10 Pr Domingo MARTIN Director of the Roth Williams Center for Spain and Portugal President of the Roth Williams International Society of Orthodontics Active member of the European Angle Society Diplomate of the European Society of Orthodontics Visiting professor at the International University of Cataluña Barcelona and the University Complutense of Madrid Multidisciplinary treatment : the future of orthodontics Introduction to the face treatment philosophy Extractions for function, esthetics and stability A different perspective of the relationship between orthodontics and TMD Research today almost totally disregards occlusion as an etiological factor in TMD, there is no doubt that this must be taken into account, however as the philosopher Karl Popper in his work «Knowledge and the Shaping of Reality», says «I regard scientific knowledge as the best and most important kind of knowledge we have, though I am far from regarding it as the only one», as a clinician I totally agree with Popper. After treating TMD patients for the last 25 years with a high level of success I do not understand how the literature can come to this conclusion and relegate occlusion as a minor factor in TMD. Maybe there is an answer, in a recent article Dr. Okeson stated the following regarding the literature and the studies relating TMD and occlusion, «the literature finds a minor relationship between occlusal factors and TMD. It should be noted, however, that these studies report on the static relationship of the teeth as well as the contact pattern of the teeth during various eccentric movements. This represents the traditional approach to evaluating occlusion. Perhaps these static relationships can provide only limited insight into the role of occlusion and TMD «. So when we read what Dr Okeson says we can now begin to think that maybe there is a relationship and if you stop and think about it without any bias and with an open mind, it is difficult to imagine a specialty that routinely and significantly changes a patient s occlusal condition and would not have a powerful affect on the masticatory structures and their functions. If we take this to other dental specialists, most prosthodontists would be greatly concerned with developing a permanent occlusal position with no regard to joint position and even Dr. Jeff Okeson makes the following statement «the concept that orthodontic treatment has nothing to do with TMD is like stating that moving teeth anywhere will not influence how the patient functions. Certainly that is not the case in prosthodontics!!! As orthodontists we must achieve a musculoskeletal stable position and avoid posterior interferences that may lead to TMD. We know from Pullinger and Seligman that there are two factors that will determine whether an intracapsular disorder will develop: the degree of orthopedic instability, and the amount of loading. Orthopedic instabilities with discrepancies of 1 or 2mm are not likely significant enough to create a problem. However, as the discrepancy between the musculoskeletally stable position of the condyles and the maximum intercuspation of the teeth becomes greater, the risk of overload exists and thus intracapsular disorders increases. Taking this into account we now have a basis for treating patients with class II s and when approached at a young age we can not only correct the class II but also seat the condyle in the fossa and eliminate any possible fulcrums. This will help us achieve a musculoskeletal stable position and obtain orthopedic stability, thus reducing possible TMD factors. 10

Nous sommes de plus en plus amenés à demander des examens radiologiques pour nos plans de traitement implantaire, plus par souci de nous prémunir des surprises que dans le but de conforter notre diagnostic basé sur les connaissances des structures osseuses que nous devons traiter. Ce que nous fait voir un examen radiographique est-il toujours le reflet de l anatomie du patient? Le but de cette présentation est celui de présenter la corrélation, ou l absence de corrélation, entre radiologie et anatomie. Risques vasculaires en implantologie orale - Comment éviter les pièges? Les risques anatomiques dans les procédures chirurgicales implantaires concernent le plus souvent les nerfs et les artères. Bien que majoritairement dépourvues de risques graves, certaines lésions vasculaires peuvent, néanmoins, mettre en jeu le pronostic vital des patients. Ces lésions sont souvent liées à une méconnaissance des variétés anatomiques du réseau artériel maxillo-facial. L objet de cette conférence est de faire la lumière sur les accidents vasculaires potentiels et comment les éviter. 11

In recent decades the horizontal type preparations (shoulder and/or chamfer), have clearly gained popularity for a series of hypothetical benefits. This type of design apparently should be easier to be clinically performed, more readily identifiable both on the tooth by the clinician and on the stone die by the technician, and should also ensure a quantity of ceramic in the cervical areas thick enough to withstand the occlusal loads. However, it was recently, that the same objectives can be achieved through the application of an original clinical protocol called «Simplified Prosthetic Protocol» (SPP) which includes vertical preparations rather than horizontal. This protocol has been successfully applied to healthy natural teeth and it has proven to give excellent clinical results in terms of: The SPPÒ (Simplified Prosthetic Protocol), has been used for the past 10 years and the accurate application of the 6 key-points showed in the following presentation have constantly guaranteed predictable clinical results. The simplified prosthetic protocol: a novel approach for the rehabilitation of natural teeth and implants The location and design of the finishing line preparation for fixed prosthetic rehabilitation, has always been a very controversial topic. What kind of preparation, among those with a horizontal or a vertical component, is the most suitable to ensure ease of clinical performance, stability and good quality of the surrounding soft tissue in the medium and long-term has also been a major debated topic. It was recently demonstrated that the same objectives can be achieved through the application of an original clinical protocol called «Simplified Prosthetic Protocol» (SPP) which includes vertical preparations rather than horizontal. This protocol has been successfully applied to healthy natural teeth and implants and it has proven to give excellent clinical results in terms of: 1- tissue stability both in the short and in the long-term around the restorative margins 2- ease of application; 3- efficacy from a practical standpoint since all the restorative procedures are carried out in much less time compared to the traditional prosthetic approach. The 6 key-elements making this protocol successful and worth to be used will be presented in the following oral presentation. 12

