About This Issue Welcome to the Spring issue of the Journal. There are a lot of activities being planned for the coming months, not the least of which is the CSRT Annual Education Forum and Trade show in Montreal. You can see a preliminary copy of the programme in this issue. Other Forum activities that are being finalized include the Airway Olympics and a fun night at the Montreal Inter national Doug Maynard Beer Festival. For past two years we have added a separate day of educational sessions for those interested in topics related to the education of respiratory therapy students. This year we have also included a Leadership Congress with content that will be of relevance to those interested in management and other leadership issues. Also in this issue you will find information on activities with Health Canada related to health human resources; an update on national activities related to changing respiratory therapy to a bachelor s degree and much more. Finally, we are also pleased to announce that the CSRT Online Discussion Forums are back up and running. Check this issue for details or go to the website under Discussion to subscribe. We hope to see you all in Montreal for the Annual Education Forum and Tradeshow. Be sure to register early. Space is limited and we expect a large turnout this year! 4 On Air À l affiche TB Vets Foundation Awards RT Bursaries Eight students from Thompson Rivers University (TRU), British Columbia, have each been awarded a $1500 bursary from TB Vets Charitable Foundation. The bursaries were awarded to students with a minimum grade point average of 2.5 with preference given to students who demonstrated financial need. The students awarded the bursaries were: Osman Azad of Surrey, Kiley Cindrich of Langley, Ranjni Kainth of Burnaby, Tamara McKenzie of Nanaimo, May My Minh of Vancouver, Kamaldip Sandhu of Kamloops, Christa Sloan of Westbank, and Kimberly Wilcox of Kamloops. Les bourses ont été octroyées aux étudiants qui détenaient une moyenne pondérée cumulative d au moins 2,5 et la préférence était accordée aux étudiants ayant démontré un besoin financier. It is the only educational institution in B.C. to offer this program and we are pleased to be able to provide assistance to tomorrow s respiratory therapists, said Ron Goyette, Executive Director of the TB Vets Foundation. The Foundation receives no government funding. Donations to their annual Keytag Campaign and other initiatives allow TB Vets to help children and adults across B.C. with respiratory ailments. 5 On Air À l affiche New Global Initiative for Asthma Publications The new 2006 Global Initiative for Asthma (GINA) Guidelines are the first global guidelines to define the new approach of asthma symptom control rather than therapy geared to arbitrary levels of disease severity. The 2006 GINA Updated Guidelines The GINA Report 2006 is now available along with the new GINA Pocket guide as well as a GINA At-a-Glance Reference. One of GINA's most important activities is to work with international experts to produce guidelines, reports, and other resources about asthma. These materials incorporate the latest science to help improve asthma management and the lives of asthma patients around the world. GINA works with health care professionals and public health officials around the world to reduce asthma prevalence, morbidity, and mortality. There are several prominent Canadians on the Executive and Science Committees of GINA. Through resources such as evidence-based guidelines for asthma management, and events such as the annual celebration of World Asthma Day, GINA is working to improve the lives of people with asthma in every corner of the globe. Les directives GINA de 2006 mises à jour Le Rapport GINA de 2006 est maintenant disponible, de même qu un nouveau guide pratique (GINA Pocket Guide) et une carte de référence (GINA At-a-Glance Reference). 7 President s message mot du président Message from the President As I sit here writing this message, I am shocked to realize that this will be one of my last formal communication tasks before my year ends as CSRT President. I am amazed at how fast it is going by. Here we are with the Spring edition of the Journal and yet last year s Forum events in Saint John are still so clearly pictured in my mind. Montreal holds special meaning to me. When my family and I came to Canada, we settled in Montreal. I lived on one of two small streets, nestled between the two major highways, railways and the airport in Dorval, on the West side of the island. It was a great place to grow up. Riding the street car into Montreal and watching the original Montreal Canadiens win the Stanley Cup at the old Forum is a memory I will never forget. During EXPO 67 all of my high school friends agreed that it was the best city in the world. Montreal also is special to me in that I have come to learn just how important it was to the origins of our profession. Montreal was the