IRC et IC Les données L insuffisance rénale est un marqueur indépendant des risques de mortalité et d hospitalisations chez les patients atteints d insuffisance cardiaque (IC), peu importe la fraction d éjection du ventricule gauche (FEVG). L insuffisance rénale chronique (IRC) peut être définie par le taux de filtration glomérulaire estimé (TFGe). Plus de 50% des patients IC ont une IRC au moins modérée, soit un TFGe 60 ml/min/1.73 m2.

Cardioversion for Rhythm Control We recommend electrical or pharmacologic cardioversion for restoration of sinus rhythm in patients with AF/AFL selected for rhythm control therapy who are unlikely to convert spontaneously. We recommend pre-treatment with antiarrhythmic drugs prior to electrical cardioversion in patients who have had AF recurrence post-cardioversion without antiarrhythmic drug pre-treatment. Recommendations Ablation We recommend catheter ablation of AF in patients who remain symptomatic following adequate trials of anti-arrhythmic drug therapy and in whom a rhythm control strategy remains desired. Strong Recommendation Moderate Quality Evidence We suggest catheter ablation to maintain sinus rhythm in select patients with symptomatic AF and mild-moderate structural heart disease who are refractory or intolerant to at least one antiarrhythmic medication. We suggest catheter ablation to maintain sinus rhythm as first-line therapy for relief of symptoms in highly selected patients with symptomatic, paroxysmal AF. Conditional Recommendation Moderate Quality Evidence Conditional Recommendation Low Quality Evidence Values and Preferences: These recommendations recognize that the balance of risk with ablation and benefit in symptom relief and improvement in quality of life must be individualized. Stroke Prevention in AF patients with CKD For antithrombotic therapy of CKD patients, therapy should relate to egfr as follows: egfr > 30 ml per minute: We recommend that such patients receive antithrombotic therapy according to their CHADS₂ score as detailed in recommendations for patients with normal renal function. egfr 15-30 ml per minute and not on dialysis: We suggest that such patients receive antithrombotic therapy according to their CHADS₂ score as for patients with normal renal function. The preferred agent for these patients is warfarin. Strong Recommendation Moderate Quality Evidence Conditional Recommendation Moderate Quality Evidence Values and Preferences: This recommendation places a relatively higher value on prevention of ischemic stroke than on bleeding complications associated with antithrombotic therapy, as well as the limited data available for new OACs in CKD patients. No therapy may be appropriate for some patients with egfr 15-30 ml per minute (not on dialysis), with a stronger preference for avoiding bleeding complications than preventing ischemic stroke.
Stroke Prevention in AF patients with CKD For antithrombotic therapy of CKD patients, therapy should relate to egfr as follows: egfr < 15 ml per minute (on dialysis): We suggest that such patients not routinely receive either OAC or ASA for stroke prevention in AF. Conditional Recommendation Low Quality Evidence Conditional Recommendation Low Quality Evidence Values and Preferences: This recommendation places a relatively higher weight on observational data linking warfarin and ASA use with mortality in patients on dialysis, and relatively lower weight on the potential for these agents to prevent ischemic stroke. Therapy with OACs or antiplatelet drugs may be appropriate for some patients with egfr < 15 ml per minute (on dialysis) in whom there is a stronger preference for avoiding ischemic stroke.

Pourquoi cette étude? Le degré d insuffisance rénale, les anomalies stucturelles, neurohormonales et inflammatoires associées à la FA; ainsi que l utilisation de digitale et d agents anti-arythmiques pourraient-ils mitiger les bénéfices de certains traitements chez ce groupe particulier de patients? Pourquoi? Hypothèses Des études parues en 2005 avaient démontré une augmentation de la mortalité par hyperkaliémie suite à l augmentation des prescriptions de spironolactone après la parution de l étude RALES. Toutefois, dans un sous-groupe de patients de notre étude (n-183) chez qui le potassium était disponible tout au long du suivi, on notait une augmentation de mortalité chez les patients HYPOkaliémiques. Ce lien entre l hypokaliémie et la mortalité accrue en IC a également été observé par le groupe de l étude DIG en 2011.
Pourquoi? Hypothèses Il est possible que l hypokaliémie soit sous-traitée chez les patients avec IRC (fréquent en présence d IC et de FA), surtout lorsqu un traitement par la spironolactone est en cours. Cette hypokaliémie pourrait contribuer à la hausse de mort arythmique observée.
Pourquoi? Hypothèses Des interactions complexes entre la FA, la fonction rénale, les multiples agents inhibant le RAAS, les antiarythmiques et les autres comorbidités (e.g. diabète) semblent exister. Notre étude est limitée par la non-randomisation des sujets de AF-CHF à un antagoniste du récepteur minéralocorticoïde versus l absence d ARM. Ceci ne serait pas éthiquement acceptable à l heure actuelle. Il est possible que l aldactone et l eplerenone ne soient pas entièrement comparables quant à leurs bénéfices cliniques chez certains groupes de patients atteints d IC (pas de mortalité accrue dans le sous groupe de patients avec FA dans l étude EMPHASIS).
