3 OSTEOPOROSIS : Beware of the silent disease FIRST-AID MEASURES FOR FRACTURES A fracture is a broken bone and therefore requires medical attention. While waiting for medical help, or during transportation, don't move the person unnecessarily, so as to avoid further injury. Take these actions: Stop any bleeding. Apply pressure to the wound with a sterile bandage, a clean cloth. Immobilize the injured area. Don't try to realign the bone or push a bone that's sticking out back in. Apply ice packs to limit swelling and help relieve pain until emergency personnel arrive. Don't apply ice directly to the skin wrap the ice in a towel, piece of cloth. Treat for shock. If the person faints or is breathing in short, rapid breaths, lay the person down with the head slightly lower than the trunk and, if possible, elevate the legs. Osteoporosis is the weakening of one s bones, a condition which makes a person prone to fractures. It is often referred to as a silent disease, because many people are unlikely to find out that they have osteoporosis until they suffer a painful fracture. Osteoporosis is much more common in women but men are at risk too. In fact, around 25% of men over the age of 50 have had, or will have, an osteoporosis-related fractures. Bone loss is a natural part of aging, but not everyone will lose so much bone density as to develop osteoporosis. However, the older you are, the greater your chance of having osteoporosis. At the age of 35, a woman can develop osteoporosis if she has had hormonal problems or has taken some drugs which are prescribed after a procedure of removal of her ovaries. Some of the implications of osteoporosis are: more frequent fractures of the spine, hip or wrist, recurring pain, deformity and life-long disability. DID YOU KNOW? There are 206 bones in the human body. Each hand has 27 bones, and each foot 26. Which means that your hands and feet have more bones than the rest of your body! As we age, we lose more bone than we replace. The greatest change in a woman s bone density comes in the five to seven years after menopause. Women who are thin and have a small frame are more likely to develop osteoporosis. Heredity plays a role and so does ethnicity. You might not even realize you have osteoporosis until you experience a fracture or an obvious change in posture. In fact, you could have significant bone loss without even knowing it. Back pain, caused by changes in the vertebrae, may be the first sign that something is wrong. It is easier to prevent osteoporosis than to treat it. Therefore good dietary habits, weight reduction and exercises like walking, as well as a strict control of other diseases are the most important steps in the prevention of osteoporosis. Treatment includes prescriptions for calcium. For example, one may need the equivalent of about three and a half glasses of milk a day. Also vital are vitamin D supplements, hormone replacement therapy, parathormone therapy, the intake of bisphosphonates and calcitonin. All of which should be followed according to a doctor s prescription. One should also minimize the consumption of salty foods such as canned soups and processed meats. Moreover, one should avoid alcohol as heavy consumption leads to bone loss. l PREMIERS SECOURS EN CAS DE FRACTURE Un os cassé nécessite un traitement médical. En attendant l intervention d un médecin, ou pendant le transport à l hôpital, il est important de ne pas déplacer la personne inutilement, de façon à éviter d autres blessures. Ces quelques actions peuvent aussi être utiles : Arrêtez tout saignement. Appliquez une pression sur la plaie avec un pansement stérile ou un morceau de tissu propre. Immobilisez la zone lésée. Ne pas essayer de réaligner l os ou de rentrer un os apparent. Appliquez des compresses de glace pour limiter l enflure et soulager la douleur jusqu à l arrivée du personnel d urgence. Ne pas appliquer la glace directement sur la peau l envelopper plutôt dans une serviette ou un morceau de tissu. Traitez le choc. Si la personne se sent faible ou si elle a le souffle court et rapide, il faut la coucher avec sa tête légèrement plus basse que son tronc et, si possible, lui élever les jambes. Ostéoporose : Attention à la «maladie silencieuse» L ostéoporose est une fragilisation des os qui rend une personne vulnérable aux fractures. Ce trouble est connu comme une «maladie silencieuse» car de nombreuses personnes ignorent l avoir jusqu à ce que survienne une fracture douloureuse. L ostéoporose est