CONCLUSION TRITON-TIMI 38 La thrombose de stent est rare, mais c est une complication gravissime avec une importante mortalité. La réduction de la thrombose de stent passe par une augmentation de la compliance et la durée de l association ASA + clopidogrel. Prasugrel est un agent plus rapide, avec une plus grande inhibition de l agrégation plaquettaire entraînant une meilleure réduction du risque de thrombose de stent. 19

PLATO : CONSEQUENCES THERAPEUTIQUES Si on traite 1000 Pts pour SCA pendant 12 mois par TICAGRELOR, on évite : 14 décès 11 IDM 6-8 thromboses de stent Sans augmentation des saignements nécessitant des transfusions En traitant 54 patients avec TICAGRELOR au lieu de clopidogrel pendant 1 an on prévient 1 DC CV, IDM ou AVC. 31
CONCLUSION : PLATO TICAGRELOR : alternative plus efficace que CLOPIDOGREL pour la prévention de la thrombose de stent et des décès en traitement aigu et à long terme des SCA 32