basis for my earliest connections to respiratory therapy. Let me elaborate. I had some minor surgery at the Royal Victoria Hospital and I also attended university at Sir George Williams, now Concordia University. Why do I tell you all of this? Well, it will demonstrate just how small a world we live in. When I was eighteen I had an eardrum repaired. This required a short stay at the Victoria Hospital. I had to have a number of tests and as we all know, labs of any kind are never nearby; you have to travel through long dark corridors and through tunnels to reach your destination. Although I do not recall the exact spot, I do remember passing somewhere a room with the words Inhalation Therapy on it. I thought that was interesting but never gave it a second thought. Years later, when I first sat on the Board of the CSRT, I learned from Mike Andrews and Jim Coward that many of the early leaders of our profession got their start there. It was this group at the Old Vic and a similar group at the Toronto General Hospital that became the first Who s Who of Respiratory Therapy in this country. During my student days at Sir George Williams University, I was surprised one day to arrive at the campus to witness a group of students throwing stacks of computer printouts and program cards out the seventh floor windows. It seemed that several of the students took exception to an event and decided to occupy the Computer Labs. Protests of this nature were not unheard of in the late years of the 1960 s. After all, this was the period of Vietnam war protests. Years later, when I was the Manager at the Peel Memorial Hospital in Brampton, I hired one of those students; he had become a respiratory therapist. It was really interesting to hear the story of those events from the other side. So you never know what may happen. The Forum in Montreal will be an exceptional one for me and I hope that you will also find it special. I would like to express my sincere appreciation to the many people from L OPIQ and the organizing committees who have assisted us in putting this event together. Montreal will be among the largest Forums that we have ever had. We have a truly incredible lineup of speakers with some of the hottest topics in respiratory care today. I am especially pleased to point out that morning sessions will be available through simultaneous translation. Many afternoon sessions will be in French. This is particularly Rob Leathley important to us in order to reach out to the many Francophone therapists in the country. So make your way to Montreal this year and enjoy the many great things that it has to offer. If you can t find me around the many meeting rooms, look for me at Swartz s having a real Montreal Smoked Meat sandwich; with an extra pickle of course. Finally, I d like to add just how much of a pleasure it has been to be President. Many people consider it to be too big a job and say they would rather have a root canal done than volunteer for this position. In many ways I can see their point. The thing is that during this past year I have had an incredible time. I have met some of the greatest people you could ever meet. I have spoken to the leaders of our profession and I have had a chance to talk to others about respiratory care throughout the world. I work with a staff at the Head Office who put up with my many shortcomings and make me look good, as well as a group of the best therapists around who sit on the Board of Directors. It can be tiring at times but it is also something that is so rewarding an experience that you may never understand it unless you try it for yourself. To that end, the CSRT needs you to become involved. We need people to serve on committees and to become members. Please consider this when you send in your membership renewal form and let us know that you are interested. You will be amazed at how much fun you can have. 8 President s message mot du président Mot du président En rédigeant ce texte, je suis surpris de constater qu il s agit d une de mes dernières communications formelles avant la fin de mon mandat à la présidence de la SCTR. Comme le temps file! Bien que nous en soyons rendus au numéro du printemps de la RCTR, les souvenirs du Forum de l an dernier me paraissent encore très récents. La ville de Montréal revêt une signification particulière pour moi. Lorsque ma famille est déménagée au Canada, c est à Montréal que nous avons élu domicile. Notre maison était située sur une petite rue entre les deux autoroutes principales, les chemins de fer et l aéroport de Dorval, du côté ouest de l île un endroit magnifique où grandir. Je n oublierai jamais l expérience de me rendre en ville en tramway jusqu à l ancien Forum pour voir les Canadiens de Montréal gagner la Coupe Stanley. Pendant l Exposition de 1967, tous mes amis du secondaire s entendaient pour dire qu il s agissait de la meilleure ville au monde. Montréal m est spéciale pour une autre raison : je