beaucoup plus fréquente chez les femmes, mais les hommes aussi courent un risque. En fait, 25% des hommes de plus de 50 ans ont, ou auront, une fracture liée à l ostéoporose. La perte osseuse est un processus naturel quand on vieillit, mais seule une partie de la population perd assez d os pour développer une ostéoporose. Cependant, plus vous êtes âgé, plus vos chances sont grandes d avoir la maladie. Dès 35 ans, il est plus risqué pour une femme de développer l ostéoporose si elle a déjà connu des problèmes hormonaux ou si elle prend certains médicaments prescrits après une ablation des ovaires. L ostéoporose implique généralement des fractures plus fréquentes de la colonne vertébrale, de la hanche ou du poignet, ainsi que des douleurs régulières, des déformations ou même une invalidité à vie. En vieillissant, la perte de densité osseuse s accélère et le phénomène est irréversible. Chez la femme cette perte survient principalement dans les cinq à sept années après la ménopause. Les femmes minces et de faible corpulence sont les plus vulnérables à cette maladie. Orthopedic surgeons can now detect osteoporosis by doing a bone densitometry scan. 4 Dépister et prévenir le cancer du col de l utérus Le cancer du col utérin débute par une prolifération de cellules anormales dans la partie inférieure de l utérus. Ce type de cancer est souvent traité avec succès, à condition d être détecté assez tôt. Prévention Il existe plusieurs moyens de diminuer les risques d être atteint du cancer du col de l utérus. La plupart des cancers de ce type sont causés par le virus du papillome humain (VPH). Il s agit d un virus relativement commun et qui se propage par contact sexuel. Se protéger lors des rapports sexuels, en utilisant des préservatifs, et limiter son nombre de partenaires peut donc réduire le risque de contracter le VPH et le cancer du col de l utérus. Arrêter de fumer est aussi un acte de prévention, car le tabac augmente le risque de contracter cette maladie. Il existe aussi deux vaccins pour se protéger contre le VPH. Indiqués pour les filles et les femmes âgées de 10 à 25 ans, ces vaccins sont administrés en une série de trois injections, échelonnées sur 6 mois. Dépistage Le cancer du col utérin commence par des changements précancéreux, appelés dysplasie. Cette condition peut être détectée par un examen médical connu comme «le test de Pap», ou le «frottis de dépistage». La dysplasie étant généralement traitée avec succès, il est donc important que les femmes se soumettent régulièrement au frottis. Cet examen est conseillé tous les 3 ans aux femmes de 21 à 65 ans. Celles ayant subi une hystérectomie (une ablation totale ou partielle de l utérus) ne doivent plus se faire dépister, ce qui n est pas le cas de celles ayant été vaccinées contre le HPV. Dépister et prévenir le cancer du col de l utérus est essentiel. Parlez-en avec votre gynécologue aujourd hui. SCREENING AND PREVENTING cervical cancer Cervical cancer starts with abnormal cell growth in the cervix, which is the lower part of the uterus. If detected early, cervical cancer is one of the most successfully treatable cancers. Prevention A lot can be done to reduce the risk of having cervical cancer. Most of the cervical cancers are caused by the Human Papilloma Virus (HPV). It is a common virus that spreads through sexual contact. Therefore, practicing safe sex by using condoms and also limiting the number of sexual partners can help reduce the risks of getting HPV and cervical cancer. Tobacco increases the risks of getting cervical cancer, so quitting smoking can make a difference. There are also two vaccines approved to protect against the HPV that cause most cervical cancer in women. They are recommended for girls and women from ages 10 through 25. The vaccine is administered as a series of three shots over 6 months. Screening Cervical cancer starts with pre-cancer changes called dysplasia. This can be detected by a medical test known as the Pap smear. As dysplasia is treatable, it is important for women to get regular Pap smears. It is recommended that women aged 21 to 65 have their Pap smears every 3 years. Women who have had a total hysterectomy (removal of the uterus and cervix) should stop screening, whereas those who have received HPV vaccine should continue. Screening and preventing cervical cancer is essential, so talk to your gynecologist today. 07

