Pertanto, è facile controllare una deviazione antero-posteriore all età di 14 anni, perchè si produce soltanto in due piani dello spazio e per essere corretta al massimo, esige soltanto due sistemi tre punti. The lines marking the vertebral plates are parallel and the intervertebral spaces are almost similar to those between the other vertebrae. The little scoliosis has almost been rubbed out on both heights (Figure 2E). Discussion. Concerning diagnosis, those anomalies are doubtless of congenital origin. A vertebra plana should have different symptoms. The wedge, appeared for a short time, should be visible on the back side of the body in form of an angle, the apex of which being behind and at right side. Moreover, A vertebra plana should have another radiological aspect and should be isolated, not bifocal. We would like to discuss the wedging involving the antero-posterior plane, as in the Scheuermann s disease, the lateral wedging often present in severe scoliosis, and the congenital half vertebrae. Concerning the Scheuermann s disease, there are very few reports. After having written the word Scheuermann on a research motor of internet, we got almost 4000 answers and read some 250 of them. We surprisingly saw only a few authors who had mentioned braces, and only one who writes, that a wedged vertebra can be rendered more normal by bracing. We are not surprised by those poor results reported upon brace treatment. In the whole world, we almost every times saw braces the concave sided expansion of which being hindered by huge brace parts as in figure 3A. An antero posterior wedging however is most easy to cure at the age of 14 years by a brace when really based on the three points system.

5 peu partout dans le monde entier à l occasion des formations que nous y effectuons pêchent très, très souvent par la présence d appuis en zones concaves. (Figure 3). Il est pourtant bien facile de contrôler une déviation antéro-postérieure à l âge de 14 ans, car elle ne se produit que dans deux plans de l espace et n exige pour être corrigée au maximum que deux systèmes trois points. Lorsque le corset est bien fait, la cunéisation disparaît. En ce qui concerne la scoliose, jamais nous n avons vu, lu ni entendu signaler la reconstitution de la symétrie d une vertèbre cunéisée par la scoliose. En recherchant les rares clichés radiographiques lisibles publiés et sur internet, nous n avons rencontré que des modifications minimes et variables entre clichés de départ et sous corset. Concernant le minuscule moignon qui a généré une néo-vertèbre : Nous n avons jamais vu, lu ni entendu qu une vertèbre se soit constituée spontanément ou sous corset à partir d un moignon. La seule publication que nous avons pu relever et qui se rapproche de notre cas se trouve sur internet skoliose/fehlbildung/htm. Dans les deux cas présentés, la minime inflexion scoliotique s est très bien redressée. et les vertèbres anormales avaient grandi de façon homogène en diminuant leur cunéisation, respectivement de 44 à 27 et de 43 à 12. Ce coefficient est obtenu en soustrayant la hauteur du petit côté de la vertèbre, de celle de son grand côté. On divise par la largeur et on multiplie par 100 Le coefficient est nul si la vertèbre est symétrique et très grand à 50. Cela peut être considéré comme un très bon résultat. Mais jusqu à plus ample information, le cas que nous présentons est unique : la génération sous corset d un corps vertébral presque normal à partir d un minuscule moignon. Tutto questo impedisce tanto la correzione quanto la crescita. Di più, tutti questi appoggi concavi operano delle riflessioni di forza che raddoppiano la pressione degli appoggi e nuociono gravemente al comfort. B. Abbiamo tentato di modificare la vista. Ci vuole una larga aperta ai lombi. Le linee di forza del corsetto devono passare sui lati e non dietro. Le pareti devono divergere in tal modo che nessun punto di pelle concava non tocchi il torace sopra come sotto degli apici, qui segnati da cerchi neri, qualsiasi sia la posizione del paziente. Per quanto riguarda la scoliosi, non abbiamo mai visto, letto ne sentito segnalare la ricostituzione della simmetria di una vertebra cuneizzata dalla scoliosi. Ricercando le rare lastre leggibili, pubblicate e su internet, abbiamo incontrato soltanto modifiche minime e variabili tra lastre iniziali e sotto corsetto. Relativamente al piccolo moncone che ha generato una neo-vertebra : non abbiamo mai visto, letto ne sentito che una vertebra si sia costituita spontaneamente o sotto corsetto a partire di un moncone. L unica pubblicazione che abbiamo potuto rilevare e che si avvicina al nostro caso, si trova su internet http: // htm. Nei due casi presentati, la minima inflessione scoliotica si è raddrizzata benissimo e le vertebre anormali erano cresciute di modo omogeneo diminuando la lordo cuneizzazione, rispettivamente da 44 a 27 e da 43 a 12. Under the buttocks, the corresponding parts are hook-shaped and restrict growth and correction a similar way. B. I tried to overwork the photograph. Imagining a wide window behind the lumbar area, the brace edges around the window being made far from the lumbar skin. Over the apex (dark circle) the brace parts are still directed forwards, but also upwards in spite of downwards. On the lower posterior brace, the parts are now no more hook shaped, but only vertical, directed only forwards. All those details are most important. Concerning idiopathic scoliosis, we never heard or read of a recovery of symmetry after bracing when a wedge shape was present. Looking at the rare radiographs published which were visible in details, we could see some changes after a brace treatment, but in a very variable way. Concerning the congenital half vertebrae, we never heard of the normalization of a wedged vertebra even spontaneously or after a brace treatment. The only publication which we met and which nears our case is on internet http: // In the two cases of this paper, the small scoliotic deformation had well been mastered. The anormal vertebrae had grown homogeneously, the wedge shape of which having respectively decreased from a coefficient of 44 to 27, and 43 to 12. This coefficient is got while substracting the smaller side of a vertebra from the greater one, then dividing by width and multiplying by 100. A coefficient 0 shows no wedging. With 10 is the wedging slight and with 50 very high. Those two cases can be considered as very good results. But till a wider information comes, no case of a tiny half vertebra which has developed into an almost symmetrical one has been reported. The present case was presented by us, among other places, in Budapest, Prague, Beijing and Katowice to many world specialists. No one had seen, read or heard of a similar event. The only treatment proposed was up to our times the excision of the wedge. And the result was only favourable when a good brace was immediately delivered.

