EPIDEMIOLOGIE Fréquence fibrillation auriculaire : 10% 80 ans 25% AVC ischémiques 80 ans liés à FA Fréquence VTE : 0,6% 80 ans Risque hémorragique sous AC : +40%/10 ans Analyse bénéfices/risques plus complexe chez sujet âgé Prévalence accrue de l insuffisance rénale : complique usage curatif de la majorité des anticoagulants.
LES ENJEUX Altérations cognitives : difficultés pratiques du traitement et sous-représentation dans études cliniques Augmentation co-morbidités et traitements Sous-utilisation des anticoagulants : redoutés par le médecin et le patient : 35% des patients FA 85 ans ont AC (vs 50-60%) Evaluation initiale et suivi fonction rénale: Cockroft (> MDRD, CKD) Cl Cr < 30 ml/min : 20-30% patients hospitalisés Cl Cr 30-50 ml/min : 60% patients ambulatoires
LES ENJEUX Rapport d hémorragies sous DOACs : 65% > 80 ans, 58% insuf. rénale, 50% < 60 kg. Saignements majeurs : x2 après 65 ans Hémorragies intracrâniennes: x2 après 80 ans Evaluation du risque hémorragique : FA : score HAS-BLED VTE : score validé (ORBI, Riete, Kearon...) aucun score ne donne 1 bonne prédiction du risque hémorragique à 90 jours scores existants peu pertinents chez sujet âgé
HBPM (1) Elimination rénale prépondérante Différences entre HBPM : élimination extra-rénale tinzaparine et daltéparine > enoxaparine et nadroparine Augmentation risque hémorragique en relation avec accumulation HBPM insuffisance rénale petit poids < 45 kg et interaction AINS, SSRI
HBPM (2) 2 attitudes pour traitement prolongé chez insuffisant rénal : monitoring anti-xa à interpréter pour chaque HBPM pour chaque mode administration 1 ou 2x/jour réduction empirique en cas insuffisance rénale peut conduire à des réductions excessives (anti-xa < 0,5 U/ml) et risque thrombotique
AVK (1) Sous-utilisation chez patients à risque : 53% utilisation chez patients âgés sans risque hémorragique surajouté Difficultés équilibre : dans la FA, efficacité anticoagulation disparaît si TTR < 65%. TTR habituel 50% Amélioration équilibre AVK : clinique AC, self-monitoring,... : peu utilisable chez sujet fragile
AVK (2) Risque hémorragies intracrâniennes : 85 ans : 2,5x par rapport à 70-74 ans INR 3,5-3,9 : 4,6x par rapport à INR 2-3; pas de différence entre < 2 et 2-3 Diminution dose entretien : +- 10%/décade : diminution poids, insuffisance cardiaque, maladie hépatique, insuffisance rénale sévère Interactions médicamenteuses : amiodarone, macrolides, azolés, SSRI, AINS
AVK (3) Optimiser AVK diminution dose induction génotypage : pas utile évaluation risque hémorragique éviter surdosage faible dose vit K (75-150 µg) augmenter la compliance

Conclusions DOACs (I) Dans le traitement de la FA : les sujets âgés sont représentés dans les études le profil d efficacité des DOACs par rapport aux AVK n est pas modifié chez sujet âgé le risque hémorragique est significativement accru chez le sujet âgé avec tous les anticoagulants le profil de sécurité des DOACs par rapport aux AVK est préservé chez le sujet âgé sauf dabigatran (absence de réduction posologique dans étude RE-LY?).
Conclusions DOACs (II) Dans le traitement de la FA : réduction des hémorragies intracrâniennes pour tous les DOACs, par rapport aux AVK augmentation hémorragies digestives pour les DOACs (dabigatran, rivaroxaban, edoxaban 60 mg), par rapport aux AVK
Conclusions DOACs (III) Dans le traitement de la TEV : les sujets très âgés sont peu représentés dans les études; âge et fragilité moindres que FA pas de réduction posologique sauf edoxaban le profil d efficacité et de sécurité est conservé chez le sujet âgé réduction des hémorragies graves sous rivaroxaban dans un sous-groupe de patients fragiles
Conclusions DOACs (III) Optimisation traitement anticoagulant : respecter contre-indications monitorer fonction rénale et adapter posologie prudence par rapport aux petits poids penser aux interactions médicamenteuses réduire traitements anti-plaquettaire superflus, AINS et autres (SSRI) monitoring DOACs des cas complexes (insuf. rénale +- interaction médicam +- petit poids)?