5 L implant cochléaire, un espoir contre la surdité profonde LE SAVIEZ- VOUS? Vos oreilles ne cessent jamais d entendre. Pendant votre sommeil, votre cerveau décide tout simplement d ignorer les sons! DID YOU KNOW? Your ears never stop hearing, even when you sleep. Your brain just ignores incoming sounds! HOW DO WE HEAR? The human ear has three anatomical parts: the outer ear, the middle ear and the inner ear. Sounds travel through the air as sound waves and thus reach our ears. The external part of the ear helps to direct the sound waves down the ear canal to the eardrum. This causes the eardrum (tympanic membrane) to move and vibrate. There are three small bones, called the malleus, incus and stapes (commonly known as the hammer, anvil and stirrup) that are anchored to the ear drum. The vibration of the eardrum also causes these small bones to vibrate. The cochlear is the main organ involved with processing of sound. It is lined with hairs and filled with fluid. Movement of the membrane (the oval window) of the cochlear by the stapes bone causes shifts in this fluid and movement of these hairs. It is this movement that results in sending signals to the hearing nerve. These signals transmitted by the nerves are the noise information that the brain can decode. COCHLEAR IMPLANTS, HOPE FOR THE COMPLETELY DEAF A cochlear implant is a small and complex electronic device that provides a sense of sound to people who are completely deaf or very hard of hearing. It is implanted surgically. Is it just another hearing aid? The cochlear implant is quite different from a hearing aid (an external device attached on the outer ear). The Cochlear impart comprises of two parts: an internal part that is placed surgically inside the inner ear and an external one acting as a microphone and which is attached magnetically to the internal part. Hearing aids provide amplified sound for the inner ear to process, whereas Cochlear implants deliver electrical current directly to the nerves of the inner ear. Who needs it? Both children and adults can seek cochlear implants, explains Dr Dinesh Murmu, consultant - ENT, Head & Neck Surgey at Apollo Bramwell Hospital. However, he adds that there are a number of criteria to decide who can undergo the implantation surgery. For example, the person should be completely or almost completely deaf in both ears, and experience almost no improvement with regular hearing aids. Moreover, children need to be enrolled in special programs that prepare them to speak and process sounds. Before the procedure, an ENT (Ears, Nose, Throat) doctor (Otolaryngologist) will examine the patient to see if Cochlear Implants are recommended. What happens after the implant? After the surgery, the person will be able to perceive sounds, listen to music and take part in conversations, even in a difficult listening environment. A Cochlear Implant Team, comprising of audiologists, speech therapists and ENT doctors, will help the patient to learn to hear and speak. This process, which involves activating sound perception using the Cochlear Implant and helping the patient to speak, may take several years for optimal results. l COMMENT ENTEND-ON? L oreille humaine comporte trois parties, l oreille externe, l oreille moyenne et l oreille interne. Les sons voyagent dans l air sous forme d ondes et atteignent nos oreilles. La partie externe de l oreille permet de diriger les ondes à travers le conduit auditif jusqu au tympan qui se met alors à vibrer. Trois petits os, appelés respectivement le marteau, l enclume et l étrier, sont ancrés sur le tympan, qui est situé dans l oreille moyenne. La vibration de la membrane du tympan entraîne celle de ces trois os. Dans l oreille interne, la cochlée est le principal organe impliqué dans le traitement du son. Elle est bordée de poils et remplie de fluide. Le mouvement de la membrane de la cochlée, occasionné par l os de l étrier, provoque des fluctuations du fluide et un mouvement des poils. Cela aboutit à l envoi de signaux au nerf auditif. Ces signaux sont des informations que le cerveau peut décoder en tant que «son». Un implant cochléaire est un petit dispositif électronique complexe qui sert à restaurer l audition de certaines personnes atteintes de surdité neurosensorielle sévère ou profonde. L appareil est implanté par une intervention chirurgicale. Une aide auditive comme les autres? Cet implant est différent des supports auditifs habituels. Il est composé de deux parties: une partie interne, qui est implantée chirurgicalement à l intérieur de l oreille, et une partie externe, qui fonctionne comme un microphone, se