connais son importance vis-à-vis des origines de notre profession. C est là que j ai tissé mes premiers liens en thérapie respiratoire. Je m explique J ai subi une chirurgie mineure à l Hôpital Royal Victoria et j ai fréquenté l Université Sir George Williams, maintenant l Université Concordia. Pourquoi vous raconter ces détails? Ils me permettront de vous démontrer à quel point le monde est petit. À l âge de 18 ans, la réparation d un tympan a exigé un bref séjour à l Hôpital Royal Victoria. Les nombreuses investigations de laboratoire qui s avéraient nécessaires m ont obligé à me promener dans des tunnels et de longs corridors sombres afin de me rendre à destination, les laboratoires n étant jamais à proximité. Je me souviens d avoir passé devant une porte quelque part, où les mots «Inhalation Therapy» (Inhalo - thérapie) étaient inscrits. Je trouvais cela fort intéressant mais je n y ai pas repensé par après. Plusieurs années plus tard, alors que je siégeais sur le Conseil d administration de la SCTR, Mike Andrews et Jim Coward m ont informé que c est précisément à cet endroit que bon nombre des premiers leaders de notre profession ont débuté leur carrière. Ce groupe à la «Old Vic» et un groupe semblable à l Hôpital Général de Toronto sont devenus les premières fines fleurs de la thérapie respiratoire au Canada. Pendant mes études à l Université Sir George Williams, je suis arrivé un jour sur le campus pour voir un groupe d étudiants lancer des quantités d imprimés d ordinateur et de cartes-programmes par la fenêtre du sixième étage. De toute évidence, soulevant une objection à une activité quelconque, ils avaient décidé d occuper les laboratoires d informatique. Ce genre de protestation n avait rien de surprenant à la fin des années 1960, période de protestations contre la guerre de Vietnam. Quelques années plus tard, en qualité de gestionnaire à l Hôpital Peel Memorial de Brampton, en Ontario, j ai embauché un de ces étudiants qui était devenu thérapeute respiratoire. Il me fut très intéressant d entendre sa version de ces événements. Ainsi, nul ne sait jamais quand une personne croisera de nouveau son chemin. Le Forum à Montréal me sera donc bien spécial et j espère que vous en conviendrez avec moi. J aimerais exprimer ma sincère reconnaissance aux nombreuses personnes de l OPIQ et des comités organisateurs qui nous ont aidé à planifier cette activité. Il s agira d un des plus importants Forums de la SCTR. Une brochette impressionnante de conférenciers traiteront des sujets de dernière heure liés aux soins respiratoires. En outre, j ai le plaisir d annoncer que l interprétation simultanée sera disponible lors des sessions matinales. Plusieurs sessions prévues en après-midi seront offertes en français. Cela revêt une grande importance pour nous, alors que nous nous employons à joindre les nombreux thérapeutes francophones au pays. Donc, prévoyez vous rendre à Montréal cette année et jouir de ses nombreux attraits. Si vous ne réussissez pas à me trouver dans l une ou l autre des salles de réunion, venez me voir chez Swartz où je serai en train de savourer un véritable sandwich de viande fumée à la montréalaise, avec des cornichons, bien sûr. Enfin, j aimerais vous dire à quel point j ai eu du plaisir à titre de président. Bon nombre de gens sont d avis qu il s agit d une tâche très lourde, avouant qu ils préféreraient subir un traitement de canal avant d assumer ce poste. À plusieurs égards, je vois ce qu ils veulent dire. Par contre, je me suis bien amusé au cours de cette dernière année. J ai rencontré des personnes exceptionnelles, j ai parlé aux leaders de notre profession et j ai eu l occasion de discuter de la thérapie respiratoire à l échelle mondiale. Je travaille avec un personnel au siège social qui tolère mes faiblesses et qui fait en sorte que je parais bien, de même qu avec un groupe des meilleurs thérapeutes respiratoires qui soient au Conseil d administration. Bien qu elle puisse s avérer essoufflante par moment, il faut passer par la présidence afin de vraiment comprendre à quel point elle est enrichissante. À cette fin, la SCTR a besoin de vous. Nous sommes à la recherche de personnes qui veulent siéger à des comités et qui veulent devenir membres. Je vous invite à bien y penser au moment d acheminer votre formulaire de renouvellement d adhésion. Communiqueznous votre intérêt. Vous serez surpris du plaisir que vous en retirerez. The principles of evidence-based medicine provide the tools to incorporate the best evidence into everyday practice. Evidence-based medicine is the integration of individual clinical expertise with the best available research evidence from systematic research and the patient s values and expectations. A hierarchy of evidence can be used to assess the strength of evidence upon which clinical decisions are made, with randomized studies at the top of the hierarchy. Although high-level evidence is becoming increasingly available