An important question is to what extent natural teeth can be integrated into an implant-based prosthetic restoration. Our teeth-evaluation protocol is presented. Possible prosthetic solutions based on natural and implant pillars for supporting fixed or removable dentures are discussed. Furthermore, specific clinical procedures are mentioned. The edentulous upper jaw: restoration concepts and indications the edentulous upper jaw remains a big task for the restorative dentist especially since dental implants are added to the therapeutically portfolio. But how to do it best? Should the normal conventional tissue-borne removable denture be replaced by a fixed bridge in every case? Does an implant-borne removable denture lead to lack of comfort compared with a PFM bridge? What is the best solution concerning esthetics, phonetics, function, longevity, but also for the dentist in his efforts for the maintenance of the prosthetic superstructure during the service period? These and other questions about technical items, such as bar profiles, metal, veneering, etc. will be discussed on the basis of different clinical cases. The experiences in our clinic with different means of implantbased prosthetic superstructures will be presented. 13

14 Dr Andrea PARPAIOLA Degree in Medicine and Surgery Padova University Postgraduated Odontology - Padova University Teacher, tutor in Master Program of Implantology and Visiting Professor at Padova University Single tooth replacement: clinical and biological aspects The clinical replacement of missing teeth by osseointegrated implants has represented a significant advance in dentistry, and numerous studies have documented optimal results. The substitution of a single tooth with an implant is a challenging procedure, in particular in the upper anterior segment. Prior to selecting an implant-borne solution, the clinician should review all the possible treatment modalities available with advantages and disadvantages and the periodontal endodontic conditions. In this context it is important to evaluate periodontal patient biotype for the esthetic outcome, and timing of implant placement. Careful selection of immediate or delayed installation and loading will optimize the treatment. During the restorative phase it is crucial to respect the soft tissue biology to allow a perfect healing and maintenance of the esthetic outcome. The soft tissue barrier at implant and teeth The gingiva at teeth and the mucosa at implants made of titanium have some characteristics in common, but differ in the composition of the connective tissue, the alignment of the collagen fibres which run a course parallel with the implant surface. It is then possible to move this tissue in lateral direction creating more appropriate and natural emergency profile for the outcome of the definitive restoration.the goal of the therapy is indeed to mimic as close as possible natural dentition harmony. The initial evaluation is important of the tissue biotype, in order not to jeopardize the treatment from esthetic view point. Temporization is used in this context to sculpture the soft tissue, because the natural shape of the teeth is not rounded and determined by a temporary healing abutment. It is critical to transfer the new profile to the lab with accurate definitive impression to optimize the restorative process, using a customized implant pick up at chair side. Abutment material selection is also important since soft tissue healing may differ facing titanium, gold- alloy or zirconium, as demonstrated in experimental studies. 14

Au cours de cette conférence, nous aborderons 10 points importants : 1- Beaucoup de femmes ne signalent pas spontanément leur grossesse en consultant un chirurgien-dentiste. La grossesse n est pas une maladie mais elle est susceptible de modifier notre prise en charge, tout comme la période d allaitement. 2- La femme enceinte est généralement sensible à toute information sa santé et surtout celle de l enfant à naitre. Cette attitude est donc favorable à la découverte ou à la consolidation d acquis en matière éducation sanitaire et de prévention bucco-dentaire. 3- Les praticiens doivent être conscient de leur rôle et se former si nécessaire à informer sans dramatiser, écouter la future maman et assurer le suivi, non seulement en ce qui la concerne mais aussi en matière de consultation de jeunes enfants, voire du nourrisson. 4- Cette information doit porter sur l hygiène bucco-dentaire en général et les moyens de la conduire : cette évidence pour le praticien n est pas forcément partagée, et la question du brossage chez le tout petit mérite d être développée. 5- Si le brossage reste à la base d une hygiène dentaire bien conduite, il peut être complété utilement par d autre moyens, voire être avantageusement remplacé par un bain de bouche bicarbonaté en cas de reflux gastrique ou de vomissements fréquents. 6- «Un enfant, une dent» est une idée fausse qu il faut combattre : il n y a pas de fatalité mais plutôt des comportements alimentaires inadaptés qu il faut amender. La prévention des redoutables «syndrome du biberon» s inscrit dans la diffusion de quelques notions de nutrition bien comprises. 7- Chronologie dentaire et éruption, dans la banalité ou l exceptionnel (dents néonatales) : au-delà des petites recettes, il s agit pour le praticien de se positionner clairement en professionnel de référence, pour ce qui relève de sa compétence, la bouche et les dents 8- Faire le point sur la supplémentation en fluorures en fonction des récentes recommandations de la Haute Autorité en Santé. 9- Il convient également de ne pas négliger les possibilités de contamination dans la vie quotidienne, qu elle soit d origine virale (HSV) ou bactérienne si une personne de l entourage proche présente un risque carieux élevé. 10- Comment organiser cette rencontre pour une consultation réussie, dont le bénéfice est certain pour les intéressé(e)s, profitable pour l image du praticien et de la profession, c est à dire pour l ensemble de la communauté. 15