rattachant magnétiquement à la partie interne. Qui en a besoin? «L implant cochléaire peut être porté aussi bien par des enfants que par des adultes», explique le Dr Dinesh Murmu, consultant - ENT, Head & Neck Surgey à l Apollo Bramwell Hospital. La sélection des patients pour l implant se fait selon plusieurs critères. L implant cochléaire est destiné aux personnes présentant une surdité des deux oreilles et pour qui les aides auditives ne sont d aucune aide. Les enfants doivent, eux, avoir eu accès à des programmes d apprentissage du son et du langage. Avant un implant cochléaire, le patient doit être examiné par un spécialiste, l oto-rhinolaryngologiste (ORL). Ce dernier recommandera un implant cochléaire seulement si toutes les évaluations et bilans préopératoires sont jugés adéquats. Que se passe-t-il après l intervention? Après l implant, une équipe composée d audiologistes, d orthophonistes et d ORL aidera le patient pour qu il apprenne à entendre et à parler. Ce processus, qui implique une activation de la perception sonore grâce à l implant cochléaire et un apprentissage du langage, peut courir plusieurs années pour produire les meilleurs résultats. l 08 09

The BMI can help to determine if you are underweight, overweight, or just right. Calculate your BMI by doing the following operation: Divide your weight in kilograms by the square of your height in meters. Over 40 indicates that you are in a high-risk obesity. Research has indicated that lower BMIs (less than 18.5) and higher BMIs (25+) are associated with increased health risks. At Apollo Bramwell Hospital, we have recently acquired the Alcon Wavelight Refractive Suite, a state-ofthe-art equipment, to be able to offer a blade-free LASIK to our patients. What is the specific benefit of blade-free LASIK? Traditionally, the LASIK is performed by cutting the corneal flap with a blade before reshaping the cornea with an excimer laser. Our new equipment allows us to make a flap in the cornea using a Femtosecond laser instead. It s more precise and also safer. The procedure is quicker, the recovery time significantly shorter and the risks of complications much lower. For instance, with the blade-free LASIK, we can stop the procedure if unforeseen issues are uncovered during the operation. In that case, everything will be back to normal for the patient, who can even undergo a second attempt after a few months. In Mauritius, this technique is only available at our hospital. We have performed on 100 eyes so far and all our patients are doing well. Who should consider LASIK surgery? 20 to 30% of glass-wearing persons are eligible for LASIK. We have to assess their refractive stability, see if there are no diseases in their eyes and determine whether they have both good corneal thickness and a normal corneal topography. This is assessed through the Pentacam, a very advanced and precise diagnosic tool. LASIK is advised for patients who want to fix permanently their near-sightedness, farsightedness and astigmatism and who have issues with wearing glasses. 9 Treating Coronary Heart Disease: How the FFR technique helps Coronary heart disease, which affects millions of people worldwide, is caused by an accumulation of plaque in coronary arteries. This restricts blood flow and reduces the amount of oxygen available to the heart cells. To measure the severity of blood flow blockages in arteries, the technique of the Fractional Flow Reserve (FFR) can be very useful. A coronary angiogram is the traditional tool which is used to aid doctors in the treatment of coronary heart disease. FFR is an advanced tool which provides a more detailed, physiological analysis of blood flow blockages in the heart. An FFR measurement indicates the severity of blood flow blockages in the coronary arteries and allows physicians to identify which specific lesion(s) require stenting. The FFR involves placing a wire, fitted with a pressure sensor at the tip, into the affected artery. After administrating a drug (adenosine) to dilate and increase blood flow to the heart, the pressures in the normal part of the artery and in the affected part are compared. A ratio is then calculated and helps the doctor to decide between a number of treatment options: revascularization with stenting, open-heart surgery or optimal medical therapy alone. FFR thus not only helps to save health care costs, but also contributes to more appropriate patients care. l The FFR technique is now available at the Apollo Bramwell Hospital.
