Chimioembolisation : consensus? 1. A un effet nécrosant sur la tumeur et retarde la survenue d une invasion macro vasculaire 2. Son principale effet délétère est la dégradation de la fonction hépatique qu elle peut entraîner (procédures non sélectives) 3. Elle peut avoir un effet bénéfique modérée sur la survie de patients biens sélectionnés

Complications Syndrome post-embolisation (PES) : douleur abdominale ou fièvre et cytolyse (augmentation de l aspartate amylase transferase (ASAT) sérique ou de l alanine amylase transferase (ALAT) sérique à au moins plus de 3 fois la normale). Cholécystite ischémique / Infarctus splénique / Hémorragie gastro-intestinale / Défaillance rénale aigue / Embolie pulmonaire. Atteinte des voies biliaires et formation de biliome intra-hépatique. Nécrose hépatique / nécrose tumorale / Abcès hépatique. Défaillance hépato-cellulaire aigue : poussée d ascite, encéphalopathie, augmentation de la billirubine sérique et diminution du taux de prothrombine (TP).

Key points In elderly fragile patient with large solitary HCC endoarterial is a rational first therapeutic approach in attempt to debulk the tumor and have a test of time of evolution of tumor disease.
