Recommanda/ons lors de la transi/on de la warfarine au dabigatran Cesser l administration de la warfarine et surveiller le RIN Débuter l administration du dabigatran lorsque le RIN est 2,0 Évaluer les facteurs de risque d hémorragie du dabigatran ou toute atteinte rénale Administrer 150 mg bid de dabigatran, ou, s il y a un risque d hémorragie, utiliser la dose de 110 mg bid Recommanda/ons lors de la transi/on de la dabigatran à an/coagulant par voie parentérale et vice versa Amendre 12 heures après administra/on de la dernière dose de dabigatran avant de passer à an/coagulant par voie parentérale Instaurer dans les 2 heures précédent l administra/on prévue de la dernière dose de l autre an/coagulant par voie parentérale ou au moment de l interrup/on si tx con/nu Quoi faire? Patients âgés de plus de 80 ans 110 mg bid préférable et aussi si plus de 75 ans avec un facteur de risque de saignement Si oubli d une dose Reprendre aussitôt que possible mais si moins de 6 heures avant la prochaine dose, ne pas reprendre. Y a-t-il un antidote NON sauf produits sanguins (FVIIa recombinant ) Si pas de RNI, comment savoir que fonctionne!!!, mais possibilité de doser temps de céphaline et thrombine pour détecter présence d un anticoagulant si urgence Que faire si besoin de CAVE Ne pas cesser et certain de ce type de patients faisait partie de RELY 6

Eliminated by metabolic degradation (approximately 2/3 of administered dose) as well as by direct renal excretion of unchanged active compound (approximately 1/3 of administered dose). 1 Absence of a clinically relevant interaction potential by rivaroxaban through induction or inhibition of major CYP isoforms or the P-gp transporter confirmed in vivo. Concomitant systemic treatment with strong inhibitors of both CYP3A4 and P-gp (e.g. ketoconazole, itraconazole, voriconazole, posiconazole, ritonavir) is contraindicated. 1 None. Grapefruit juice, a moderate CYP3A4 inhibitor, is not expected to cause a clinically relevant increase in rivaroxaban exposure. 1 Apixaban - ARISTOTLE Déjà révélé supérieur à l'aspirine dans l'essai AVERROES, d'ailleurs interrompu prématurément pour cette raison (réduction de 55% du risque d'avc, sans augmentation des saignements majeurs Évalué par rapport à des doses de warfarine ajustées sur un INR de 2-3, chez 18 201 patients présentant à la fois une FA et au moins un facteur de risque additionnel d'avc (âge > 75 ans, antécédent d'avc, d'ait ou d'embolisme systémique, IC symptomatique dans les 3 mois précédents, FEVG < 40%, diabète, HTA traitée). Dans l'effectif recruté, le CHADS2 score moyen était de 2,1. La dose d'apixaban habituelle était de 2 x 5 mg / j. Une dose réduite, de 2 x 2,5 mg/j a été utilisée chez les patients (4,7%) présentant au moins deux des critères suivants : âge > 80 ans, poids < de 60 kg, créatinine sérique > 133. Les patients présentant une insuffisance rénale sévère exclus de l'étude. Résultats ARISTOTLE Suivi moyen de 1,8 ans, Temps passé à l'inr cible était 62,2% en moyenne Incidence annuelle du critère primaire a été de 1,27% parmi les patients traités par apixaban, contre 1,60% parmi les patients sous AVK, soit un RR de 0,79 en faveur de l'apixaban, correspondant à un p<0,001 pour la non infériorité, et égal à 0,01 pour le test de supériorité. Les saignements majeurs (critères ISTH), leur incidence annuelle était de 2,13% dans le groupe apixaban, contre 3,09% dans le groupe warfarine (RR=0,69 ; p<0,001) En rapport avec ce moindre risque hémorragique avec l'apixaban, on note que le bénéfice de l'anti-xa sur les AVK est dû à la réduction des AVC hémorragique (incidences : 0,24% vs. 0,47%/an ; RR=0,51 ; p<0,001). Vis-à-vis des AVC ischémiques ou d'origine incertaine, l'écart entre apixaban et warfarine n'est pas significatif (0,97% vs. 1,05%/an ; RR=0,92 ; p=0,42). 8
