Vulnerable Population Center, Lausanne Department of Ambulatory Care and Community Medicine (Policlinique Médicale Universitaire), Lausanne, Switzerland The Vulnerable Population Center (VPC, http://www.pmu-lausanne.ch/pmu-unite-populationsvulnerables) was created in 2002 at the Department of Ambulatory Care and Community Medicine (Policlinique Médicale Universitaire, PMU) at the University of Lausanne, Switzerland. It aims to provide clinical support, teaching (at the pregraduate, postgraduate, continuous level) and research, focusing on vulnerable groups with limited access to the health care system because of low income, limited education, racial / ethnic minority status, lack of insurance and / or basic resources required for health (e.g. food, housing, safety). Over the past 13 years, the VPC has made substantive contributions by improving the health of vulnerable patients (i.e. asylum seekers, undocumented immigrants, homeless, ED frequent users, patients with low health literacy, prisoners) and coordinating clinical activity with public health needs, taking into account the socio-economic and political context. The VPC is a key actor locally, connected with the Social Population Commission and the Public Health Office of the canton of Vaud. Nationally, the VPC is integrated within the Swiss Hospitals for Equity financed by the Swiss Federal Office of Public Health. At the international level, there are health services links with the Platform for International Cooperation on Undocumented Migrants (PICUM) and the Task Force on Migrant Friendly and Culturally Competence Healthcare. The VPC has academic connections with the Massachusetts General Hospital (Harvard University) Boston, UCSF San Francisco, the University of North Carolina Chapel Hill and the London School of Hygiene and Tropical Medicine. Four operational units provide services The Health and Migration Unit (USMi for Unité Santé et Migration), mainly for asylum seekers and refugees The Mobile Team for patients with multiple vulnerabilities (EmvS for Equipe Mobile Vulnérabilités) acting primarily at the emergency department with frequent users The HIV and STI Consultation (VIH - IST - A, consultation VISTA) Clinical care for persons with a history of incarceration Staff The VPC includes medical, nursing and administrative staff, all working with an interdisciplinary perspective: Medical team: 2 senior physicians and 3 chief residents Nursing team: 1 head, 2 senior nurses and 26 nurse practitioners Administrative team: 1 head, and 14 staff members Social Assistance team: 2 social workers Pharmacy team: 2 pharmacists 1

Teaching team At the pregraduate level the VPC provides 32 teaching periods annually. Topics include: migration and health, HIV patients in the community, health disparities, and health care approaches for prisoners, patients with low health literacy and deaf patients. At the postgraduate level the VPC provides 14 sessions per year focused on cross-cultural competencies: trans-cultural care and communication, social determinants of health, stereotypes, and the explanatory model of disease. Finally, at the continuous level, the VPC is responsible for teaching «Migration and Health» (created in 1995), participated in the creation of a Certificate of Advanced Studies on «Diversity and Health», and participates in five other Certificates of Advanced Studies. Research activities The current research priorities include: 1. The frequent users at the Emergency Department (efficacy and efficiency of case management intervention) 2. Forgoing health care for economic reasons and primary care implications 3. The impact of teaching clinical trans-cultural competencies 4. The impact of teaching health literacy tools 5. The presence of unconscious bias among primary care providers These projects are financed by several different grants (Swiss National Fondation, Académie Suisse des Sciences Médicales, fonds locaux DUMSC, Fond d Innovation Pédagogique ; grants from the Swiss Office of Public Health, the local service of Public Health, the Direction of the Lausanne University Hospital). 2