for some respiratory care practices, lower levels of evidence must often be used. There is always evidence, although sometimes it is no better than the experience of caring for similarly presenting patients in the past. Moreover, the evidence continues to evolve and better therapies should be adopted when they become available. Anecdotal evidence, however, should be abandoned if higher levels of evidence become available. Craig Campbell This lecture will explore the new Biphasic NCPAP modalities available on the Infant Flow SiPAP System with insight from experience at a tertiary perinatal referral centre. Sponsored by Maquet-Dynamed L assistance ventilatoire par ajustement neural (NAVA) est un mode de ventilation mécanique délivrant une pression en réponse à la stimulation neurale diaphragmatique du patient (mesurée par l activité électrique du diaphragme (EAdi). En conséquence, le NAVA agit tel une «pompe musculaire respiratoire externe» contrôlée par la demande inhérente des mécanismes respiratoires du patient. Le NAVA maintient la synchronie de la ventilation et s adapte aux fluctuations de la demande. Les tests chez les animaux et patients indiquent que le NAVA délivre des volumes courants et pressions moyennes dans des valeurs considérées protectives. En supplément le NAVA maintient la synchronie même en présence de fuites, lesquelles représentent une des limitations majeures en ventilation non-invasive par pression positive. En plus de l usage de l EAdi pour le contrôle du ventilateur, le monitorage de l EAdi fournit l information concernant la stimulation neurale respiratoire et de son rythme propre ainsi qu une réponse de ces paramètres à une variété de traitements et interventions. En résumé, le NAVA améliore le monitorage du stimuli respiratoire, permet une assistance ventilatoire en synchronie avec les efforts du patient et le contrôle de l assistance n est pas affectée par les fuites. MODULE E Location LACHINE en français La Ventilation Assistée à Domicile et les Aides Techniques Brigitte Fillion Le Programme national d assistance ventilatoire à domicile (PNAVD) est un programme ultra-specialisé qui est responsible de fournir des soins respiratoires à une clientèle ventilo-assistée à domicile. Couvrant l ouest de la province pour la clientèle adulte et toute la province pour la clientèle pédiatrique et blessé médullaire à dépendance ventilatoire, le Pnavd assure un ensemble de services pour maintenir à domicile cette clientèle. L accessibilité au programme et l ensemble de services offerts seront présentés. En conformité avec le cadre de référence pour l assistance ventilatoire à domicile du Ministère de la Santé et des Services Sociaux, le Programme d aide technique du PNAVD assure la gestion centralisée d un parc d équipement et de fournitures médicales pour la clientèle du PNAVD. Le mandat et les objectifs sont précisés dans la présente conférence. Les différents types d équipements utilisées seront décrits selon les modalités thérapeutiques espérées auprès de la clientèle et du type de pathologies à traiter. BREAKOUT SESSION FOUR 4:00 5:00 pm MODULE A Location MONT-ROYAL Advances in Mechanical Ventilation: Primum non nocere (First Do No Harm) Dean Hess, PhD, RRT, Associate Professor of Anesthesia, Harvard Medical School; Assistant Director of Respiratory Care, Massachusetts General Hospital; Boston, MA, USA. In recent years, a number of advances have occurred in mechanical ventilation. It has become increasing appreciated that mechanical ventilation, a life support treatment, can contribute to excess mortality if it is not applied properly. Moreover, mechanical ventilation strategies that improve gas exchange might not affect mortality. Lung protective ventilation strategies are becoming increasingly accepted. Noninvasive positive pressure ventilation has been shown to have a survival benefit in appropriately selected patients. Systematic reviews and meta-analyses have shown that patients with an exacerbation of chronic obstructive pulmonary disease and patients with acute cardiogenic pulmonary edema are most likely to benefit. Ventilator-associated pneumonia is usually related to the artificial airway rather than the ventilator per se. Strategies to reduce aspiration of upper airway secretions from around the cuff of the endotracheal tube are important to prevent ventilator-associated pneumonia. Ventilator-induced lung injury is known to occur with alveolar over-distension and repetitive opening and closing of alveoli. Over-distension injury is prevented by volume and pressure limitation (i.e., targeting a tidal volume of 6 ml/kg and plateau pressure < 30 cm H 2 O in patients with acute lung injury). Appropriate setting of PEEP is also important, although the role of strategies for precise setting of PEEP is unclear at present. Techniques to improve patient-ventilator synchrony are important for patient comfort. Available evidence suggests that spontaneous