16 Optimiser ses techniques et ses stratégies pour une meilleure prise en charge de la douleur chez l enfant Une bonne prise en charge de la douleur chez l enfant est capitale pour permettre de créer des liens de confiance, indispensables pour prodiguer des soins de qualité. La douleur n est pas une fatalité dans les soins dentaires pourtant leur perception négative conduit le patient à éviter ou à s opposer aux soins. Cette attitude préjudiciable est souvent le résultat d expériences précédentes. Les praticiens doivent se préparer à recevoir ces patients et se former aux techniques de communication et de gestion des comportements. Cela sera bien sûr profitable au patient mais pourra également les prévenir du burn-out ou épuisement professionnel. Si Les progrès de la technique et de la pharmacie ne peuvent occulter la dimension relationnelle du soin, il n en reste pas moins que le praticien doit également posséder les connaissances nécessaires à une bonne compréhension de la physiologie de la douleur, et maîtriser l exécution des protocoles opératoires. Pour traiter cette thématique, nous aborderons des questions comme : - Physiologie pratique : que faut-il savoir exactement des mécanismes de la douleur pour prendre les bonnes décisions dans son cabinet? - Evaluer la douleur : pourquoi et comment évaluer la douleur? - Peur et douleur : une confusion au cœur du problème des soins ambulatoires - Anesthésier sans douleur : les petits «trucs» et les bons protocoles - Antalgiques chez l enfant : avant ou après nos traitements, quels médicaments utiliser? - Méthodes de sédation : il y a toujours une solution, encore faut-il savoir poser les indications, savoir pratiquer et parfois savoir déléguer 10 actes cliniques au quotidien en odontologie pédiatrique Lorsque l enfant paraît au cabinet dentaire, le sourire du praticien disparaît parfois, mais pas toujours. The periodontally compromised posterior prosthetic abutment: When to maintain the tooth v/s placing an implant One of the challenging decisions that we often make is to determine if we can trust the periodontally compromised natural element as a prosthetic abutment, or if it would be beneficial to extract and substitute with a dental implant. This analysis is even more complicated when we are looking at a posterior tooth that presents with peculiar anatomical features and complexities. The analysis of this clinical dilemma, and the conclusions that can be drawn, may profoundly affect our treatment planning. Many aspects of the overall management of these situations will be discussed, both by reviewing precious information coming from the literature and by outlining a multidisciplinary working method emphasizing the diagnostic and prognostic phase of treatment. Defining predictability in smile enhancement / Preservation: the periodontist s responsibility Today s patients are increasingly demanding when it comes to their smile s esthetic appearance and probably they should! In an age where physical beauty surely plays its role, our contribution in preserving or improving our patient s smiles has become increasingly significant. We now have a multitude of new materials and techniques that can assist us in the often challenging task of working in the esthetic zone, where every slip can quickly turn into a clamorous fall. Our primary objectives are to properly diagnose the oral abnormalities that we are confronted with, communicate to the patient our prognostic forecast with special emphasis on the possible limitations that might affect treatment, and ultimately to restore health and function with a pleasant esthetic outcome. When the goal of therapy is purely cosmetic in its finality our responsibility is particularly relevant because every surgical procedure has a certain margin of unpredictability during the healing process. Cet examen complémentaire, réalisé dans le cadre d un traitement par voie orthograde ou chirurgicale, a considérablement évolué ces dernières années. Il est réalisé lors des différentes étapes opératoires (pré, per et postopératoire) mais également pour le suivi des pathologies d origine endodontique (ex. suivi de cicatrisation de lésions périradiculaires). Si l examen de routine et de première intention, la radiographie intraorale est systématiquement effectuée par le clinicien, d autres types d examens, extra-oraux telles que l orthopantomographie ou encore la scanographie linéaire (tomodensitométrie) ou volumique (tomographie à faisceau conique ou CBCT en anglais) peuvent être parfois réalisés de manière complémentaire. Ces examens de 2 intention permettent d évaluer par exemple l extension des lésions, les rapports anatomiques, la qualité d obturation endodontique, etc. Il est à signaler que de nombreux travaux de recherche s intéressent également au domaine des images en endodontie. L objectif de cette présentation sera d exposer les différentes possibilités d imagerie endodontique en 2012 et celles du futur. Nous aborderons également la recherche biomédicale réalisée dans ce domaine. 19

Un implant près d une dent est actuellement assez bien décrit. Pour plusieurs dents, l utilisation de pontiques pour éviter deux implants adjacents est aussi une pratique courante. L obligation de poser deux implants adjacents pour remplacer deux centrales se pose aussi parfois. Deux cas cliniques vont servir à montrer certaines astuces cliniques, certaines étapes, certains actes, qui peuvent préparer les sites pour recevoir les implants dans des conditions qui servent alors à avoir des résultats esthétiques optimaux. 20