Conclusion. Depuis des années maintenant, personne n a le droit d affirmer que les corsets sont inefficaces. Pour que la phrase soit honnête, il importe d y ajouter, par exemple :»du moins, la plupart de ceux que je vois», ou plus simplement: «dans mon service». Il est exact que les mauvais corsets sont inefficaces, surtout dans la mesure où ils ne comportent pas d espaces d expansion suffisamment vastes et profonds. Pour faire accepter ces espaces indispensables, il faut et il suffit d expliquer aux scoliotiques et surtout à leurs mères que ce sont des espaces de liberté, pourquoi et comment. D ailleurs, avec la précision qui augmente d année en année avec le progrès, ces espaces sont devenus très discrets à présent. Le médecin traitant et celui contrôleur des Caisses d Assurances devraient eux aussi veiller à ne pas avoir une influence négative à ce sujet. Une somme énorme de connaissances en biomécanique est indispensable pour mener à bien un traitement orthopédique de la scoliose, tant à la phase de fabrication qu à celles d application et de maintenance. Remarquons aussi que dans le cas particulier des scolioses congénitales, comme celui-ci, il ne faut pas rechercher la dérotation car il n y a pas de rotation. piccolo lato della vertebra, di quella del suo late grande. Ma fino a maggiori informazioni, il caso che presentiamo è unico : la generazione sotto corsetto di un corpo vertebrale quasi normale a partire di un moncone minuscolo. Abbiamo fatto la domanda a dei specialisti di alto livello di tutto il mondo, a Budapest, Prague, Berlin, Bad Sobernheim, Katowice e Pékin. L unico trattamento proposto fin allora nel caso di un emivertebra era la sua exeresi. D altronde, l effetto ne è realmente favorevole, soltanto se va messo subito dopo un corsetto. Conclusione. Da anni ormai, nessun ha il diritto di affermare che i corsetti sono inefficaci. Affinchè la frase sia onesta è importante aggiungere ad esempio : almeno, la maggior parte di quelli che vedo, o più semplicemente nel mio reparto. E giusto che i pessimi corsetti sono inefficaci, soppratutto nella misura in cui non comportano spazio di espansione abbastanza vasti e profondi. Per fare accettare questi spazi indispensabili, bisogna e basta spiegare ai scoliotici e specialmente alle loro madri che sono dei spazi di libertà, perchè e come. D altronde, colla precisione che va aumentando d anno in anno, questi spazi sono diventati discreti oramai. Il medico curante e quello del Controllo della Previdenza Sociale dovrebbero anche loro stare attenti a non avere un influenza negativa a questo proposito. Un enorme somma di conoscenze in biomeccanica è indispensabile per condurre a bene un trattamento ortopedico della scoliosi, tanto alla fase di fabbricazione quanto a quelle di applicazione e manutenzione. Notiamo anche che nel caso particolare delle scoliosi congenite, come questo, non si deve ricercare la derotazione perchè non esiste rotazione Conclusion R.E.R. Volume 13, N 39 p For years, no one has a right to state that braces are not efficient. This statement is honest when a part of sentence like this one is added :»As I generally see them» or «in my hospital». Bad braces actually are not efficient, one of the reason of it being the lack of concave sided expansion rooms. Those wide spaces are accepted by mothers when they get a clear explanation of their role, being a condition for freedom and curvature correction. The treating doctor as well as the one controlling for the assurance companies are also requested not to have a negative role in this matter. A huge amount of knowledge is requested when treating a scoliosis by brace. This knowledge is available for manufacturing, but also for the first essay and later for further adjusting the brace on each state of the scoliotic patient. A last notice. Do not try to derotate a congenital scoliosis : it is not rotated.
