Radiofrequency ablation Tumor complete necrosis 60% for tumor < 3cm but only 15-29% for larger tumor (3-5 cm) Pompili M et al. Liver Transpl 2005 Progressive increase of viable tumor during waiting time Mazzaferro V et al. Ann Surg 2004
Transarterial chemoembolization: lobar vs selective Independent predictor of complete tumor necrosis are: Selective/supra-selective TACE Single nodule Best response for tumors btw 3-5 cm Poor correlation btw Lipiodol uptake and histological necrosis (56%) Dharancy S et al. Liver Transpl 2007 Golfieri R et al. Hepatology 2011
TACE as a tool for selection of candidates Tumor response to TACE could be a surrogate marker of less aggressive tumor Otto G. et al J Hepatol 2013
TACE to improve recurrence rate after liver transplantation No clear OS benefit But decrease of drop-out rate Safe Fewer tumor recurrence in the TACE group (13 vs 23, ns) More non-tumor related deaths (15 vs 7, ns) Decaens T et al. Liver Transpl 2005 Recommended if the expected waiting time between 4-9 months (Aloia T et al. JGastrointestSurg 2007)
Transarterial radioembolization (TARE) 5% to 10% of bridging therapy in the OPNT registry Scanty data based on very small number of pts The down-staging seems to be more effective (58%) compared to TACE (31%) Lewandowski RJ et al. AmJTranspl 2009 But RFA in addition to TARE in 42% compared to 23% in TACE group. Post-operative complications? Same recurrence rate (22% vs 18%)