breathing trials are the best strategy to determine when mechanical ventilation can be discontinued. Although a variety of new ventilator modes has become available, evidence is lacking that any new ventilator mode improves important patient outcomes such as survival. Professor Ola Stenqvist, MD, PhD Consultant anaesthesiologist and professor in Sahlgrenska University Hospital, Gotheburg, Sweden Sponsored by GE Healthcare The gap between research methods for lung mechanics measurements and what are clinically feasible methods for monitoring is very wide. Lung compliance is volume, but also time dependent. Thus, the increase in FRC when increasing the end expiratory pressure is more than twice the volume expected calculated from quasi static compliance during tidal breathing. This indicates that it is important to monitor lung mechanics during therapeutic tidal breathing rather than during staic conditions, which are not representative for the behaviour of the lung during normal ventilator treatment. We have developed an algorithm for calculating the alveolar pressure breath by breath from a direct intratracheal pressure measurement with a prerequisite of near equal inspiratory and expiratory airway resistance. This algorithm makes it possible to display an alveolar pressure/volume curve during on-going ventilation. This Dynostatic pressure/volume curve reveals that compliance is volume dependent, which means that in most ALI/ARDS patients the initial compliance, at a very low volume, is more than twice as high as the final compliance of the tidal volume. Thus, compliance is decreasing throughout the breath. Even if a lower inflection point is seen at a level ~10 cm H 2 O during semistatic conditions, such as a slow inflation procedure, this lower inflection point is rarely seen when the end-expiratory pressure is above 5 cm H 2 O during normal tidal ventilation. This is explained by the fact that the time during which the pressure is below 10 cm H 2 O is short, in contrast to the situation during a slow inflation where there is sufficient time for the lung to collapse. Successful recruitment is indicated by an increase in oxygenation, compliance and a decrease in carbon dioxide tension. Another important feature is an increase in FRC. We have developed a modified nitrogen washin/out method, where the inspiratory O 2 only needs to be changed by 10 % to make a reliable measurement and this can be performed even at 100% inspiratory oxygen, as a lowering of inspiratory oxygen by 10% for some few minutes does not affect oxygen delivery in a critical way. These life-saving, practical strategies are difficult to teach using traditional didactic methods and hence are often neglected. Medical Simulation offers a way to practice this essential skill set, also known as Crisis Resource Management,1 and does so without risk to patients. In the interests of patient safety, continuing competence, error reduction, and even social justice, simulation is increasingly being seen as a key priority. However significant challenges exist for anyone eager to promote simulation. This discussion will cover an array of simulation strategies (both high-tech and low-tech), an array of simulation devices (medical, surgical and other) and will outline numerous practical lessons learnt during five years of expanding a simulation program. In other words, the hope is to provide background on what simulation can offer; ammunition to kickstart a simulation program; and insights that might make it flourish. Il n existe pas de consensus sur les modalités optimales de réglage des paramètres ventilatoire en HFO. Sponsored by the Canadian Intensive Care Foundation Severe sepsis: defined by inflammation (aberrant temperature and white blood cell count, tachypnea and tachycardia); suspicion of infection; and evolving organ dysfunction, is associated with an incidence, mortality and cost similar to acute coronary syndromes. 1. However, sepsis has not until recently received similar recognition or funding. Evidence suggests golden hours during which aggressive therapy can prevent progression to intractable organ failure and death. 2. Guidelines now provide evidence-based strategies to expedite a continuum of care from the Emergency Room, to the ward, to the ICU. 3. As such, the next decade holds great potential. Conceptually this is similar to the chain-of-survival recommended for cardiac disease. 4. Early goal directed therapy, early antibiotic administration, tight glycemic control, low dose corticosteroids, consideration of recombinant protein C, and consideration of early transfer to ICU are all likely to improve mortality. 5. However, evidence also suggests that a significant care gap exists: namely a difference between best care and usual care, between what is known and what is done. This discussion will present the provocative evidence and rationales regarding how best to treat severe sepsis and septic shock.
