For non-congo hosts, please also provide proof of their legal status in Congo (DRC). Yellow Fever: A certificate for vaccination against yellow fever is obligary. Please submit your original yellow vaccination book with your application form. Other vaccines and anti-malaria treatment are recommended. Consult your primary physician for details. For non-canadian passport holders only: Proof of Legal Status in Canada is required. Please include a copy of your Permanent Resident Card, Work Permit or Student Visa.
