Donc Cancer colique non métastatique MSI: groupe hétérogène Sporadique et Lynch, BRAF mutés et non mutés, CIMP+ ou non Etude des ces pts reste difficile car peu fréquent et les études mèlent différents stades, traitements MSI est un marqueur génétique utile au dépistage du syndrome de Lynch et doit être proposé à tout malade > 60 ans qqsoit le stade MSI a une réelle valeur pronostique pour les stade II et aide à la décision thérapeutique dans cette population (pas de CT adjuvante)
Donc Pourrait être un facteur de résistance au 5FU mais pas d effet délétère sur les dernières données FOLFOX aussi efficace sur MSI ou MSS chez les stades III Questions en suspens: Stade II T4 MSI + autre facteur de mauvais pronostique??? Stade III ne pouvant recevoir de l oxaliplatine???

Pour Conclure On peut recommander: MSI patient< 60 ans tous stades et stade II et IV qqsoit l âge RAS en situation métastatique (anti-egfrs) BRAF en situation métastatique (traitement agressif dés L1) MSI+RAS+BRAF comme critère de stratification pour tous les essais adjuvants à venir
Pour Conclure Pour moi dans un monde idéal: MSI <60 ans MSI, RAS, BRAF chez tous les stades II,III, IV Pour mieux connaître le pronostique de nos patients non métastatiques Pour aider à la décision de CT adjuvante (stii) Pour aider au choix thérapeutique optimal à la rechute pour les rechuteurs
