We are pleased to inform you that the CLSCs of the CSSSG de Gatineau will help you meet the registration requirements for iunization (needed for clinical experience), in accordance with the Protocol d iunisation du Québec (PIQ) and the Ontario teaching institutions. ASAP, contact the CLSC near your home to make an appointment. Identify yourself as a new Cegep Heritage College nursing student. However, to gather the required information, your collaboration is needed. You must: Make an appointment with the CLSC in your residential sector, as soon as possible. There might be a one (1) month delay; Bring your personal vaccination record (booklet or card) when you go for your appointment, so that the nurse can update your vaccination history. You should also check with your parents or guardians whether you have ever had either a severe or mild case of varicella (chicken pox); Bring the College s Iunization Record, and complete the personal identification section before the appointment. The nurse will add the iunization information; Bring all vaccination related documents (e.g. proof of vaccination submitted to your school, vaccination records, etc.), so that we can attest to the fact that your iunization status is up-to-date. Please note that the number of vaccines you will receive depends on your current vaccination status (have you received all your vaccines and boosters in accordance with the Québec vaccination schedule?). If you are a new Canadian Citizen, go to the iigration clinic located in Hull CLSC to obtain a verification of your iunization status (vaccines previously received). If we have no proof of vaccination (e.g. vaccination record/booklet) you will have to receive all the vaccines indicated in the iunization schedule, which you will find attached to this information sheet. Veuillez noter que le nombre de vaccin à recevoir dépend de votre statut vaccinal actuel (avez- vous reçus tous les vaccins et rappels selon le calendrier vaccinal du Québec?). Si vous êtes nouvellement arrivés au Canada, présentez-vous à la clinique d iigration située au CLSC Hull afin d obtenir une vérification de votre bilan de vaccination (vaccins reçus antérieurement). Si nous n avons aucune preuve vaccinale (ex. carnet de vaccination), vous recevrez l ensemble des vaccins suggérés sur le calendrier d iunisation que nous annexons à cette feuille. 3 CONCERNING THE MEDICAL & IMMUNIZATION CERTIFICATE Introduction for Physician A nursing student must be a stable, responsible individual possessing good manual skills and physical mobility. She/he must be in good physical and mental health, be free of contagious disease, have an up-to-date iunization status, be able to receive the annual influenza vaccine, and be able to work safely in a clinical environment with potentially harmful substances and biological specimens. Re: Pregnancy The Nursing Department and Cegep Heritage College have to abide by the Quebec`s guidelines from the La Coission santé et sécurité au travail (CSST) as they pertain to the pregnant health care worker. Consequently, a pregnant nursing student must realize that she will probably have to add one (1) more year to her program of studies because of her automatic withdrawal from all clinical (practicum) courses during her pregnancy and postpartum period. Re: Seasonal flu iunization The yearly iunization is mandatory for clinical placements in Ontario where most of the clinical placements will take place. Only a student with allergies to the components of the vaccines is exempted from the vaccine. She/he must provide a medical certificate attesting of her/his allergy status. Re: Tuberculin Skin Testing (TST/Mantoux) The TST is required annually by the health institutions in Ontario where most of the student`s clinical placements will take place. Because the Province of Quebec does not recoend annual testing anymore, a nursing student from Cegep Heritage College must go to a private iunization center or private clinic to get the TST. The attached medical certificate and the completed iunization profile are required for all conditionally admitted Nursing Students. These completed forms must be submitted before registration. Student will not be permitted to register if these forms are not submitted as required! Elle/ il doit être en bonne santé tant sur le plan physique que mental; elle/ il ne doit pas être atteint d une maladie contagieuse, et doit avoir un profile iunitaire à jour, pouvoirannuellement- recevoir le vaccin contre l influenza, et être en mesure de travailler de manière sécuritaire dans un milieu clinique où se retrouveront possiblement des substances et des spécimens biologiques dangereux. Au sujet de l étudiante enceinte: Le département de soins infirmiers et le Cégep Heritage College doivent respecter les directives de la Coission de la santé et sécurité au travail (CSST) du Québec en ce qui a trait à la personne enceinte œuvrant dans le secteur de la santé. Conséqueent, une étudiante enceinte doit être consciente qu elle devra d ajouter une (1) année à son prograe d études car elle sera retirée des stages pendant sa grossesse et le post-partum. Au sujet de l iunisation contre l influenza: L iunisation annuelle contre l influenza est obligatoire pour les stages en milieux hospitaliers ontariens où ils (stages) se dérouleront en grande majorité. Seulement l étudiante/étudiant qui a une allergie aux composantes du vaccin sera exempté de cette obligation mais elle /il doit soumettre un document médical attestant de cette allergie. Au sujet du test cutané à la tuberculine (TCT): Les milieux cliniques ontariens, où se dérouleront la grande majorité des stages, exigent un TCT annuel. Parce que le PIQ ne recoande plus que ce test soit effectué annuellement au Québec; l étudiante/étudiant en soins infirmiers du Cégep Héritage College doit obtenir le TCT à un centre de vaccination privé ou une clinique privée. Le certificat médical et le profil iunitaire suivants sont requis pour l étudiante/étudiant en soins infirmiers. Les formulaires doivent être soumis avant l inscription. L étudiante/étudiant ne pourra s inscrire tant que les documents n auront été soumis.
