Emergency transportation available to Wawa A drop in service is also available for women who wish to access support, information, or referrals. Children services include: providing programs and activities that meet the safety, emotional and educational needs of the children. Outreach/transitional support services, available weekly in Chapleau for any woman who requires information or support. Whether they are in an abusive relationship or not available to women who do not wish to or cannot come to CHADWIC Home. It is not necessary to reside at the shelter to access the services. ADDICTION Provides housing opportunities and support services to assist individuals with a history of serious and persistent mental illness to secure and maintain permanent, safe, affordable and high standard housing -The program services the residents of Chapleau, Sultan, Pineal Lake, Island Lake, Foleyet, and Gogama. After hours crisis intervention is available by Turning Point staff at the Chapleau General Hospital weekly from 4:30 p.m. to midnight, and from noon to midnight on weekends and statutory holidays. Child Find works closely with law enforcement agencies, social service agencies, lawyers and corporate partners on behalf of missing children and their families. Child Find Ontario has been endorsed by the Ontario Association of Chiefs of Police and is recognized by the RCMP/National Missing Children Services. The following services are provided free of charge by Child Find Ontario: search and Recovery Programs such as ongoing communication and support provided to the families, qualified case managers who assist investigating officers and families, national, confidential toll free line 24 hours a day, 7 days a week and so on. CCN serves as the central point for information about programs and service offered by Ministry of Children and Youth Services (MCYS) funded children s services and other programs for children and families available in the communities. This program is funded by the Ministry of Children and Youth Services and administered by the Hôpital régional de Sudbury Regional Hospital. THERE IS NO COST FOR THE SERVICE TO PARENTS. Services are provided within the home and include: Evaluating the child s development and family needs; Setting goals and introducing activities to encourage the development of the child s physical, social, and intellectual skills; Supporting the family and providing guidance regarding child development and parenting skills; Assisting the families in accessing community services. Every day, professional counsellors provide immediate, caring support to young people in urban and rural communities across the country. The service is completely anonymous and confidential we don t trace calls, we don t use call display. You don t even have to tell us your name if you don t want to. Family Support Worker Program is offered to the majority of our catchment communities. The main focus is to strengthen the family unit. Recreation Bursaries provide recreation subsidies for children from low-income families to participate in activities such as sports, school activities and clubs. Social Housing Providing clean, safe and affordable housing to residents requiring rent subsidized housing. Healthy Communities Fund If you are a low income individual threatened by homelessness, you can apply to the MSDSB to help you retain your current accommodation or assist you in moving off the streets into emergency or permanent housing. Our mission is to provide and advocate for a continuum of mental health services for children and their families with a focus on treatment, prevention and promotion. Provides assistance to children and families who are faced with social, emotional, or behavioural struggles they cannot resolve themselves. 12

The Occupational therapist assesses a child s level of function and independence in the following areas: -Fine motor skills (use of hands and arms) -Visual-perceptual skills -Printing and cursive writing -Play skills -Cognitive skills -Sensory integration -Self-care skills: dressing, bathing, feeding, toileting) Special adapted equipment, treatment programs and splints may be recommended as required. The physiotherapist assesses the child s gross motor and mobility skills. Movements of individual joints and muscles, as well as overall movement patterns are observed including balance, strength and coordination. Equipment needs, that may optimize function, are addressed as appropriate. The speech language pathologist assesses and treats communication and swallowing. This may include speech sound production, voice, stuttering, language and augmentative communication The assistive Communication Clinic assesses children who need alternative methods of communication such as a communication book and / or voice output device to supplement their speech. The clinic team also assesses children who need a word processor (i.e. computer system), adaptive hardware and /or software to support their written output. 13

What does this program do? - Provides parents with advice and support about pregnancy and parenting. - Helps parents provide young children with a healthy start in life. - Links families with helpful support services in the community. How does it work? - A public health nurse and family home visitor work closely to meet your need. A public health nurse will: - answer your pregnancy or parenting questions - put you in touch with community resources - work with a family home visitor to identify what you want to learn - monitor your child for healthy growth and development - offer breastfeeding support A family home visitor is an experienced parent who will: - answer your questions about pregnancy and caring for children - give you tips on parenting and your child s development - provide extra support and encouragement during difficult times - put you in touch with other community agencies that can help you How Do I sign up? - Call your local office of the Sudbury & District Health Unit. Personalized information and support to individuals with learning disabilities. Resource library of LD kids for parents and teachers, literature and advocacy supports available from our office. Advice on assistive technology available to individuals with LD. Extensive and current collection of books and videos on learning disabilities offered in partnership with the Greater Sudbury Public Library check out the Learning Disability Section at the MacKenzie Street. This lab has two computers loaded with the latest assistive 14

15 technology software to assist individuals with learning disabilities. These computers will operate Kurzweil Read and Scan, Dragon Naturally Speaking and Inspiration Software. Information workshops, presentation and our Speaker Series with special guests who provide informative and interesting dialogues. Quarterly newsletter Working Together for Rising Stars and information bulletins. Representation on School Board Special Education Advisory Committees (SEAC) for Chapleau. Will send out information packages, have phone consultations, and direct clients to accessible resources through the library systems or internet. Clients may attend our office as well.
